*What you will do: *
* Supports referrals from providers in the ambulatory setting seeking biopsychosocial and behavioral health services for their patients.

Job Summary:
Northwestern University Mesulam Center for Cognitive Neurology and Alzheimer's Disease (Mesulam Center) is seeking an exceptional socialworker who demonstrates a pronounced interest in working with persons with neurocognitive disorders and their families in a diversity of roles. The SocialWorker will work as a team member to provide clinical care for persons living with dementia and their families and achieve the goals and objectives of the Outreach, Recruitment and Community Engagement (ORE) Core, the Miller Family Buddy Program, in addition to other psychosocial initiatives.
Specifically, the SocialWorker will provide clinical social work services for persons living with dementia and their caregiving families in the Neurobehavior and Memory Clinic of Northwestern Medical Group; contribute to the recruitment and retention of research participants that meet Center research priorities; manage and facilitate monthly care partner support groups; and participate in the coordination and planning of an annual scientific conference. In addition, they will participate in the preparation and review of annual progress reports and competitive renewal for the NIA and other funding agencies.
The Mesulam Center is a multi-departmental component of the Feinberg School of Medicine. We specialize in the clinical care and scientific study of neurological diseases that interfere with cognition and behavior. Areas of emphasis include age-related memory, language-based and behavioral dementias. We are focused on Alzheimer's disease, Primary Progressive Aphasia and Frontotemporal Dementia. Our three interacting missions are to investigate the causes and clinical characteristics of neurodegenerative diseases, to ensure that our patients and families are the beneficiaries of the latest research advances, and to prepare future scientists and clinicians in our field.
Specific Responsibilities:
* Provide clinical social work services to patients and families of the Northwestern Medicine Neurobehavior and Memory Clinic and center research participants. Complete psychosocial assessments and provide education, counseling and linkage to appropriate community services. Participate in weekly interdisciplinary team meetings.

Required Skills
Qualifications:
Master's Degree from a school of social work accredited by the Council on Social Work Education

*Care coordination*:
Collaborating with the hepatology team, including doctors, nurses, and dietitians, to ensure patients receive coordinated care and access necessary resources.
*Resource linkage*:
Identifying and connecting patients with community support services such as financial assistance programs, housing support, transportation assistance, and support groups.
*Advocacy*:
Representing patients' needs to insurance companies and other healthcare providers to navigate complex medical billing and access to treatment.
*Education*:
Providing patients and families with information about liver disease, treatment options, and coping strategies.
*Crisis intervention*:
Responding to urgent situations, such as emotional distress or sudden changes in health status, and providing immediate support.
*Post Discharge planning*:
Assisting with post-discharge planning, including coordinating follow-up appointments, home healthcare needs, and community support services.
*What you'll need for this job:*
* Minimum Education: Required: Master's Degree in Social Work
* Minimum Experience: Preferred: 1-2 years of previous job-related experience
* Licensure/Certifications: Required: Licensed SocialWorker State of Illinois Preferred: Licensed Clinical SocialWorker State of Illinois in Hospital Setting Preferred
* Will consider a license pending MSW. Candidate must achieve LSW with 6 months of hire. Candidate will also receive weekly individual supervision.
* Graduate level internship in hospital setting with minimum of 1-year intensive training required.

Salary Range: $30.00 - $41.59 per hour
