The duties of the Caregiver Support Senior Social Worker duties include: * The incumbent works with a team of other Social Workers and Nurses assigned to the Caregiver Support Program to ensure implementation of the program is consistent across the NF/SGVHS and in accordance with VA policy and guidelines, OIG reviews, Joint Commission requirements, and other, as appropriate. * Develops, implements, evaluates and maintains the Caregiver Support Program specific to his/her area of assignment and ensures compliance collaboration with local facility, VISN and VA Central Office Caregiver Support Program initiatives. * Serves as a subject matter expert, understanding related State and Federal law, State and Federal documentation requirements, understanding clinical implementation requirements and is able to execute implementation of the VA Caregiver Support Program within NF/SGVHS. * Completes screening and assessment of the Veteran/caregiver in collaboration with the interdisciplinary treatment team and the goals of the assessment are to identify psychosocial, medical and mental health needs and formulate a plan to meet those needs. * Demonstrates an understanding of the range of treatment interventions and skills required to implement a variety of treatment intervention which may include specific evidence-based interventions related to positive outcomes for psychosocial and mental health care for assigned specialty care population. * Conducts assessments of Veterans and Caregivers applying to the program and follows national protocols for these assessments and also responsible for receiving, processing and tracking applications to the program from receipt to determination, and following all processes and procedures outlined in guidance provided by VACO, VISN and local facility. Responsibilities Identifies appropriate discharge social work needs for assigned patients (providing resources, community support, ensuring advance directives/next of kin is identified). Partners with community agencies to coordinate care across the continuum to offer options, tools, and resources to allow patients the opportunity to achieve the desired goals. Assists with patient evaluation and treatment to further their understanding of significant social and emotional factors underlying a patient's health problem(s). Acts as a patient advocate: investigates and reports adverse occurrences and performs staff education related to resource deployment, discharge planning and psychosocial aspects of healthcare delivery. Assists care team and family with end of life conversations, offers grief counseling, and performs hospice consults Assist with determination when care conference is appropriate; coordinates scheduling with team & caregivers Plan and support patients in coping with emotional, economic, social, and environmental challenges. Assess for the need/eligibility for financial assessment and refer to the appropriate programs. Performs discharge planning activities, including referrals to Home Health, Skilled Nursing, Inpatient Rehab, DME, etc. for assigned patients. Documents patient data, plan, interventions and outcomes according to department guidelines. Qualifications Master's Degree in Social Work or a related field is required. 1 year of recent clinical experience applicable to the designated population served is required.
