Position Overview: The Intensive Outpatient Clinic (IOC) at University of Utah Health is seeking a Licensed Clinical SocialWorker (L.C.S.W.) or Clinical Mental Health Counselor (C.M.H.C.) to provide professional counseling services to a population of individuals with complex medical and psychosocial needs. Providing comprehensive and integrated care, the IOC operates as an interdisciplinary team of medical and behavioral health providers aiming to improve clinical outcomes and decrease the overall cost of care for the population we serve. In 2023, Governor Cox's One Utah Health Collaborative recognized the IOC has as leader in novel comprehensive care delivery and sustainable financial innovation. The IOC was also featured in Meet Me Where I Am - a short film that premiered in the 2022 Sundance Film Festival - that tells the story of one IOC patient and how this clinic provides unique integrated care.
This position provides direct therapy and works collaboratively with the medical and behavioral health teams to deliver comprehensive care oriented around treating the bio-psycho-social needs of the IOC patient population. Therapy activities include individual and group counseling. At minimum, the ideal candidate will have a working knowledge of trauma and substance use disorders. Case management experience is preferred. The IOC exclusively serves a Medicaid population and operates under a capitated model with the University of Utah Health Plans. No billing is required.
This position reports to the IOC Medical Director and will support the IOC patient population and the clinic team located at the University of Utah Population Health Center near Rose Park.
This position is considered Category I: Mandatory and required on campus or health care facilities to support on-site operations. Employees hired into this category will be required to be physically located at 168 North 1950 West, Salt Lake City, UT 84116 if they are healthy and without COVID-19 symptoms.

Responsibilities
Job Specific Responsibilities and Accountabilities:
* Provides regular (e.g., weekly, bi-weekly, monthly) therapy to patients to work towards achieving psychosocial goals and to address sources of adversity, including histories of trauma, using evidence-based and trauma-informed strategies.
* Develops and facilitates groups focused on skill building and creating community and trust among patients.
* Provides acute mental health support via phone and in-person visits.
* Works collaboratively on the interdisciplinary team - making sound therapeutic recommendations and supporting conflict resolution for individual patients, couples, and/or in groups.
* Develops and implements individualized treatment approaches to meet the needs of each patient and their families.
* Works in close collaboration with all members of the treatment team, including medical providers, to improve awareness of the significant impacts that social and emotional factors have on patient health and well-being.
* Expertly applies the principles of life span growth and development to support patients achieving their behavioral health goals and improving their clinical outcomes.
* Delivers high-quality, exceptional, and tailored care as described in the IOC's policies and procedures and at the University of Utah standard of care.
* Identifies and arranges community resources that provide individualized assistance for patients, which may involve post-treatment financial assistance, shelter, and/or transportation.
* Identifies appropriate outside resources, such as community agencies, to assist patients with resuming life in their own community and empowering patients to independently optimize their medical and behavioral health.
* Works collaboratively with the medical and behavioral health teams to update IOC policies and procedures and participates in clinic process improvement efforts.
* Oversees and supervises SocialWorkers, Interns, and Caseworkers within their area(s) of assignment.
Minimum Qualifications
Required
* Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
* One of the following
* Current Licensed Clinical SocialWorker (L.C.S.W.) certificate for clinical practice in the State of Utah.
* Current license to practice as a Certified SocialWorker in the State of Utah.
This position is patient-sensitive and must fulfill all associated requirements. Job Essentials
Organize and prioritize daily work by reviewing new, current, and discharging patient needs in area(s) of responsibility. Complete documentation as required.
Gather psycho-social information for patients and caregivers.
Educate, communicate, and coordinate with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, a timely intake and discharge, and to identify and resolve delays and issues.
Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care team, outside agencies and other resources as necessary.
May provide initial intake, crisis intervention, and adjustment to illness counselingservices as well as appropriate referrals to treatment and/or resources.
May identify patients at risk for potential avoidable delays, extended stay, readmission, financial or complex discharge needs. Coordinate with care providers and leaders to reach optimal solutions.
May maintain an active role in denial prevention. Proactively intervene with payers to prevent inpatient denials. Communicate any necessary information to payers to help appeal existing denials.
May coordinate the discharge planning process from patient admission to departure. Screen for and identify patient needs, develop a discharge plan, and coordinate with internal and external services for care during and after the hospital stay.
Provide support and education to patients, families and staff members on Advance Directives.
Minimum Qualifications
Bachelors degree from an accredited institution (degree will be verified).
Current state licensure as a Social Services Worker.
Computer skills (email, word processing, and spreadsheets).
Preferred Qualifications
Experience working with in a healthcare setting.
Hearing/Listening, Manual Dexterity, Seeing, Sitting, Speaking.
