The primary role of the Medical Social Worker II is to assist patients and families/caregivers to cope with the social/emotional issues and practical arrangements related to the patients illness. Under general direction of the Social Work Manager/LCSW, delivers age-appropriate social work care to patients and their caregivers in accordance with agency policy and procedure and state and federal regulations. The Medical Social Worker II serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings.
Essential Responsibilities:
Provides psychodynamic interventions, crisis intervention, grief/bereavement counseling, problem solving, stress reduction and developing healthy coping strategies in individual/family/group settings. Provide counseling for disease acceptance and understanding.
Responsible for developing and implementing individual Plan of Treatment which assist patients and families to cope and/or restore social, emotional, financial and environmental factors which affect and/or affected by illness.
Completes psychosocial assessments. Partners with patient to identify needs and develop and implement individual treatment plan based on mutually agreed upon treatment plan.
Discuss options for care proactively including Kaiser resources and external community/government resources to assist patient and family in developing short and long term care plans as appropriate.
Team with other disciplines in assessing, planning and providing services for patients utilizing biopsychosocial information.
Assist patient in advocating for self to receive appropriate services within Kaiser and in the community.
Assist patient and family with care planning and discharge plans.
Takes, reviews, evaluates and prioritizes written and oral referrals.
Maintains documentation, records and data collections.
Responsible for completion of required documents in a complete and timely manner.
Functions as part of the Skilled Nursing Facility Team to assure appro priate, timely placement of Kaiser members in nursing facilities.
Liaison between patient and Kaiser maintaining positive relationship with Kaiser and providing for continuity of care.
Identifies appropriate levels of care and facilities for referred patients, were applicable.
Obtains placements, where applicable.
Collaborate with internal and external resources in Kaiser and the community to meet mutually agreed upon goals and objectives.
Provides information and referral to community resources as requested.
Coordinates exchange of information between Kaiser, families, members and skilled nursing facilities.
Determines application of Kaiser, Medicare and Medi-Cal benefits to specific patient situations.
Participates in Utilization Management/Quality Assurance activities.
Assist in coordinating communication between regional offices, clinics, hospitals, and field staff, triaging of phone calls from members/families.
Works with referral sources to clarify and complete required clinical and psychosocial information.
Perform other related duties as necessary.

Step I: 0 - 2 years social work experience.
Step II: 2 - 4 years social work experience within the last five (5) years.
Step III: 4 or more years social work experience within the last ten (10) years.

Job Description
Under general supervision, Protective Service Workers perform professional investigation, case management, and casework services at the, Family & Children's Services program. This includes activities related to: engagement of children, youth and families; assessment of safety and risk; case planning and service delivery; and transitioning children and families to appropriate permanent and stable living situations.

Qualifications
1.
Education
a. Possession of a Master's Degree in Social Work from a school accredited by the Council on Social Work education (CSWE);
OR
b. Possession of a Master's Degree from a two-year (2) counseling program from an accredited college or university. (Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum).

a. A passing score must be achieved on the written multiple choice exam in order to continue in the selection process. Candidates who pass the exam will be placed on the eligible list in rank order according to their written examination score.
Score Banking
:
Scores attained on the Protective Services Worker Exam for Family and Children Services will be 'banked' for two years, starting from the date of the examination. This means that, during this two-year time period, candidates need not take this test again. However, after one year, a candidate has the option to either (a) keep the same score and apply their test score to the other announcement or (b) re-take the test. Re-testing is permitted no sooner than one year from the date of the test. If a candidate opts to re-test, the re-test score becomes the candidate's official score since it is the most recent.
If a significant modification is made to this exam in the future, the application of “banked” scores may no longer be appropriate. In such a rare circumstance, candidates with “banked” scores would be required to re-take the exam sooner than the two-year period specified above should they apply to another job opportunity where the modified exam is used.

