The socialworker demonstrates proficiency in providing traditional social work services in a professional and respectful manner with the goal of helping older adults live safely and comfortably in their homes and communities for as long as they can. This position is full time M-F 8am to 4pm.

RESPONSIBILTIES:
Participates on the IDT's initial assessments, care planning and on-going re-assessments of participant care.
Attends IDT meetings; actively participates in team meetings by sharing pertinent information, providing follow up to assigned tasks and helps to develop participant's plan of care.
Completes all assigned assessments - (initial, semi-annual, annual, service request and/or significant event).
Assesses the psychosocial needs of the participant and provides supportive counseling, working collaboratively with behavioral health providers.
Facilitates hospital, rehabilitation and nursing home (NH) admissions and discharges as determined by the Interdisciplinary Team. Ensures that PASRR documentation is completed for NH admission.
Assists in the conversion process of the participant from community to long-term care. Works collaboratively with Medicaid Specialist, skilled nursing facility, and participant's caregiver to complete conversion.
Arranges and facilitates family meetings, as needed.
Refers participants and families to appropriate community services and acts as liaison and/or advocate with community organizations for participants.
Maintains professional, accurate and timely social service documentation in the participants' medical records.
Conducts participant council meetings as assigned.
Works collaboratively with Director of Social Work and Behavioral Health provider to ensure guardianship is up to date. Educates participant regarding health care proxy (HCP). Assists participant in completion of HCP form.
Works collaboratively with fiscal department to maintain participant insurance benefits and completes required documentation of fiscal information in the medical record.
Reviews plan of care with participants, guardian, and/or activated health care proxy as assigned.
Complete authorizations for home care and other approved services timely and accurately.
Completes home and/or skilled nursing facility visits to assess participant as indicated.
Works collaboratively with Palliative care team; Assists with end of life planning as indicated.
Provides timely communication to appropriate staff regarding the following: (disenrollment, conversion to long term care, transfer of sites, participant and/or caregiver demographic changes).
Reports allegations of abuse to appropriate state agency; provides support and resources to participant as he/she will accept; completes required documentation.
Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
Performs other duties as required.
Frequent local travel.
Qualifications:
Current Social Work licensure in the Commonwealth of Massachusetts at the Masters level (L.I.C.S.W. or L.C.S.W.) required
Minimum of 1 experience in Social Work providing traditional Clinical or Case Management services with a geriatric population
Current C. P. R. Certification or ability to become certified
Treat all participants in a welcoming and professional manner.
Strong verbal, written and listening skills with ability to multi-task in a fast pace environment
Vietnamese and/or Khmer speaking but all MSW eligible candidates to apply.

This is a per diem position. We are looking for a LICSW (Licensed Independent Clinical SocialWorker) to work at Mass Eye & Ear at our main campus in Boston, MA.
Job Summary
Summary
This position is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients. The Clinical SocialWorker Senior provides support to those who are dedicated to patient and family-centered care that values personal self-determination, are skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

Essential Functions
-Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
-Formulates biopsychosocial assessment, disposition and treatment plans.
-Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessment when violence is suspected Plan discharge from care facility to home or other care facility.
-Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
-Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
-Screen, address, and manage social determinants of health.
-Prepares and maintains all pertinent records, statistics, and progress notes.

Minimum Entrance Requirements:
Applicants must have (A) a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education or criminal justice or a relevant human services degree and (B) a current and valid Licensures as a Licensed Social Work Associate, Licensed SocialWorker, Licensed Certified SocialWorker or Licensed Independent Clinical SocialWorker issued by the Massachusetts Board of Registration*. *Applicants at the Department of Children and Families must obtain the required license in Social Work within the first nine (9) months of employment. The classification may require possession of a current and valid Motor Vehicle Driver's License at a class level specific to assignment.
Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.

Job Description
The role of the PC is to assist clients in strengthening and establishing lifelong connections that will facilitate success throughout their lives. Working as part of a team that is led by the Permanency Specialists (PS), the PC meets with clients to provide support and counseling to them, their parents and family members. PCs are expected to work independently and be guided by the treatment plan. PC's typically provide some or all of the following services: strength-based counseling and support that includes working with the school or employer (if applicable), to monitor functioning and activities and provide coaching based on interests and or needs. This could include job searching, monitoring school attendance, finding pro-social community-based activities and encouraging the youth to engage in the activities that may assist in building their self-esteem and the positive connections that they have.
Representative Duties
· Meet individually with clients, or with family if identified by PS directives, and meet hours required by contract with DCF or DMH
· Provide mentoring and identify client and family strengths
· Communicate with local schools and/or job sites if appropriate
· Support clients in completing community service if appropriate
· Support clients and their families at night & on weekends as needed or when on call
· Set up pro-social activities for clients in their home community
· Attend court as requested by the PS.
· Provide transportation when necessary and appropriate as determined by treatment team goals
· Support adult living skills development
· Complete appropriate sections of weekly and progress reports, in accordance with Becket policies and regulatory requirements
· Complete and file documentation in accordance with Becket timelines, including SOAP notes and Gateway Delivery
· Attend supervision as requested by supervisor, at a minimum of weekly
· Attend office and Treatment Team meetings
· Inform the creation of treatment plans and be guided by the treatment goals
· Provide case management assistance including: community resource referrals and outreach, school outreach/collaboration, employment outreach.
· Establish and facilitate support group activities in the community that foster growth including Adult Living, Job Search, Networking, Physical Health, Social Skills and individual client passions.
Qualifications
Requirements:
- Bachelor's Degree in Social Work, Psychology, Education, Mental Health Counseling, or related field, with an emphasis in Human Services and Family Systems
- Candidates with additional Therapy, Psychology, Counselor, and/or Mental Health licensures, certifications, and/or trainings are encouraged to apply (i.e. Behavior Analyst - BCBA).
- Bilingual (English / Spanish)
