MAJOR FUNCTIONS:
The SocialWorker position requires a wide range of clinical skills including interviewing, individual and group counseling and a thorough knowledge of the substance abuse, mental health and human service system resources. The SocialWorker is responsible for the assessment and treatment of clients that have co-occurring mental health issues requiring a specialized group curriculum.

SPECIFIC DUTIES & RESPONSIBILITIES:
Complete clinical assessment and re-assessments at regular intervals to measure progress towards addressing life areas deficiencies.
Provide individual counseling to address outstanding family, medical, psychiatric, and housing concerns.
In collaboration with each consumer, formulates a treatment plan, monitor achievement of objectives, review and update treatment plans in accordance with regulatory requirements and as clinically indicated.
Conduct evidence-based group counseling services related to a wide range of clinical and community issues.
Maintain up-to-date resident charts including assessments, treatment plans, case notes and documentation of collateral contacts.
Provide crisis intervention services.
Assist in the development and implementation of consumer discharge plans.
Work closely with utilization review staff to ensure completion and accuracy of all necessary health insurance authorization documentation.

Function as liaison and provide education and direct services to family members, as appropriate, in regards to consumer's substance use and related behavioral issues.

Function as “Manager” on Duty in the absence of an on-site manager.

REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES
Master's Degree in Social Work, Counseling, or Marriage and Family Therapy required.
One (1) year experience working in a social service setting providing group counseling, family therapy and case management with chemically dependent clients preferred;
NYS Licensure as a SocialWorker, Mental Health Counselor or Marriage and Family Therapist preferred. Individuals obtaining a limited permit will be considered meeting this requirement.

Position Summary:
The Geriatric SocialWorker will provide trauma-informed services to older adults to ensure client safety, dignity, and access to appropriate resources. Principal Responsibilities:
* Conduct in-depth assessments of older adults with identified abuse, neglect, or exploitation.
* Evaluate risk factors, determine the immediate safety needs of the client, and conduct safety planning.
* Perform home visits as necessary to gather information.
* Provide supportive counseling, case management, and referrals to appropriate services (i.e. medical, legal, housing).
* Assist with applications for emergency food cards and/or financial assistance as needed.
* Work closely with law enforcement, healthcare providers, legal professionals, community partners, and Adult Protective Services.
* Advocate for clients safety and assist with navigating systems (i.e. Family Court Order of Protection petition).

JOB SUMMARY

JOB-RELATED DUTIES - ESSENTIAL FUNCTIONS
These duties are essential and specific to the successful implementation of this position.
SERVICE AND CARE COORDINATION
Ensure residents receive quality and appropriate services per organizational principles and program requirements.
Conduct comprehensive biopsychosocial assessment to identify immediate and long term needs in healthcare, food and nutrition, childcare and youth education, employment and vocation, immigration and citizenship status, English language proficiency, trauma care and recovery, and other social need as disclosed by residents.
Coordinate with Case Managers to design service and care plans that correspond to biopsychosocial assessments and address human, health, education and employment, recovery and stability needs.
Coordinate workshops and activities that enhance self-care and healthy habits, cultural and social adjustment, recovery and stabilization, and English language acquisition.
Collaborate with external providers and organizations to assist residents with social and cultural adaptation, address social and human service needs, access preventive and remedial healthcare, and access to mental health services including trauma recovery and therapeutic counseling.
Meet with residents in assigned caseload at least once a week to assess progress with service and care plans. Modify plans as needed.
Coordinate with Case Managers to conduct weekly room inspections to ensure healthy hygiene and housekeeping practices.
Coordinate with Case Managers on discharge plans to ensure needed service linkages and referrals to community-based organizations are in place.
INTERVENTION SERVICES
Develop and facilitate workshops and activities with external service providers; monitor resident engagement and assess impact of activities and workshops to individual residents and the general resident population.
Provide crisis intervention and de-escalation interventions as needed to ensure the safety of residents, staff, and visitors in the building. Follow-up with residents post Priority 1 and 2 incidents and bridge residents to services as needed.
GENERAL DUTIES
Document interactions and communications with and on behalf of residents in CARES and AWARDS.
Perform other duties as assigned by supervisor or Program Director.

What do I need to apply?
Education and Experience: Master's Degree in Social Work, LMSW preferred. Three (3) years of experience in case management, counseling, or direct advocacy for clients in social service settings. Experience with immigrants and immigration issues prioritized.
Communication: Excellent verbal and written communication skills. Proficiency in English required, multi-lingual and Spanish proficiency prioritized.

MAJOR FUNCTIONS:
The SocialWorker position requires a wide range of clinical skills including interviewing, individual and group counseling and a thorough knowledge of the substance abuse, mental health and human service system resources. In collaboration with each consumer, formulates a treatment plan, monitor achievement of objectives, review and update treatment plans in accordance with regulatory requirements and as clinically indicated.
Conduct evidence-based group counseling services related to a wide range of clinical and community issues.
Maintain up-to-date resident charts including assessments, treatment plans, case notes and documentation of collateral contacts.
Provide crisis intervention services.
Assist in the development and implementation of consumer discharge plans.
