Provide in-home services to at-risk families or individuals as determined by referral. This would include a hybrid caseload ranging from low to high-risk referrals. Provide case management services - conduct home visits, complete family assessments, create service plans, manage crisis intervention, advocate, connect to resources, and keep case files according to program standards. Coordinate with school personnel, DHS social workers, and service providers as needed to communicate progress and work toward successfully connecting families to resources. Safeguard the confidentiality of client families. Collaborate with FEC Coordinator to complete ASQ Assessments and any needed referrals based on ASQ results. Promptly report all conflicts among client families, or serious incidents involving clients and families within 24 hours of the incident. Provide services and support outside of traditional working hours to accommodate the family's needs when necessary. Be available on-call 24/7 for client emergencies (occasional). Regularly attend and actively participate in staff meetings, supervision, staff training sessions, case reviews, etc. Perform other duties as assigned. Supervisory Responsibilities: This position supervises support staff (TBD). Minimum Experience, Education, and Qualifications Minimum Education: Master's degree in social work or LSW. Minimum Experience: 1-3 years' experience required. Familiarity with the child welfare system. Trauma-focused training is preferred but not required. Certification/License: A mandated Reporter training certificate is required and may be completed post-hire.
