Required to work 4 hospitals in the Southside or Western Tidewater regions. Location and unit to be assigned based on greatest need.There are three region options.Must be within 75 miles and 90 minutes of all four hospitals in assigned region.
• Operational hours = 8:00 AM - 4:30 PM 7 days per week
• Flexi - Hours: The team member must enter at least 48 hours of prescheduled time per calendarmonth.
• Facility supported: Members of the team are required to work assigned shifts in all units within the case management specialty. Required to work in 4 hospitals in the Southside or Western Tidewater regions.

• Flexi - Hours: The team member must self-schedule at least 48 hours of prescheduled time per calendar month in API. Of these prescheduled hours, 16 hours are to include availability to be scheduled on a weekend.

Education:

As an Inpatient Care Manager, responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs of each referred patient. Collaborates with patients, families, healthcare team members, and community organizations, and uses strong advocacy skills to identify needs and resources in the hospital and community.
Develop and evaluate effective transition plans taking into consideration the client's diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care. Provide comprehensive school social work services to parents and students under McKinney-Vento. The role is to address barriers to housing and explore community or school base resources to families experiencing homelessness. ESSENTIAL TASKS
* Provides families with information related to the needs of their child and acts as a resource to parents/guardians by providing family support activities and communicating available services; supports homeless unaccompanied youth in securing necessary services and connects them to subject experts to ensure their needs are being met to include their rights as independent students under the Higher Education Act of 1965.
* Collaborates with school staff to develop customized interventions for students identified as homeless, including school site determinations and develops individualized service plans.
* Serves as a liaison between schools and community agencies/facilities/Boards such as homeless shelters, social services, transportation vendors, and court services to coordinate assistance for homeless students. Serves as a liaison between the students/families and other VBPCS-based resources.
* Manages the McKinney-Vento dispute resolution process for all students/families regarding the appropriate identification or school site determination. * Acts as a resource to school-based administrators and staff and social workers regarding homeless students, guidance on homeless/school attendance policies and laws, and record-keeping requirements.

* Creates monthly data reports of McKinney-Vento program and collaborates with multiple departments to gather program data and develops an annual data summary report and shares data with key stakeholders.

* Conduct parent interviews to acquire socio-developmental information as needed for special education referrals.

KNOWLEDGE, SKILLS AND ABILITIES

EDUCATION AND EXPERIENCE

The Role

Fitters are often called the "carpenters" of shipbuilding or "Artists of Steel" because they need to read and interpret blueprints or plans to place, maneuver, or fabricate steel structures or hulls of ships. Fitters use a variety of tools to measure, cut, grind, fit, align and tack weld parts as small as a fingernail to the large sub-assemblies and super lifts that form the structural body of a ship. Fitters also build in-place all interior structures, hatches, doors, bulkheads, etc., to complete all the steel work in the ship.
Fitters at NNS attend and become qualified through the shipyard's state of the art welding school. Fitters are expected to climb and work in high areas, crawl and work in cramped, confined spaces, and perform tasks in all kinds of weather conditions. A fitter will apply their skills in various locations across the shipyard, including the shops, platens or concrete and metal structures, dry-docks, and onboard ships in support of new construction, ship repair, and ship overhauls.

Assess each resident with a valid consent to determine individual needs. Develop and follow a treatment plan to meet each resident's needs and ensure that all treatment provided is medically necessary.

Document treatment plans, psychotherapy sessions, and treatment plan updates according to Eventus WholeHealth's procedures and policies and provide the facility with written documentation within 48 hours of service for inclusion in the patient's facility medical record.
Meet with Resident Care Coordinator or other staff as designated by the facility upon arrival to and departure from facility to review current facility care needs and communicate findings or concerns.
Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding treatment plans and other pertinent caregiver information.
Be knowledgeable of and adhere to Eventus WholeHealth standards, policies, and procedures. Be aware of and adhere to all legal and regulatory agencies' rules and guidelines and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
Conduct self in a professional manner at all times.
