A Brief Overview

What you will do
Interviews patient and family members to obtain information about home environment, family systems, health history, and patient's personality traits.
Evaluates data gathered in terms of the medical plan of treatment, available social service programs, financial counseling and plan a pertinent therapy program that will provide maximum benefits and safety for each patient.
Assists patients and families to understand, accept, and follow medical recommendations with emphasis on assisting patient to be medically compliant with care plan.
Performs a variety of services such as advising on social problems, arranging for discharge or post-operative care at home, assisting patients/families needing placement into assisted living or nursing homes, arranges for transportation to appointments and treatments, and works to alleviate anxiety and fears of patients and family members.

Maintains department policies, procedures, objectives, and quality assurance program, safety, infection and environmental control.

Conducts individual, marital, family and group therapies as necessary. Coordinate care conferences as needed. Serves as the social work representative in multi-disciplinary team approach to patient care.
Utilizes research findings to improve outcomes and support, and provide quality of life for patients and their families. Works collaboratively with Case Management in providing coordinated services, which facilitates continuity of care for our patients.
Works with emotional problems including but not limited to assisting patients, residents and families with anxieties and stress caused by illness, admission to hospital/nursing home, crisis situations, difficulties in coping with physical disabilities, fears related to helplessness and death, the need for nursing home/assisted living/specialized care, and providing educational materials.

Attends child support hearings at the Tribal Court when requested; may be required to appear as a witness for the tribal Staff Attorney or presenting officer in child support cases
Notifies all parties concerned regarding continuances, cancellations and rescheduling of court hearings by phone, mail or personal service

Required Qualifications for SW III
Master's degree with a major concentration in social work
6 months-1 year of clinical social work experience

Social work experience in a health care setting with the appropriate patient population and their families.

Experience in a fast paced, high volume work environment

ESSENTIAL JOB FUNCTIONS:
Conduct Special Education Evaluations in the area of behavior.
Conduct Functional Behavioral Assessments and develop/consult in the development of Behavioral Intervention Plans and Positive Behavioral Support Plans.
Model and coach school personnel on implementation of behavior plans.
Support school personnel through modeling and coaching students on utilization of skills taught and written in the behavior implementation plan.
Facilitate the PTR process and/or other department specific processes in identification of function of behavior with school staff.
Provide direct services (individual, group) to students as needed.
Participate in identification and consultation on the development of Tier 2 and 3 intervention plans, data collection and parent meetings for at-risk students with challenging behaviors.
Participate in monitoring student progress to address student and teacher needs for students in both tiered intervention and those currently on an IEP.
Provide liaison services with community, AEA and school resources in the area of behavior.
Participate in and attend IEP meetings for students with significant social/emotional/mental health or behavioral needs.
Informally use diagnostic skills to provide information in the development of behavior/support plans for students and link them to appropriate services.
Maintain documentation of each student served.

Provide staff development with LEA/AEA for personnel in the areas of Behavior and Mental Health issues/areas.
