With 40-55 full-time social workers on staff, we offer a variety of employment locations for PRN employees, including inpatient, home care, long term care, and assisted living facilities. Position Details Locations: Various settings in and around Duval County Education: Masters of Social Work (MSW) degree required PRN Eligibility: Work a minimum of four shifts per month; up to four shifts available per week for interested employees Compensation: $35.00/hour plus additional paid driving mileage Job Responsibilities:Visits patients across a variety of settings, including but not limited to hospice inpatient units, private homes, long term care or assisted living facilities, group homes, shelters, or other locations where services to patients/families may need to occur. Manages a caseload of patients including, but not limited to, assessing for and assisting with advanced directives, funeral and memorial planning, assessment of care situation, and the provision of more complicated case management.     The Social Worker PRN provides social services and support to patients and their family members. A contributing member to each patient's Interdisciplinary Group (IDG), the social worker provides psychosocial interventions, referrals, assessments, supportive counseling, network referrals, and resource procurement to patients and families. With 40-55 full-time social workers on staff, we offer a variety of employment locations for PRN employees, including inpatient, home care, long term care, and assisted living facilities. Position Details Locations: Various settings in and around Duval County Education: Masters of Social Work (MSW) degree required PRN Eligibility: Work a minimum of four shifts per month; up to four shifts available per week for interested employees Compensation: $30.00/hour plus additional paid driving mileage Job Responsibilities:Visits patients across a variety of settings, including but not limited to hospice inpatient units, private homes, long term care or assisted living facilities, group homes, shelters, or other locations where services to patients/families may need to occur. Manages a caseload of patients including, but not limited to, assessing for and assisting with advanced directives, funeral and memorial planning, assessment of care situation, and the provision of more complicated case management. Assesses for financial needs or barriers, and facilitates referrals as needed to internal and external resources. Provides patient/family education as appropriate and within the scope of practice Identify and utilize appropriate community resources and assesses patient/family/caregiver ability to access them. Evaluate patient/family/caregiver response to intervention(s) when referred to community agency and the satisfaction of the services(s) provided. Provide clinical therapeutic services to patients and families such as, but not limited to: Short-term individual counseling (loss, anticipatory grief and bereavement, coping, transition) to patient and/or family Crisis intervention Assistance in addressing/supporting decision-making, ensuring the best interest and rights of the patient Coordination and collaboration with ancillary therapeutic services, including but not limited to Child Life services and Music Therapy Evaluate, intervene, and support the patient/family/caregivers throughout life transitions, regardless of disease trajectory and location on the continuum of the condition. Participate in IDG meetings as a contributing member of the team in ongoing communication, coordination, and continuity of the patient and family care/service needs from the psychosocial perspective, including the development and revision of the patients POC. Assist the physician and other team members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness. Identify support systems available to reduce stress and facilitate coping with acute and end-of-life care. Educate and evaluate patient/family understanding and acceptance to change in program/level of care; communicate psychosocial information to include goals of care to new care providers, to include hospital or Community Hospice inpatient facility when transfer to a different care setting. Provide services to patient/family/caregivers within the National Association of Social Workers (NASW) Standards of practice in Palliative and End of Life Care. Participate in sharing information with other professionals through participation in organizations such as NASW. Participates in the organization on call schedule as needed Other duties as assigned. Education/Experience: Master's Degree in Social Work (MSW) and three (3) years direct service experience. Experience in supportive counseling in group and individual settings, and/or direct service with clients and their families in health care setting. Experience in provision of case management services. Principles, practices and methods of psychosocial services interventions, case management. individual assessment. and counseling techniques (including but not limited to grief, loss and bereavement)

Major Tasks, Duties, and Responsibilities * Attend clinical case conference meetings and meetings assigned by the Director of Nursing, Clinical Supervisor and/or Administrator. * Appropriately refer to other disciples or community resources as needed. * Complete safety and needs assessments accurately and identify actual or potential problems and solutions to patient problems. * Maintain a working knowledge of available outreach programs. * Respects confidentiality of information in the client clinical record and only shares this information in accordance with Agency policy and HIPAA * Educate patients and families on services and interventions. * Responsible for ensuring that the standards of clinical practice are being followed at all times. * Responsible to report weekly schedule and report if over/under productivity. * Acts in a professional manner, work as scheduled and report to work on time. * Works with the Clinical Supervisor and Director of Nursing to ensure that Medicare guidelines and revisions are being followed. * Follow procedures to work within the frame of the law and scope of practice. * Record the coordination of care from the field staff into a patient's chart. * Report any problems of patients to the Physician and Clinical Supervisor. * Communicate visits with the Clinical Supervisor to establish proper care of the patients. * Prepares social histories to augment existing service, or as a guide in determining or changing level of service. This may include cultural factors, financial concerns and support system * Ensure that all documentation is completed in a timely fashion. * Keeps the Clinical Supervisor, Director of Nursing or Administrator apprised of any problems or potential problems. * Provide in medical social work to homebound patients without prejudice. * Ensure that all documentation is completed in a timely manner within the agency. * Meet with the nurses, therapists, home health aides and other disciplines involved in the patient care for case conference on a weekly basis. * Takes and writes physician orders as necessary and appropriate. * Establishes and maintains an ongoing relationship with the physicians and their office staff, facilities, or other members of the community involved to promote continuity of care. * Respects confidentiality of information in the client clinical record and only shares this information in accordance with Agency policy and HIPAA * Represent the Agency at all times in a professional and loyal manner. * Attend meetings, luncheons and in-services as needed. * Communicate well verbally and in writing. * Have excellent planning, time management and organizational skills * Promote patient and community relations * Participates in the development of the plan of care * Participates in discharge planning and in-service programs * Acts as a consultant to other agency personnel * May perform other duties as assigned.
