POSITION SUMMARY
The Medical Social Worker (MSW) provides medical social services to clients in their homes in accordance with physician orders and under the direction and supervision of the Director of Nursing.
RESPONSIBILITIES
Assists in the admission process of the client to the Agency by performing an initial evaluation, assessing the client's psychosocial status and evaluating the client, family and home to identify socioeconomic and emotional factors that will affect the plan of treatment.
Assists in the development and implementation of the interdisciplinary client care plan as it pertains to medical social work.
Observes, record, and report changes in the client's emotional and social factors that affect the client's illness and his/her need for care, and his/her response to treatment.
Consults with the attending physician concerning the alteration of the plan of treatment.
Maintains and submits written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
Evaluates the client's and family's response to, and effectiveness of, the medical social work intervention.
Confirms, on a weekly basis, the scheduling of visits with the Director of Nursing to facilitate coordination of other staff visits.
Participates in Agency activities and committees when appropriate including staff development activities and in-service education.
Participates in discharge planning.
Assumes responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and individual research and reading.
Assesses the client's potential to cope with his/her social and health problems.
Acts as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the client/s health problems.
Helps clients to utilize the resources of their families and the community.
Provides rehabilitative and supportive casework.
Assists clients and their families in coping with personal and environmental difficulties which might predispose them toward illness or interfere with obtaining maximum benefits from medical care.

**Responsibilities**
Provides social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families. Degree: social work. Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education.

MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: provide individual, group, family, therapy; work with clients to develop strategies to change behavior or cope with difficult situations; and refer clients to other resources or services, develop and adjust treatment plans with the client, physicians, and other healthcare workers.
Make diagnosis or treatment decisions based on understanding of Trauma Informed Care practices.
Participate in workplace relationship with healthcare facilities to coordinate care.
Review and evaluate patient's progress in relation to measurable goals described in treatment and care plans.
Document patient notes in the medical record using an electronic health record systems.

Selective Placement Factor
Must possess a current, unrestricted U.S. State license as a Licensed Independent Social Worker (LISW), a Licensed Clinical Social Worker (LCSW), or a Certified Independent Social Worker (CISW) or a Licensed Master Social Worker (LMSW).
Time In Grade

**Minimum Qualifications**
§ Bachelor's degree in social work from an accredited school and a minimum of 3-5 years palliative care or hospice experience.
§ Must have demonstrated ability to assess emotional needs, as well as social needs of patients and their families.
§ Requires the ability to administer psychological/emotional assessment instrument and define treatment planning from results.
§ Excellent communication and interpersonal skills.
§ Ability to effectively collaborate with interdisciplinary team in providing patient care.
§ Solution driven, creative and resourceful problem solving skills.
§ Ability to function independently and effectively anticipate needs and problems.
§ Skilled in organizing and prioritizing work.
§ Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
§ Must possess professional image, positive attitude, enthusiasm and self-motivation.
§ Comfortable in maintaining professional boundaries.
§ Proficient computer skills.
§ Broad understanding of community resources including those related to financial benefits.
§ Awareness and sensitivity to cultural diversity.
§ Knowledge of self-care, personal boundaries; abilities, limits and inner resources.
§ Current Arizona Driver's license and automobile insurance.
**Preferred Qualifications**
§ Master's degree in Social Work from an accredited school and a minimum of one year social work experience in a health care setting (hospital, home health, hospice, behavioral health.)
§ Previous hospice experience.
§ LCSW or LMSW licensure.
§ Bilingual abilities a plus.
