BAYADA Home Health Care is currently seeking an energetic and experienced Licensed MedicalSocialWorker, MSW, for a per diem opportunity performing home health visits our OFFICE. This office services clients on a per visit basis in territories throughout TERRITORY. Prior home care experience preferred, but not required. As a MedicalSocialWorker, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
Responsibilities include:
- Making home visits to clients in designated geographic territories.
- Assist our team in understanding the social and emotional factors related to our clients' health problems.
- Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
- Identify appropriate community resource referrals to address the client's and the family's practical and environmental needs.
- Act as an advocate for both the client and the family to navigate the patient through the community system.
- Participate in the development of the total plan of care with the multidisciplinary team of BAYADA clinicians, as appropriate.
- Develop and maintain contracts with public and private agencies as resources for the patient.

- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided utilizing a state-of-the-art touch pad tablet.
Qualifications for the ideal candidate:
- Must be licensed by the STATE Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
- A minimum of a Masters in Social Work, with at least 2 years of community-based experience.
- Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.
- Home care experience a plus.
- Demonstrated ability to read, write, and effectively communicate in English.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases, as assigned.

I.A.Primary Purpose of Organizational Unit This socialworker position is assigned to the Adult Service Unit, which is part of the Service Division of a local department of social services which provides crisis assistance and on-going supportive services to eligible individuals and their families.
B.Primary Purpose
The socialworker in this position has the responsibility of assisting aged, disabled, and other adults experiencing medical, social, and community problems to identify their problems. The worker strives to enhance the client's problem-solving and coping capacities with the purpose of providing services to enable the individual to live independently in his or her community.
C.Work Schedule
The normal work hours are from 8:00am until 5:00pm. The worker may have to work overtime on some occasions for which he/she will receive compensatory time.
D.Change in Responsibilities or Organizational Relationship
This position now has the added responsibility to investigate Adult Protective Service reports, assess competency of adults and make recommendations regarding the need for a guardian.
Protective Services for Adults is a multi-factored service based on an awareness of the vulnerability of some adults to abuse, neglect, and exploitation. Abuse means willful infliction of physical pain, injury, or mental anguish or unreasonable confinement, or the willful deprivation by a caretaker of services which are necessary to maintain mental and physical health. Neglect refers to a disabled adult who is either living alone or not able to provide for himself or herself the services which are necessary to maintain mental or physical health or is not receiving services from his caretaker. Exploitation means the illegal or improper use of a disabled adult or his resources for another's profit or advantage. The protective service component consists of provisions in the state law for intervention in crisis situations in which adults are reported to be in need of protection. It is the role of the socialworker to ensure that the agency's actions are consistent with statutory and state requirements. Adults who are in need of this protection are persons 18 years of age or older with disabilities or handicaps such as age, physical, mental or socioeconomic, which have incapacitated them to such a degree that they are unable to take care of themselves and who have no one able or willing to assist them with the care of their needs.
Guardianship involves the provision of services to individuals who are alleged to be incompetent and their families. It includes legal proceedings in which an adult is declared incompetent by the court and another party is given the responsibility for duties relative to the adult's personal affairs and/or property. The nature and scope of a guardian's responsibility are determined by the court, based on applicable provisions of the law and the circumstances of the individual case. Guardianship ends with the death of the incompetent adult or with a judicial restoration of the adult's competency.
The legal determination that an adult is incompetent means that the adult is considered incapable of making important decisions concerning his/her personal welfare and/or financial resources. The authority in this position is given to a guardian to make decisions such as where the adult will live, including the geographical area and type of living arrangement; how the adult's income will be spent; and whether the adult will have recommended medical
treatment or surgery. To the extent that the guardian is given authority over these and other areas of the adult's life, the adult no longer has the right to make decisions in those areas. Because of the effect of declaring an adult legally incompetent and giving his/her authority to make decisions to guardian, guardianship should be considered only when other less drastic methods of assisting adults are not sufficient.
Description of Responsibilities and Duties (written in order of importance)

