PACE is not a nursing home. However, an alternative to nursing home placement is allowing Seniors to remain at home. McGregor PACE is seeking a qualified Social Worker (MSW, LSW) to work with an Interdisciplinary team in managing a caseload of 55-62 seniors, alongside team members. McGregor PACE aims to have our participants remain in the community with the support of our Day Health Center services and staff. This candidate must be willing to conduct semi/annual assessments of PACE applicants and care plans. The MSW along with other team members, will be responsible for communicating with community agencies, participants, and families on an ongoing basis. Work Schedule: Monday - Friday (NO WEEKENDS! No Holidays! FIRST, SHIFT ONLY!) Responsibilities: Conducts semi/annual assessments of PACE applicants. Works with nurse partner to ensure the interdisciplinary team has accurate information to assist participant and family members. Conducts psychological testing, i.e., Folstein Mini-Mental Exam, Geriatric Depression Scale Index, and other cognitive tools, if needed. Plans social work interventions; accordingly, views problem resolution from the standpoint of client benefit. Must demonstrate competency in implementing defensive tactics and satisfactorily complete all of the requirements in training of defensive tactics; including passing the recertification. New employees are required to pass Defensive Tactics Training by the end of the introductory period. Must perform tasks, duties, and job responsibilities without restrictions and/or limitations. Knowledge, Qualifications, Skills, and Abilities To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Demonstrable skills in interviewing/counseling; group facilitation; strong communication skills; ability to identify problems in behavioral terminology; working knowledge of adolescent growth and development, varied cultural norms, peer subcultures and socialization techniques. Must be able to work non-traditional shifts and weekend hours based on departmental needs. Education and/or Experience Bachelor's Degree in Social Work, Sociology, Psychology, or Behavioral/Social Sciences with coursework and training successfully completed in counseling, group behavior, child psychology, and human behavior. A related Master's degree is preferred. AND Two (2) years of work experience counseling and/or interacting with adolescents preferably in an institutional setting. Successful completion of a full academic school year internship within the Juvenile Detention Center will count toward the experience requirement. Licensure or Certification Requirements LSW, LISW, LPC, or LPCC preferred.
