Key Responsibilities
Clinical & Case Management Services
Conduct comprehensive assessments to identify client needs, strengths, and areas of risk.

Provide direct behavioral health services, including counseling, crisis intervention, and ongoing support.
Monitor, manage, and document client risk, safety concerns, and progress.
Support clients in achieving emotional and social readjustment within community settings.
Coordination & Collaboration
Coordinate care across multiple service providers to ensure continuity of support.
Refer individuals to appropriate community and social service agencies based on identified needs.
Provide consultative support and guidance to state agencies, private organizations, and community partners.
Administrative & Compliance Duties
Maintain accurate and timely documentation in compliance with Department of Health and Human Services (DHHS) standards.
Ensure services support the mission and integrity of the DHHS - Division of Mental Health and Substance Abuse.
Participate in multidisciplinary team meetings, case reviews, and professional development activities.
Reporting Structure
Reports directly to an administrative superior.
Works independently while contributing to the goals and standards of the department.
Required Qualifications
Bachelor's or Master's degree in Social Work or a related behavioral health field (as required by state).
