Position Summary: Performs professional level social work in areas such as: assessing patient needs and coordinating a safe, comprehensive discharge plan which may involve emotional support to the patient, the family or significant others. Education/Experience: Equivalent to a Bachelor's Degree in Social Work and one (1) year of professional level social work experience in a social service agency or hospital setting; or an internship or field work related to the profession.