This worker will have the case management responsibility to provide supportive services to individuals and families residing in the county. The socialworker receives case assignments through intake as assigned by supervisor and from applications taken in the field, or as referrals from current service recipients. The clients served by this worker represents a cross-section of the adult population from age 18 until death and are from variant socio-economic backgrounds. A common characteristic of the clients served as their inability and inadequacy to maintain an independent lifestyle in his or her community due to physical health problems, limited economic resources, and an immediate change in household composition, an agency, community and/or a breakdown of the family's support system. The socialworker networks the services that are needed to assist the clients in maintaining or improving their quality of life and to continue an independent lifestyle. The work is performed by completing the following duties in order to enhance the individual and/or family functioning.
* 30% Assessment and Reassessment (To include APS responsibilities)
The Adult Service SocialWorker will make the initial interview and assist the client in identifying problems and translating these problems in existing programs. The socialworker is called upon to use his/her extensive interviewing skills to enable the client to be able to verbalize his/her feelings. The socialworker completes a thorough social work assessment of all clients evaluating their social, environment, mental, emotional, physical health ADL (Activities of Daily Living) IADL (Instrumental Activities of Daily Living), and economic status. The above process is accomplished through observations make by the worker in the home as to the interaction of individuals in the family as well as availability of resources to meet that person's basic needs in life. As the socialworker gathers this necessary information, she applies his/her knowledge base of human behavior, developmental stages, family dysfunction, psycho-social and emotional conditions as well as other socio-economic problems and available services and solutions, which enable the socialworker to accurately assess the individual family's need for services and willingness to accept assistance. These services include At-Risk Case Management, In-Home Aid, Special Assistance In-Home services, Individual and Family Adjustment, Health Support, Housing and Home Improvement, Representative payee, and all programs related to medical transportation. The socialworker, on an on-going basis, assesses and reassesses services, the need to continue services as well as the need to increase or decrease services. The socialworker must also be aware of neglect, abuse, and exploitation definitions in order to make appropriate referrals to protective service if deemed necessary.

He/She will be assigned to screen in/screen out Adult Protective Services referral for him/her to evaluate the need for this semce. He/She will gather information for this service as for any other adult service case; assessing their social, environment, mental, emotional, physical health, ADL (Activities of Daily Living), IADL (Instrumental
Activities of Daily Living) as well as the need for protection and their capacity to consent to services. Responsibilities also include assessing the competency of adults, making recommendations regarding the need for a guardian and testifying in court to achieve safety outcomes for the adult.
Development/Treatment/Goal/Service Plan
The Adult Services socialworker, after gathering the information needed to complete the assessment, helps the client come to terms with the reality of his/her situation and pointing out the individual strengths and weaknesses which the client possesses to deal with the situation. This enables the socialworker and client to develop realistic goals and service plans. These goals and plan are usually time structures and task specific. Individualized goals are directed toward allowing the client to remain in their own home at his/her highest level of functioning and the prevention of problems. This may include providing the client with a mechanism to establish links with needed community resources. The goals and plans are monitored and reviewed at least quarterly and more often if needed. A complete reassessment is done yearly.
Counseling/Therapy
Examples of techniques utilized by the Adult service worker places emphasis on the family systems approach. These include problems identification, client advocacy, reality therapy, crisis intervention, problem solving, ego enhancement, environmental manipulation, positive reinforcement, and psycho-social assessment. The worker, at times, seeks community resources to assist the client in attaining and maintaining goals. Because some of the situations the client faces are very sensitive, the socialworker offers supportive counseling.
Implementation
The Adult Service socialworker on initial visit begins assisting the client in formulating and implementing the service plan by assessing appropriate and available services, presenting options to the client and encouraging client decision-makingwith the focus on problem solving and independent actions. The involvement of other services and/or other professionals is initiated at the discretion of the socialworker in consultation with supervisor.
Monitoring/Maintenance
The Adult Service socialworker monitors that the services plan on an on-going basis for progress, appropriateness and changes if needed. Changes may occur if there are changes in client family composition, health (i.e. physical or mental), economic, social environment, or etc.

