EDUCATION AND EXPERIENCE

We are seeking a compassionate and experienced Licensed Clinical SocialWorker (LCSW or LICSW) to join our hospice care team, dedicated exclusively to weekend coverage. Perform weekend patient visits, ensuring continuity and quality of care.

Conduct admission inquiry visits, assessing patients for hospice eligibility and collaborating with the clinical and admissions team as needed.
Conduct psychosocial assessments and develop individualized care plans for patients and families receiving hospice care.
