Position: Medical Social Worker

NHC HomeCare Merritt Island is looking for a Medical Social Worker to join our team. The Medical Social Worker supervises and provides for the evaluation of the social and emotional needs of the patient related to their medical condition, and provides counseling, etc. based on this evaluation.

Qualifications - ALL states:

1. Master's Degree from a school of social work accredited by the Council on Social Work Education.
2. One year social work experience in a health care setting is required.

PLUS state-specific requirements:

FLORIDA specific

• a. To provide clinical counseling to patients or caregivers, the social worker must have a current state license as a Licensed Clinical Social Worker (LCSW), pursuant to Chapter 491, F.S.

Position Highlights:

• Provides and accurately documents direct social services to patients planned to restore the patient to optimum social and health adjustment; including:
  •Assesses social and emotional factors, to minimize impact on the patient's health and ability to cope with problems of daily living
  •Helps the patients and caregivers to understand, accept, and follow medical recommendations
  •Assists patients and caregivers with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care; and
  •Identifies resources, such as caregivers and community agencies, to assist the patient to resume life in the community, including discharge planning
  •Assists the providers and home care team members in understanding the significant social and emotional factors related to the health problems and
  •Participates in the development of the plan of care.

• A minimum of a Masters in Social Work, with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.
• Must be a Licensed Social Worker.
• Home care experience a plus.
• Demonstrated ability to read, write, and effectively communicate in English.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases, as assigned.

Social Worker for Medically Fragile Children in Treatment Foster Care Do you have experience in working with individuals with developmental disabilities and health challenges? As a full-time Clinical Coordinator for Medically Fragile Treatment Foster Care youth, you will advance the company's mission of providing quality service by facilitating and providing case management and counseling services.

+ Provide case management to an assigned individual caseload of Medically Fragile treatment foster care children, ages birth to 21.
+ Lead development and coordinator of child's treatment and service plan.

+ Make weekly home visits to each child on caseload .

+ Maintain current and complete records in accordance with corporate, contractual, and regulatory standards.

+ Assist with emergency coverage and crisis intervention support as needed.

+ Assist with legal, licensing, regulatory and certification activities as needed.

Qualifications:

+ LBSW or LMSW

+ Bachelor's or Master's degree in Social Work.

+ Self-motivated and detail-oriented with ability to multi-task.

+ Valid driver's license in good standing with access to a registered vehicle with proof of insurance.