Most consultation is centered on client related problems and is provided for family and other informal support systems and for other professionals, agencies and community resources. The purpose of such consultation is to advocate for the client, to promote the goals of the service plan and to provide the needs of the client.

The socialworker, by advocating for the client, allows him/her to establish linkage to community resources. These resources are sometimes the lifeline needed to assist the client in remaining in his/her own home and having a sense of independence. It is often necessary begin this process by acting as liaison for the client with agency and community programs such as Medicaid, Food and Nutrition, Crisis Programs, churches, medical providers, civic organizations, mental health, law enforcement personnel, public housing, and any other resources available to help clients meet their needs.

Administrative duties are necessary to insure appropriate service delivery. This involves maintaining required statistical reports, accurate documentation, and record keeping in accordance with agency, county, state, and federal mandates, regular consultation with direct supervisor, and periodic in-service training sessions.

B.Other Position Characteristics
Accuracy Required in Worker
Accuracy must prevail in the position because of determining and documenting clients' eligibility for required service programs. The worker must be accurate in giving correct information and being current on information that is shared with client and community. Worker must be accurate when managing client's finances when serving in the capacity of representative payee. It's extremely important to accurately assess issues related to APS and guardianship.
Consequences of Error
If eligibility determination is not completed correctly, eligible clients may not receive needed services or in the reverse, non-eligible clients could receive services. In either case, the agency may be sanctioned negatively by the state at additional monetary cost to the county. Errors in APS or guardianship could cause irreparable harm.
Instruction Provided to Employee
Instructions are provided in written and oral form. Bi-monthly meetings are held to develop and receive instructions on policies and procedures. Worker also has scheduled conferences with supervisor. Instructions are provided by the sate through manuals and ad ID;inistrative letters. The worker attends appropriate• workshops and training regarding policy and procedures and social work trends.

Supervision Received by Employee
This employee receives supervision by scheduled conferences, meetings, informal interaction, reports, and an annual performance appraisal. Review for quality is also received through process of random case review and reading of assessments and reassessments completed on all active cases b supervisor. Random monitoring is also performed by State DSS consultant and Medicaid Consultant.
Variety and Purpose of Personal Contacts
The employee has contact with unit, staff, and other staff within the agency. Majority of worker's time is field work. Contacts will be made with clients in the private home as well as in the office. Worker also makes contacts with local health care providers, out of county health care providers, mental health, crisis organization, churches, civic organizations, State Office Personnel, Division of Aging Personnel, Region L Council of Governments Staff, law enforcement, doctor, nurses, hospital socialworkers, environmental health personnel and the general public. These contacts may be needed to gather information for assessment, and service planning. Physical Effort
The physical effort required on this job is to have the stamina to continue at maximum performance at all times. At times, effort is needed in assisting clients with mobility. Physical effort is needed for employee to move within the agency, climb stairs, and use office equipment described in #9 below on a frequent basis.
Work Environment and Conditions
Work is performed in the field and worker has a private office in the agency. Employee is placed in unpleasant or dangerous situations when dealing with hostile clients. Employee is occasionally exposed to infectious diseases such as scabies, head lice, flu AIDS, TB, and etc. All staff is aware of precautions to take when such events occur.

ROLE OVERVIEW:
SocialWorkers at KIPP North Carolina provide crucial support and services to students. In this role, socialworkers will work directly with students, teachers, community partners and families to provide individual and group counseling, as well as link families to long-term solutions and external support organizations. SocialWorkers report to their school Principals and are coached by the Director of School Social Work on the Regional Support Team.
Responsibilities of a School SocialWorker include, but are not limited to:
Service Delivery 30%
Facilitate individual sessions using evidence-based, student centered interventions to guide conversations and achieve student's goals.
Facilitate group sessions using evidence-based, student centered interventions as a model to guide conversations and influence social emotional skills/ behaviors.
Facilitate classroom lessons modeling evidence-based, student centered interventions for students and teachers.
Build peer connections by using evidence-based, student centered interventions during restorative practices (such as circle, peer conflict resolution) and lunch bunch.
Participate in continuous training to develop and build their evidence-based and student centered toolbox.
Respond to acute mental health crises and provide appropriate intervention services to students and families.
Develop and manage 504 plans, monitor progress, and provide support and consultation to general and special education teachers to promote implementation. Provide support in the development of student behavior plans.
Conducts student and classroom observations to monitor individual student progress
Provide on-going consultation to teachers, parents, and other school personnel to support students' learning, behavioral challenges, and mental health needs.

