This is a day shift, Monday - Friday position covering the hours of 8:00am - 4:00pm or 9:00am - 5:00pm with rotating weekend/holiday coverage at NewYork-Presbyterian/Westchester Hospital in Bronxville; right across from the Metro North Station.

Requirements include:
Must have Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).

Previous experience in providing social services to dialysis patients preferred.

Under the supervision of the School Principal the SocialWorker is responsible for:
Individual and Group Counseling with students.

Coordinates donations for families during the Holidays.
Conduct psychosocial assessments and develop treatment plans.

Qualifications

RESPONSIBILITIES

Location: Local PD Contract within Fairfield County, CT - Bridgeport
Hours: Monday, Wednesday, Friday: 12:00 - 8:00 pm; Tuesday and Thursday 9:00 - 5:00 p.m.
CORE FUNCTIONS
· Provides support to RNP's Law Enforcement Partners and ensures access to community based behavioral health services for the community they serve. Assists in identifying and resolving barriers to care.
· Conducts and documents evaluations in the field in collaboration with RNP's Law Enforcement Partners. Works collaboratively with RNP's Law Enforcement Partners when mental health crisis calls arise.
· Assess and record medical, psychological, and social factors contributing to the individual's unique circumstances, including an assessment of strengths, needs, abilities, and preferences.
· Refer individuals to appropriate medical, psychiatric, housing and/or behavioral health services utilizing designated collaborating organizations (DCOs) whenever possible to ensure immediate admission into programs.
· Work closely with colleagues and community-based providers to ensure clients and their families receive access to timely, high-quality care and services, including follow-up to ensure connection. Serve as a client advocate in obtaining necessary services.
· Document all contacts; work collaboratively with the clinical team to assist in identifying and placing clients in appropriate aftercare services that offer interventions tailored to their unique needs and circumstances.
· Participates in interdisciplinary continuous quality improvement activities on an ongoing basis; participates in weekly clinical meetings and individual supervision. Maintain detailed records of program goals and objectives.
· Attend forums and meetings to advance the CRI's services that are provided by RNP.
· Adhere to all our Law Enforcement Partners guidelines and policies regarding behavior, staff interactions, boundaries and communicates through the proper chain of command within the respective Law Enforcement agency.
· Adheres to all documentation and reporting requirements per RNP and Law Enforcement policy.
· Assumes all other appropriate duties as deemed necessary by the supervisor.

· Master's degree in the behavioral sciences, with license or license eligible. MSW with (2) years' experience preferred or equivalent combination of education and experience while working on LCSW.

· Knowledge of the physical, psychological, social and economic effects of substance use, mental health issues and trauma.
· Crisis intervention skills

· Ability to establish and maintain cooperative professional relationships with excellent communication skills and the ability to project a professional image.
· Must be able to pass a background check to obtain clearance to enter Law Enforcement Agencies. This background check is in addition to RNP's required checks.
CERTIFICATES, LICENSES
· Master's degree in the behavioral sciences, with license or license eligible. MSW with (2) years' experience preferred or equivalent combination of education and experience while working on LCSW.
· Must possess a valid CT Drivers' License and reliable transportation.
