This includes outpatient and inpatient services. Includes individual and group counseling regarding the impact of illness and hospitalization. Collaborates with other health care professionals as part of a multi-disciplinary team. Facilitates communication regarding relevant psychosocial issues of patients and families to health care teams through recording in medical records (inpatient and outpatient), consultation and participation in relevant meetings. Functions as a coordinating link between patient, family, hospital, and community resources to assure continuity of care. Represents social work perspective and serves as department representative to designated Hospital and Lombardi Cancer Center committees and community organizations. This may be post-hospitalization or when clinical conditions warrant. Actively cooperates as a participant in the Social Work Department, assuming backup-up and on-call responsibilities, committee work, research, QA/QI, and other duties as appropriate. May provide supervision to LGSW-level social workers. Qualifications: Master's degree of Social Work from an accredited training program. 3-4 years of experience.
