* Meets with patients in clinic and prepares timely documentation for discussion and referrals on standardized clinic encounter forms
* Refers clients to community resources for financial assistance, assists clients in filling out applications for emergency financial assistance: DEFA (Direct Emergency Financial Assistance)
* Links clients to community resources for basic needs ie., food, clothing and shelter
* Collaborates with agencies in the community who also serve HIV clients
* Counsels clients on end of life issues
* Attends educational trainings as required by Grantees (federal, state and county)
* Performs grant writing for care coordination position
* Consults with HIV patients admitted to the hospital and provides information and resources pertaining to HIV/AIDS with complete assessment documented in the history and physical.
* Refers to inpatient socialworker for discharge planning

Provide and document patient/family counseling services.
Assess patient's circumstances to provide data on strengths and weaknesses of their social, emotional, functional and environmental situations and assist other disciplines in the same as a consultant and educator.
Assess and evaluate psychosocial and financial parameters for those "high risk" patients identified by but not limited to:
Patient displays self-destructive behaviors.
Patient has a documented history of psychological problems or developmental disability and may need long term care for services.
Patient is a risk for HIV/AIDS and has a need for follow-up services/counseling.
Patient has no financial base or health coverage.
Patient is indigent and needs medications, equipment or transportation on discharge.
Patient cannot communicate and there is no one to provide information or participate in planning.

Patient desires assistance for adoption services.
Assist in dealing with and understanding changes associated with illness. Intervene with patient and/or family members focusing on dealing with personal problems aggravated by illness and possibly death.
Provide patients and families information about community resources, Advance Directives, illness adjustment and coping strategies.
Provide information and referral services as appropriate and assist patients and family members in developing a comprehensive plan for post-hospital care for psychosocial and financial issues.
Network with community agencies. Maintain up-to-date community resource file/cabinet to facilitate patients' linkage with appropriate resources. Keep Case Managers informed about available resources.
Provide educational inservices on topics of interest, to Johnson Memorial Health (JMH) employees and community individuals.
Educate staff about psychosocial issues and how to deal effectively and sensitively with patients and families.
Collaborate with Case Managers and others on the interdisciplinary team.
Participate in interdisciplinary care conferences.
Conduct all activities in compliance with applicable laws, regulations, standards, and JMH policies and procedures including Blood and Body Substance Precautions.
Perform additional duties not addressed here as assigned.
EDUCATION, EXPERIENCE AND TRAINING:
Bachelor's degree inSocial Work and current Indiana licensure as a SocialWorker (LBSW) required.

What You'll Do
* Conduct comprehensive psychosocial assessments to identify patient needs, including emotional, financial, and environmental factors
* Complete initial social work evaluations and ongoing reassessments throughout the episode of care
* Communicate significant findings, changes, or concerns to supervisors, physicians, and care team members
* Participate in interdisciplinary care planning to promote continuity, quality, and safe discharge planning
* Develop, implement, and update individualized Plans of Care in collaboration with the care team and physician
* Connect patients and families to appropriate community resources and support services
* Educate and counsel patients and families to support care goals and social work-related needs
* Document all assessments, interventions, and outcomes in accordance with regulatory and agency standards
* Report unsafe conditions or concerns to leadership in a timely manner
What You'll Need
* Master's or Doctoral degree inSocial Work from a CSWE-accredited program
* At least 1 year of social work experience in a healthcare setting
* Current state licensure as a SocialWorker (LSW)
* Strong verbal and written communication skills
* Ability to work independently while collaborating with an interdisciplinary team
* Willingness to travel up to 50% within the assigned territory
* Physical ability to sit, stand, bend, lift, and move intermittently, including lifting up to 50-100 lbs
* Valid driver's license, auto insurance, and reliable transportation