4.Initiation of court action when necessary.
Primary Purpose of the Position
This agency is mandated to provide services to children who have been identified as neglected, abused, or dependent. The primary purpose of the Child Protective Services In-Home SocialWorker, Investigations, Assessment and Treatment, (SW IA&T) is to execute this mandate. This is an advanced, professional level casework position, providing counseling and case management services for children who are identified as neglected and/or abused or dependent. This is done so by assessing family situations to determine the strengths and weaknesses of the family system and to evaluate the level of risk the child may face in the home. The socialworker also provides other services to families with children under the age of 18 years who have requested such services from the agency in conjunction with protective services. Treatment and immediate responses will be provided to crisis situations for at risk children in the most complex and hostile of cases.
Work Schedule
Regular working hours are from 8:00 a.m. until 5:00 p.m. Monday through Friday. However, in order to make the mandated number of face-to-face contacts in some situations, clients may need contact after hours and overtime may be required. Employees are given the option of flexing their workday to accommodate their clients' schedule without putting the socialworker in an overtime situation. Employee serves on a rotation basis for periodic radio coverage.

Following is a description of responsibilities and duties of this position listed in order of importance. All responsibilities and duties are essential to the effective performance of an In-Home Services SW IA&T.
Intervention developed
The In-Home Services SW IA&T is responsible for reviewing the case and becoming familiar with the family dynamics, identified problems and strengths of the family. Utilizing the Child and Family Team process, a Family Services Agreement is devised jointly with the family based on the Risk Assessment and Initial Case Plan. The Family Services Agreement contains objectives that are measurable, time limited, describe specific desired outcomes and identify necessary behavior changes. Identified needs, objectives and activities are directed toward alleviating the problems that lead to the abuse, neglect or dependency and addressing underlying issues that may contribute to the need for protective services. Once the case plan is devised, the helping process is defined.
Providing/arranging for counseling or supportive services
The In-Home Services SW IA&T provides counseling to the client directed toward the development and utilization of family support systems including recognition of develop-ment and utilization of strengths, overcoming personal, social, and environmental barriers, and utilizing agency and community resources. The worker seeks community resources to assist the client in attaining their goals.
Monitoring and assessment
The In-Home Services SW IA&T must monitor the Family Services Agreement and complete a Risk Assessment at various intervals in all neglect, abuse and/or dependency cases requiring continued mandated services. After a Family Services Agreement is negotiated, ongoing monitoring and assessment is necessary to determine if the plan continues to be appropriate and that the plan of intervention continues to address the identified needs of the parent and children. This monitoring includes regular interviews with the client in the home and requires collateral contact with other public agencies such as mental health, medical professionals, day care providers, health department personnel, and relatives and friends of the client. Updating the Family Services Agreement is necessary when changes in the family occur and it becomes necessary to revise the plan in order to implement an appropriate plan of protection for the child. Court action is used in the event the client does not meet the objective and the safety of the child is at risk. It is the worker's responsibility to provide the supervisor with a factual assessment in order for the joint decision to be made regarding court action. Prior to court action, the worker must complete the Risk Assessment, which indicates the risk and/or harm to the child by remaining in the home.
Use of the court and court preparation
