How to qualify 1. Education a. Possession of a Master's Degree in Social Work from a school accredited by the Council on Social Work education (CSWE); OR b. Possession of a Master's Degree from a two-year (2) counseling program from an accredited college or university. (Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum). Score Banking: Scores attained on the Protective Services Worker Exam for Family and Children Services will be 'banked' for two years, starting from the date of the examination. This means that, during this two-year time period, candidates need not take this test again. However, after one year, a candidate has the option to either (a) keep the same score and apply their test score to the other announcement or (b) re-take the test. Re-testing is permitted no sooner than one year from the date of the test. If a candidate opts to re-test, the re-test score becomes the candidate's official score since it is the most recent. This is a per diem/on call inpatient position supporting patients in Santa Rosa Hospital but will be supporting Healdsburg and Petaluma Valley as needed. REQUIREMENTS JOB TITLE: GeroPsych Social Worker/Discharge Planner CC/Department: 6341/Geriatric Behavioral Health Reports to: Director of GeroPsych JOB SUMMARY: The psychiatric unit social worker assists patients on admission and completes treatment plans per hospital policy. The social worker provides individual and group counseling, communicates with families and patient's authorized representatives, conservators, physicians and other health team members. He/She coordinates discharge-planning, assists with advance health care directives, financial and legal issues and maintains documentation of services in the medical record. The SW participates in the development and implementation of the comprehensive treatment plans, follows the discharge aftercare process, participates in the weekly reviews, and coordinates the discharge plan with the team.