Program and Role Summary
Wayfinder's Contra Costa Psycho Social Assessment program assesses resource families, including adoptive families, by completing resource family evaluations within county timeframes.
The primary focus on the Social Worker, is to conduct resource family assessments for relative and non-relative families; and write the assessment in the county contract's allotted time. Psychosocial Assessment (PSA) Social Workers may also assist with finalizing the adoption for these families.

The Social Worker will assist SMMC patients assigned to the Ron Robinson Senior Clinic (RRSCC), serving older adults address the social/medical barriers that impact their health. RRSCC's patient population consists of individuals, predominantly 60+ year old with: moderate to severe medical conditions; have mental health diagnosis'; are high utilizers of services; those who need support at home; may require out-of-home placement, and who may need end of life care. The Social Worker will provide intensive case management, linkages to supportive services, and will collaborate with County and community partners to ensure a patient receives patient-centered care and timely services.
The Social Worker will make as needed home visit to determine medical needs and in providing case management duties. The Social Worker will travel throughout the County as needed, to conduct home visits, attend meetings, and perform other duties of the position. The Social Worker will conduct intake and on-going case management activities, coupled with warm-hand offs in the clinic. Some of those activities include:
* Complete necessary documents, paperwork, and work within different information technology systems, like EPIC Systems, Avatar, and Q software.
* Conduct as needed monthly follow up with patients.
* Provide education, outreach, and community resources information to patients, family members and caregivers.
* Participate in quality improvement efforts.
* Engage in interdisciplinary team meetings or other activities to support San Mateo County's pilots.
The ideal candidate will possess the following:
* Master's degree in Social Work or Clinical Psychology.
* Two years+ of experience working with aging adults, palliative care, hospice, or related scope of work preferred.
* Fluency in Spanish preferred.
* Ability to independently develop and implement sound case plans.
* Experience balancing client-centered case coordination and the accompanying administrative documentation.
* Superior organizational skills to manage an active high-volume caseload and related documents.
* Proficient computer skills to use several independent data base systems.
* Excellent oral communication and writing/documentation skills.
* Ability to coordinate and facilitate case conference meetings with various community partners.

Duties may include, but are not limited to, the following:
* Provide intensive, highly skilled casework services with patients or clients and their relatives.

* Provide therapeutic and/or protective services to patients, clients, and community groups in individual or group situations.
* Interpret programs and consult with individuals and various community groups to provide a better understanding of social aspects of environmental, emotional and physical problems and appropriate methods of alleviating them.
* Assist in the development of training for other staff.
* Interpret the rules, regulations and policies of the department to clients and applicants.
* May serve as lead supervisor.
* Prepare and maintain casework records, reports, and relevant correspondence.
* Perform related duties as assigned.
Licensure/Certification:
* California class C driver license or equivalent.
Education:
* Satisfactory completion of 36 semester units of social welfare and/or appropriate behavioral science courses
Experience:
* Three years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.
* Substitution: Possession of a Master's Degree in Social Work from an accredited school of Social Work and two years' experience performing duties comparable to those of a Social Worker II or higher in San Mateo County.
Knowledge of:
* Theories of human behavior and their practical applications.
* Psycho-social, medical, and economic interrelationships that result in individual and social abnormalities.
* Philosophy and function of social work systems.
* Appropriate utilization of a variety of social resources.

Reporting to the Social Work Manager, the Social Worker maintains a caseload of participants working in close coordination with the IDT. In addition to delivering standard social work services such as psychosocial assessments, care planning, counseling and case management, the Social Worker functions as a liaison between the participants, family members, support network and care team, as appropriate.
Essential Job Duties:
Conduct Social Work assessments to determine the psychosocial needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans
Deliver and document social work interventions as agreed upon in the participants' care plans including but not limited to arranging necessary resources and services, assisting with care transitions, providing individual as well as group counseling and case management
Work with the primary care physician and other members of the care team to guide smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.)
Initiate, coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and other people within the participants' support network (family, informal caregivers etc.)
Provide discharge planning when participants disenroll from the program
