JOB DESCRIPTION: CASE MANAGER/SOCIAL WORKER Case Manager/Social Worker Reports to: Case Manager Supervisor We are seeking a compassionate and dedicated individual to join our team as a Case Manager/Social Worker. Educate and empower clients to make informed decisions regarding their health, safety, and well-being. 7. Collaborate with other professionals, including healthcare providers, legal authorities, and community organizations, to develop and implement comprehensive client care plans. 8. Maintain accurate and up-to-date client records and documentation, adhering to ethical and legal requirements. 9. Stay current on social work best practices, policies, and regulations, and participate in professional development opportunities to enhance knowledge and skills. 10. Participate in team meetings, case conferences, and other multidisciplinary discussions to ensure effective client care coordination. 11. Provide crisis intervention services as needed, including conducting risk assessments and developing safety plans. 12. Contribute to program development and quality improvement initiatives to enhance service delivery. 13. Build and maintain positive relationships with clients, their families, and community stakeholders. Requirements: 1. Bachelor's or Master's degree in Social Work or a related field. 2. Valid state licensure/certification as a Social Worker (if applicable). 3. Proven experience in case management and social work, preferably in a healthcare or social services setting. 4. Strong knowledge of social work theories, principles, and best practices. 5. Excellent assessment, problem-solving, and communication skills. 6. Ability to work effectively with diverse populations and individuals from different backgrounds. 7. Proficiency in using electronic health record systems and other relevant software. 8. Ability to work independently and as part of a multidisciplinary team. 9. Demonstrated ability to maintain confidentiality and ethical standards in client care. 10. The Mental Health Residential Rehabilitation Treatment Program (MH-RRTP) provides comprehensive treatment and rehabilitation services to Veterans with a range of Mental Health and Substance Use Disorders (SUD) within a 24/7 structured residential environment. The program takes a whole health, evidence-based approach to address Veteran goals, to include medical and psychosocial needs. Specific Duties include: * Has broad management oversight, to include include administrative and programmatic responsibilities for the MH RRTP. * Establishes policies and procedures that adhere to VHA Handbook 1162: Mental Health Residential Rehabilitation Treatment Program. * Executes ongoing program evaluation and data-driven quality improvement projects to monitor the effectiveness of the MH RRTP programs. * Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including, but not limited to, admissions and discharges, referrals, patient demographics, equipment utilization and patient satisfaction. * Facilitates liaisons with other Mental Health programs in the Mental Health Service, across the continuum of care, with non-clinical services, and across VA and community partners to effectively coordinate MH RRTP services. * Day to day operation and management of the MH RRTP. * Performs the administrative and personnel management function relative to staff supervised. Incumbent establishes guidelines and performance expectations for staff, which are clearly communicated through the formal employee performance management system. Observes workers' performance; demonstrates and conducts work performance critiques. Provides informal feedback and periodically evaluates employee performance. Incumbent is able to resolve informal complaints and grievances. Develops work improvement plans and recommends personnel actions as necessary. Provides advice, counsel and mentoring workers related to clinical and administrative matters. Effects disciplinary measures as appropriate to the authority delegated in this area. Reviews and approves or disapproves leave requests. Incumbent assures that subordinates are trained and fully comply with the provisions of the safety regulations. * Clinical supervision of unlicensed/dependently licensed staff. Release of information forms must be signed by the parent/guardian. • Educates clients and their families regarding their diagnosis, prognosis, treatment, and therapeutic process. • When working in a work site and/or school site, CT should work cooperatively with teachers, guidance counselors, and administrative staff within their assigned buildings including, but not limited to, relaying appropriate information to and from school facilities, assessing needs of additional staff, and coordinating services within the building. • Identifies, recommends, networks and/or utilizes community resources to facilitate the highest possible mental health functioning of all assigned clients. Involvement in home visits may be necessary to assure continuity of clinical care. • Adheres to the agency's personnel policies and procedures, ODMH, Medicaid and Insurance standards, and fulfill documentation and reporting requirements. • Informs work site, school site and/or Minority Behavioral Health Group when ill or unable to attend. • Participates in quality assurance and program evaluation studies. • Attends and participates in scheduled or required training, staff meetings, peer review, workshops, and supervision. • Maintains at least their current or the minimum required Ohio licensure status as a qualified ODMH service provider. The Social Worker performs direct evaluations to determine admission or discharge. Patients are seen via in-person and telemedicine. It is a very fast-paced environment as patients from all entities are seen within one hour of referral time. The ideal candidate will be autonomous, reliable, a critical thinker, have a crisis background, have strong passion for the psychiatric population, and be a self-starter.
