Day to Day Responsibilities of this Position and Description of Project:
Managing a case load for healthcare members with LTSS (Long Term Support/Services) needs.
Monthly and quarterly member contact and will include 80% travel. Remote role. Will require a driver's license.

Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development.
Monitor delivery of services and follow-up with members, caregivers, or provider s through in person visits and telephonic contact
Authorize and coordinate referral for services.
Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care.
Assist in coordinating the development of informal or voluntary services to integrate into the member care plan Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-term care services!
Assist member with filing and resolving complaints and appeals.
Licensed Clinical SocialWorker/Registered Nurse License and 2+ years of physical health care management experience, 4-6+ years of physical health care management experience preferred. Hospital CM, Home health, discharge planning, or long-term care experience preferred.

Position Specific Information
This position is public facing and may be located between 3205 Freedom Drive, Charlotte, NC 28208 and between partner locations in which people seeking shelter may attend. Standard hours of Sunday - Thursday from 8:00 am to 5:00 pm. Subject to change, based on need.
Mecklenburg County is the Lead Agency for Coordinated Entry (CE) for the Charlotte-Mecklenburg Continuum of Care. Coordinated Entry is the intake for the local homeless response system. It is part of the process to access homeless resources and shelter in Charlotte-Mecklenburg.
This SocialWorker I position conducts Coordinated Entry (both in person and telephonically) with individuals and families experiencing homelessness, or those at imminent risk of homelessness, to efficiently connect them to the best available (scarce) homeless shelter, temporary housing, or homeless prevention options in the community. It may require the provision of short-term Systems Navigation support to clients until which time they are connected to a homeless services case manager and/or their situation resolves.
This position also engages clients within a strengths-based, solution focused framework to identify their service needs and goals, and to facilitate referrals to community resources so that clients have a plan of action toward enhanced self-determination.
CE clientele is diverse and brings with them a wide range of experiences and concerns; and may be in different stages of mental health, substance use, and trauma recovery.
In addition to clients, the SocialWorker I requires the ability to effectively engage with community partners to provide on-site CE to clients wherever they may be (in places not meant for human habitation, hospitals, treatment centers, day centers, shelters, etc.)
Position Summary
Provides community-based services to clients, including evaluation, case management, counseling, and referrals. This position, under direct supervision, is responsible for determining the individual's specific problems and assisting in developing an action plan and/or mutual responsibility agreement.
Essential Functions
Assess needs of individuals and families
Interview clients, obtains family, social, and medical information
Determine clients' eligibility for financial assistance and if referrals are appropriate
Provide counseling and services in both individual and group settings
Discuss goals, expected outcomes and means with which to meet goals and expected outcomes
Assist clients in the completion of required paperwork for items that may determine program eligibility, referrals, or provide job assistance, such as job applications
Manage records and maintains history records and prepare reports of services provided
Refer clients to community resources for services and coordinates benefits and referrals to other agencies
Develop and evaluate service plans in consultation with clients, and perform follow-ups assessing the quantity and quality of services provided
Help clients work with government agencies to apply for and receive benefits
Transport clients to appointments such as medical, job-related, or others, if appropriate
Manage records and documentation of services provided and participate in team case conferences
Act as a liaison with other staff with referrals and transitioning of clients
May be required to staff shelters in the event of a disaster
Minimum Qualifications
Experience:
Bachelors of Social Work from an accredited school of social work and one year of social work experience; or a qualifying internship; or successfully completion of the NC Child Welfare Collaborative program; or a bachelor's degree in a related field with 15 hours of social work/counseling courses and two years of social work experience; or Master's degree from an accredited school of social work; or a master's degree in a related field with one year of social work experience.

Engages the family in planning for safety and well-being, encouraging immediate action and front-loading services. * Invokes court action when children cannot be safe in their homes, seeking direction through legal consultation, and completion/filing of all necessary court paperwork. Accompanies law enforcement in taking physical custody of child(ren). Collaborates with internal licensing/placement experts to identify and locate placement that best meets the child's needs and physically delivers the child to the caretaker.
* Participates in forensic joint interviews at the local Child Advocacy Center on appropriate cases (with law enforcement and any other appropriate professionals).
* Participates weekly in unit/team staffing, presenting completed case assessments for unit/team decision-making. Solicits guidance, direction, and support from the supervisor individually at any point in case and/or requests assistance in completing tasks, if needed.
* Documents all activities, contacts, dates, and times on a structured documentation tool in NC Fast. Requests necessary supporting documentation from external sources (i.e., medical/hospital records, police reports, 911 response sheet) and places such in the case file.
* Participates in local, professionally-enhancing, and state-mandated trainings. Participates in monthly MDT meetings to discuss specific cases in a multi-disciplinary setting. Participates in quarterly internal meetings with all fellow assessors to discuss policy changes, service gaps, equipment needs, process instruction, etc.
* Participates in a secondary backup system for after-hours on call, covering a one-day slot during periods of illness, vacancy, or emergency by regular after-hours staff. Must be available to work mandatory emergency shelters and perform other duties as assigned.

ROLE OVERVIEW:
SocialWorkers at KIPP North Carolina provide crucial support and services to students. In this role, socialworkers will work directly with students, teachers, community partners and families to provide individual and group counseling, as well as link families to long-term solutions and external support organizations. SocialWorkers report to their school Principals and are coached by the Director of School Social Work on the Regional Support Team.

In this role, you will be responsible for offering counseling, crisis intervention, and therapeutic services to help first responders manage stress, trauma, and emotional challenges inherent in their demanding work. Key Responsibilities:
Individual & Group Counseling:
Provide individual therapy, group counseling, and psychoeducation to police officers, firefighters, and other first responders. Address issues such as post-traumatic stress, anxiety, depression, substance abuse, and relationship challenges.
Crisis Intervention:
Offer immediate support and intervention during mental health crises, including emergency response and debriefing sessions following traumatic incidents.
Trauma-Informed Care:
Use evidence-based, trauma-informed practices to assess and treat the psychological impact of exposure to critical incidents, accidents, and high-stress environments.
Collaboration:
Work as part of a multidisciplinary team of clinicians, peer support specialists, and administrators to provide holistic care for first responders. Attend team meetings, share insights, and contribute to case management as needed.
Education & Training:
Provide psychoeducation and training to first responders on stress management, resilience, mental health awareness, and coping strategies. Lead workshops on maintaining mental health, managing burnout, and building emotional resilience.
Resource Coordination:
Help connect first responders to community resources, including mental health programs, substance use treatment services, and wellness initiatives.
Confidentiality & Documentation:
Maintain accurate and timely records of all sessions and interventions, adhering to HIPAA standards and ensuring the confidentiality of all clients.
Wellness Program Support:
Participate in wellness programs aimed at supporting the emotional and mental health of first responders, including check-ins, retreats, and other prevention-focused initiatives.
Qualifications:
Masters degree in Social Work (MSW) from an accredited program.
Current, active Licensed Clinical SocialWorker (LCSW) license in the state of North Carolina
Valid drivers license and reliable transportation to visit field locations as necessary.
At least 2 years of clinical experience in mental health counseling, including trauma-informed care and crisis intervention.
Experience working with first responders or other high-risk, high-stress professions is highly preferred.
Familiarity with the unique challenges faced by law enforcement and fire service personnel, including PTSD, stress management, and burnout.
Skills & Competencies:
Strong understanding of trauma and its psychological impact, particularly in high-risk environments.
