Responsibilities
* Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
Provides social work services in accordance with the plan of care.
Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
Educates the patient and family members and other team members on medicalsocial work interventions and treatment plan.
Performs care planning, transfer and discharge functions.

As a Hospice MedicalSocialWorker, here's a big-picture view of what you'll do:
Conducts an initial psychosocial assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patient's/family's needs and the IDG plan of care.
Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.
Identifies special needs related to religious or cultural diversity and how these impact the implementation of the IDG plan of care.
Provides counseling in response to identified anticipatory grief or pathological grief, if so qualified, or identifies alternative resources for such and provides direction to those resources.
Evaluates identified financial or environmental concerns and directs the patient/family to appropriate community resources, as well as provides follow-up and acts as a liaison when indicated.
Provides crisis intervention support to the patient/family, members of a facility caregiver group or the IDG.
Provides psychosocial updates to facility staff providing short term inpatient care or respite care.
Participates in the development of the IDG plan of care and attends regularly scheduled IDG meetings.
Assists the attending physician and other IDG members, including facility staff, in understanding significant social and emotional factors related to death/dying issues of the patient/family.
Participates in discharge planning for the patient/family when needed.
Assists the patient/family in electing advance directives, securing durable power of attorney, making funeral arrangements and other financial, legal and healthcare aspects of end-of-life choices.
A few must-haves for Hospice MedicalSocialWorkers:
Graduate of a Master's program in social work accredited by the Council on Social Work Education.

As a Hospice MedicalSocialWorker, here's a big-picture view of what you'll do:
Conducts an initial psychosocial assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patient's/family's needs and the IDG plan of care.
Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.
Identifies special needs related to religious or cultural diversity and how these impact the implementation of the IDG plan of care.
Provides counseling in response to identified anticipatory grief or pathological grief, if so qualified, or identifies alternative resources for such and provides direction to those resources.
Evaluates identified financial or environmental concerns and directs the patient/family to appropriate community resources, as well as provides follow-up and acts as a liaison when indicated.
Provides crisis intervention support to the patient/family, members of a facility caregiver group or the IDG.
Provides psychosocial updates to facility staff providing short term inpatient care or respite care.
Participates in the development of the IDG plan of care and attends regularly scheduled IDG meetings.
Assists the attending physician and other IDG members, including facility staff, in understanding significant social and emotional factors related to death/dying issues of the patient/family.
Participates in discharge planning for the patient/family when needed.
Assists the patient/family in electing advance directives, securing durable power of attorney, making funeral arrangements and other financial, legal and healthcare aspects of end-of-life choices.
A few must-haves for Hospice MedicalSocialWorkers:
Graduate of a Master's program in social work accredited by the Council on Social Work Education.
Licensed to practice as a SocialWorker in the state in which employed. License is active and in good standing.
Minimum of one (1) year's experience in health care.
Hospice experience preferred.
Strong interpersonal skills, good communicator, empathetic, compassionate and resourceful

Job Purpose
The Licensed SocialWorker (LISW) or Licensed Professional Counselor (LPC) reports to the Division Director. Required Minimum Training and Education
Master's degree in a social work or social welfare program accredited by the Council on Social Work Education or from a social work program or social welfare program offered by a regionally accredited institution and three years professional social work experience, one of which must have included supervisory or administrative experience. Master's degree in counseling from an accredited counseling program.
Preferred experience
Previous LISW or LPC experience preferred. Hospital experience in the area of Psychiatry is preferred.
Degree of Supervision
LISW or LPC will be assigned a Primary Supervising/Collaborating Physician employed by MUSC and will report directly to this physician.

Must have a active/valid South Carolina (SC) LPC or LISW license or be eligible to get these licenses before credentialing date and start date.
Job Duties/Tasks
* 35% - Provides individual and group therapy to adult patients in an outpatient community psychiatry treatment clinic located within an academic medical center.
* 35% - Interviews client, family and any other providers to assess the patient's level of functioning, strengths and deficits, and emotional status. Develops a comprehensive picture of the client's service and treatment needs.
* 30% - Serves as a member of the treatment team in the diagnostic evaluation of clients and the identification of treatment objectives.
Physical Requirements
Continuous requirements are to perform job functions while standing, walking and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Ability to be qualified physically for respirator use, initially and as required. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Frequently lift, lower, push and pull and/or carry objects weighing 50 lbs (+/-) unassisted, exert up to 50 lbs of force, lift from 36" to overhead 25 lbs. Infrequently work in dusty areas and confined/cramped spaces.
