Essential Education and Experience: Masters of Social Work from an accredited school of social work and holds a current license of Social Work in Texas. Minimum one (1) year experience as a social worker in long term care or medical surgical/acute care setting. One (1) year of hospice experience is preferred but not required. Responsibilities & Duties: The LMSW will be responsible for: Adhering to all regulatory requirements and professional standards Assessing the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicating findings to the RN Case Manager and other members of staff including the interdisciplinary group. Assessing the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, to include funeral arrangements if needed. Supervising the implementation of an individualized social work plan of care and notified Bereavement Coordinator of bereavement assessment needs. Participating in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses and symptoms of the terminal illness. Providing consultation, education, and support to the interdisciplinary team on social work needs. Following policies and procedures consistent with recognized standards of professional practice Providing high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores Collaborating with your Clinical Director and entire team to provide a "concierge" level hospice experience Qualifications & Skills: The best candidate for this opportunity demonstrates mastery of all facets of hospice care and displays a communication style that makes everyone who works with them want to do their best.