Court activity is very much a part of the SocialWorker's duties. Prior to the decision for court intervention or in the case of an emergency, immediately after obtaining a non-secure custody order, the worker will present her case to the Child and Family Team which makes recommendations as to alternatives to court or the children entering into agency custody. In the event that out of home placement is necessary or, to ensure permanence in a child's life, the SocialWorker initiates court action through a non-secure custody order and/or petition, assesses placement options (relatives or foster care) and conjointly with the foster care worker develops an appropriate plan for the child. The worker prepares the child and family for separation, placement and the court process. The worker prepares a written assessment for the court, solicits witnesses when necessary, shares information with the child's Guardian ad Litem and testifies in court making the agency's recommendation. The worker must also follow up with written reports for court review as ordered or required. The worker may conduct an investigation of suspected abuse and when evidence of abuse is found the worker is required to report such information to the District Attorney and law enforcement within a specified time frame both verbally and also in writing. Law enforcement and the Case Manager interview the victim and perpetrator. Should the perpetrator be prosecuted, the worker may be a witness during court proceedings. SocialWorkers may be subpoenaed to testify in both civil and criminal proceedings due to their involvement with their clients.
Monitoring, reviewing, maintaining
The In-Home Services SW IA&T is responsible for monitoring, reviewing, and maintaining cases through various means. This is accomplished through frequent contact with the client by visiting them in their home, interviews in the agency and telephone contact, collateral contacts and in some situations periodic court reviews. Both face to face and collateral contacts are made monthly according to state standard. The SocialWorker formally assesses the client's progress by updating and re-signing the Family Services Agreement on a quarterly basis and whenever there is a major change in the family, reassesses the risk to the child(ren) by completing the Risk Assessment form. These reviews assess the progress of the client in alleviating the factors that lead to the abuse, neglect or dependency and whether the client has been able to demonstrate they can provide a safe environment for their child (ren). Further services may be identified at this time. The SocialWorker is required to present cases in the agency's foster care review team when a child comes into agency custody. This team closely monitors those children whose custody has been removed from his parents or whose custody has been placed with the Department of Social Services.
Interpretation of Child Protective Services
The In-Home Services SW IA&T is a member of the agency's public forum and occasion-ally is called on to speak at meetings of community agencies and organizations in an effort to emphasize the preventive nature of the agency's services and to familiarize the commu-nity with the reporting laws. The worker assists in providing information to other agencies and local citizens to better help them understand the functions of the agency. This allows for better rapport that leads to better delivery of services and cooperation from other disciplines.
Employee will participate in disaster related programs, including shelter duty, as needed.

In all duties outlined above, all forms of documentation required by the North Carolina Division of Social Services, North Carolina General Statutes, and Nash County Department of Social Services is included.

Accuracy required in work
Great precision is required in this work. The worker must correctly gather information to determine whether or not referrals meet the neglect/abuse laws; investigate each case according to standards to determine if neglect/abuse exists; correctly assess a child's safety needs; and continually assess a family during intervention to assist in alleviating problems which led to neglect/abuse. The worker must be able to meet state standards and assure compliance with CPS policy and best practice. Documentation, time reports, day sheets, and other state and county forms must be completed accurately and timely.
Consequence of error
Initial assessments, investigations, and case management decisions directly affect the safety of children. Incomplete or inaccurate documentation or reports may affect agency funding negatively and may lead to sanctions against the agency.
Instructions provided to employee
Instructions are provided in written and oral form by the supervisor through scheduled conferences, at agency staff meetings, and in workshops and professional training sessions attended by the employee. In addition, weeknights, weekends and holidays, a consultant is always available to assist as a second party decision-maker during all phases of CPS intake and initiation, in all child placement responsibility decisions, and in all other situations when a second party is needed.

