In the role of an Social Work Care Manager, you will partner with the Registered Nurse and are a critical resource for our members as you are responsible for obtaining needed medical, social, educational, psychosocial, financial, and other services, irrespective of whether the needed services are covered Elderwood IPA.
Direct telephonic and in-person engagement with individuals with chronic medical illness or mental health issues and their families.
Utilize assessment tools to determine the needs of our member population
Link members to resources in the community outside of the scope of services MLTC provides to support social determinants of health.
Interacts continuously with members, family, physician(s), and other resources to determine appropriate behavioral action needed to address/support medical needs.
Reviews benefits options, research community resources, coordinates services, trains/creates behavioral routines and enables members to be active participants in their own healthcare.
Documents in the EMR each encounter or routine contact with members, providers or caregivers and reports changes in conditions to the RN Care Manager, which may require interventions.
Participate in Interdisciplinary Team meetings, Disease Management, Utilization Management, and Quality Improvement activities and prepare/submit activity reports.
Provide coaching and education to members and their caregivers on health care related issues
Provide discharge planning and continuous care management across care settings and coordinate the transition planning
Participate in shared on-call, after-hours coverage along with other Elderwood staff.
Qualifications
Master's degree in social work (Required)
Minimum of one (1) year Social Work experience with at risk populations in home care, case management, discharge planning or managed care.
Minimum of one (1) year experience working with a frail or elderly population and/or members with chronic illness or behavioral health issues.
Previous Managed Care experience, within a MLTC or working with special needs populations preferred.
Must have a valid NYS Driver's license, a safe driving record and dependable transportation
Must be in good standing with the Medicare and Medicaid programs.
Must be free of communicable disease.
Bilingual skills (preferred)

At each duty location, the CRC will be responsible for supporting ERO Field office and facility staff in their daily duties. Responsibilities
Assist with needs, requests (on ICE Detainee Request Form or otherwise), and complaints from the detained noncitizen population regarding non-officer or immigration case related issues.
Assist ERO officers with addressing noncitizen requests related to parent/child communication and/or child welfare involvement.
Facilitate on-site support of disability accommodations, including coordinating follow-up, as requested by ERO, with detention facilities upon submission of a Disability Accommodation Notification to ERO Headquarters (HQ).
Assist ERO and detention facility staff as a resource to facilitate the compliance of applicable agency policies and detention standards. Facilitate discussions and communicate directly with detained noncitizens. Areas of facilitation include but are not limited to the following topic areas: allegations of sexual abuse and assault, identification and monitoring of pregnant noncitizens, resources for noncitizens in segregated housing or special management units, and admission and release.
Address ERO and detention facility staff questions or concerns about specific noncitizens, including available services.
Provide noncitizen referrals to appropriate ERO and detention facility staff for care and follow-up to prevent critical incidents and issues (e.g., medical, mental health, chaplain, etc.).
Support noncitizens with specialized needs or requiring heightened attention and conduct noncitizen wellness checks and follow-ups upon request by ERO Field Office and/or ERO HQ.
Meet with newly detained noncitizens during or after intake to answer questions, address concerns, provide brief orientation to the detention facility and CRC services.
Assist ERO Field Offices who run specialized housing units and programs or services that are designed to provide appropriate care to vulnerable populations, such as: a. Provide language interpretation for facility and ERO staff communicating with the detained noncitizen population.

Assist ERO Field Office in removal planning for noncitizens with vulnerabilities and in general population by - researching referrals and identifying to resources in the noncitizen's home country and developing individualized Departure Plans.
Assist ERO Field Office in release planning for noncitizens with vulnerabilities and in general population by researching referrals and identifying needed resources in the U.S. Planning activities may include communicating with family members, securing resources such as medical/mental healthcare, and identifying housing options or placement in a shelter or hospital/institution.
Develop and maintain a national resource database of resources (in and outside of the United States) for CRC and ERO Officer use to facilitate the release and removal of noncitizens booked out of ICE custody.

You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes.
