The Clinical Care Team will receive referrals from primary care staff and will work collaboratively to carry out the following responsibilities: Perform Case Management Activities, Including: Connecting patients to relevant social service resources such as housing, clothing, food, and mental health services.
Assisting with securing necessary medical equipment.
Following up on patient care plans to ensure continuity of care.
Maintain an up-to-date directory of community resources for each county, including eligibility criteria, referral processes, and contact information.
Support patient education by providing referrals or identifying appropriate educational materials and resources.
Conduct in-person or virtual social needs screenings and assessments for primary care patients referred by providers.
Document all activities and communications in the patients medical record, coordinating with providers and clinical staff as needed.

Job Requirements:
Education and Experience:

REQUIRED EXPERIENCE AND SKILLS:

We are not a telehealth pop up and do encourage team members to be involved in the communities they live in to encourage mental health access and quality of care in underserved areas.
Therapists are responsible for providing all behavioral health services to a clinically diverse, "working-well" population & their families/significant others while having cultural lens and affirming stance. Direct service responsibilities include individual/family therapy, diagnostic assessments, treatment planning, groups & evidenced-based skills training for primary care clients.
Staff will participate in weekly group consultations, trainings, staff development & have the opportunity to join our interdisciplinary team and support local outreach & programming for the guests in our local communities.
Candidates must already have experience using video/virtual platforms, have high threshhold for using IT/Technology, have experience with using multiple softwares and electronic devices in their current day to day. WCC is IT & Technology heavy, future minded and integrates various exciting software platforms and though we provide trainings, candidates must have a strong ability to integrate technology in their work.
Caseload building timeline varies by State. Onboarding takes 1-2 weeks, Credentialing 2-3 weeks, case load building 2-3 months+ depending on the State. Candidates looking for a stable long term career group to grow within are encouraged to apply. Candidates looking for a quick and fast route to easy side money or a practice that only takes highest paying insurance should look elsewhere. Our payer source is broad and moderate as we see EAPs and Commercial payers while focusing on professional development and quality care.