Employee receives review through scheduled conferences, meetings, informal interaction, reports, and an annual performance appraisal. Variety and purpose of personal contacts
Employee has contact with Unit staff, other units within the agency, clients, Health Department, other social services departments, Guardian Ad Litem Program, Mental Health, medical personnel, crisis organizations, churches, civic organizations, judicial system, law enforcement, Emergency Services personnel, and the general public. Contacts are often used as a means to assist in planning and/or meeting the needs of clients as a part of the human delivery system and to aid in assessing client's current situation.
Physical efforts
Employee must be able to move within and without the agency, climb stairs, and use office equipment described in #9 below on a frequent basis. Employee occasionally lifts small children, camera, suitcases, toys, car seats, etc. in performance of duties. Employee must travel to meetings within the county as well as workshops and meetings out of the county. Overnight stays are sometimes necessary.
Work environment and conditions
Work is performed in a county social service agency with contacts in the field. Employee encounters hostile clients, some of whom are dangerous. Clients include mental patients, alcoholics, and substance abusers who are sometimes verbally abusive and threatening. Field contacts are sometimes required even in adverse weather conditions such as snow, sleet, extreme heat and natural disasters. Employee is exposed to infectious diseases such as scabies, head lice, flu, AIDS, etc. All staff are aware of precautions to take when such events occur. All staff are also trained in the use of fire extinguishers.
Machines, tools, instruments, equipment and materials used
Employee uses agency computer, printer, copier, calculator, camera, automobile, telephone, cell phone, car seat and iPad in order to perform duties. When conducting investigations, employee sometimes finds it necessary to use anatomically correct dolls. Employee must have immediate access to a vehicle to perform essential functions of the job.
Visual attention, mental concentration and manipulation skills
Close visual attention is required when operating a motor vehicle. Intake, investigations, and case management duties all require visual attention and mental concentration. Body language, eye contact, and behavior of family members assist the socialworker in assessing the family situation and must be viewed carefully. Preparation of court reports, development of protection plans, and completion of risk assessments require very intense mental concentration, as does reviewing placement options for children. The socialworker must carefully document times, dates, and other factual information so that accurate assessments and recommendations are made. Employee must have physical dexterity required to successfully and efficiently operate machinery and tools outlined in I above.
Safety for others
Employee makes decisions daily that affect the lives of clients and continuously assesses the safety of children and the environment of our clients. Employee devises protection plans, Family Services Case Plans and completes risk assessments. Employee also makes recommendations regarding the removal of children when homes are not safe and the return of children when homes are safe. Employee must carefully evaluate the needs of children in placement responsibility and their caregivers to determine the best plans for children. SocialWorker abides by confidentiality guidelines to protect clients and uses caution when operating a motor vehicle when transporting clients. Primary Purpose of the Organizational Unit:
The Child Services Division of the Johnston County Department of Social Services provides Protective Services Intake, Investigations, Assessments, Emergency On-Call, and Case Planning and Case Management; Permanency Planning; Foster Care Reunification and Placement; Independent Living; Recruitment and Training for Foster Parent Families and Adoptive Applicants, Agency Adoptions; and completion of Home Study requests from other North Carolina Departments of Social Services and the Interstate Compact.
The Staff Development Unit consists of CQI/Trainers who define, measure and improve outcomes on an ongoing basis in the Child Services Division. The CQI team completes case record reviews; coordinates State and Federal audits/reviews; coordinates and/or provides training to agency staff; compiles, analyzes and disseminates data; manages the agency's internship program; and completes other duties as assigned by the DSS Director or their designee.
Fifteen percent (15%) of this role involves training agency staff on essential policies, practices, and procedures to ensure consistent, high-quality service delivery. This responsibility includes developing and delivering training sessions that strengthen staff understanding and adherence to local, state, and federal mandates.
This employee's primary responsibility is to monitor performance, using case reviews, and process reviews and data analysis. This employee uses case reviews to evaluate adherence to federal, state and county policy guidelines and to assess the quality of work performed by individuals and teams within the specified Division or program area. This employee will also perform other duties as directed by CQI Social Work Supervisor or the DSS Director as it pertains to assuring quality performance in the services provided by the agency. ROLE OVERVIEW:
SocialWorkers at KIPP North Carolina provide crucial support and services to students. In this role, socialworkers will work directly with students, teachers, community partners and families to provide individual and group counseling, as well as link families to long-term solutions and external support organizations.
