Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required

The Department's responsibilities include 1) planning and coordination of state/federal investments into programs and services aimed at improving quality of life for senior adults across the region, 2) coordinating services among a network of senior centers and providers, 3) providing counseling/ assessments to help Middle Tennesseans enroll in programs and services, and 4) monitoring the performance of grant-funded programs to ensure compliance with state and federal requirements.
GNRC's CHOICES Intake team helps to screen and assess older adults who may be eligible for the TennCare sponsored CHOICES program funded through Medicaid. CHOICES provides nursing facility services and home and community-based services for adults 21 years of age and older with a physical disability and seniors (age 65 and older).
Responsibilities of this position may include:
Help inform older adults about programs and services available to assist with long-term care, transportation, nutrition, or other needs as appropriate,
Conduct telephone and in-person screenings and assessments with clients across the Middle Tennessee area,
Complete pre-admission evaluations and collect financial and medical history information from potential clients,
Keep informed on aging and disability related issues and gather information about available resources and local service providers,

What You'll Do
As an Arisa Health intern, you may:
Conduct person-centered assessments in behavioral health, education, vocational needs, relationships, and community engagement.
Develop service or treatment plans in collaboration with clients, families, and referral agencies.
Provide activity- and participation-based interventions tailored to client needs.
Connect clients with community resources and support services.
Maintain accurate, confidential documentation in a timely manner.
Participate in team and inter-agency meetings.
Support clients and families in aftercare planning.
Build positive relationships with community partners and guide clients in navigating systems of care.
Requirements
Currently pursuing a Bachelor's or Master's degree in Social Work or a related licensure-track field.
Must be at least 18 years old and able to pass criminal background checks and a drug screening.
