Job Summary and Qualifications
The Social Worker (MSW) is responsible for non-clinical, biopsychosocial assessments and interventions for the inpatient floors and the outpatient emergency department. The MSW performs crisis intervention, patient/family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other identified resource needs. The MSW assists in post-acute care placement on all discharges, primarily with those patients identified with post-discharge behavioral health and patient assistance/resource needs. The scope of this role includes intensive care units, Trauma, Oncology, NICU, Transplant, and ED. The MSW responds to physician, non-discharge planning social work consults throughout the facility.
What you will do in this role:
* You will work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
* You will identify and address barriers to discharge and recovery. These barriers include financial, emotional, psychosocial, medical, and spiritual.
* You will maintain a thorough working knowledge of the rules of Medicare, Medicaid, and private payer regulations and processes. You will possess a thorough understanding of managed-care concepts and length-of-stay management relating to patient and organizational fiscal responsibility.
* You will perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
* You will coordinate and facilitate meetings between the care team and patients/families/caregivers. You will assist patients and families in making difficult decisions while honoring their preferences and values.
* You will intervene with patients/families/caregivers for psychosocial needs, assistance, and support in coping with chronic acute terminal illness. Field Support personnel are critical members of the Social Work Field Team which facilitate and administer the field education (internship) enrollment experience for Social Work students and contribute to the success of the department in equipping students for the profession. Field Support personnel are to be professional in all aspects of the job, including but not limited to, dress, demeanor, and interaction.
Essential Functions and Responsibilities
Field Support personnel conduct student interviews to assess student needs and interests, and exercise discretion in matching students with quality field placements. They coordinate with the Gate team to assess student readiness for field and specific strengths and areas for improvement to consider in arranging placements. They ensure the correct implementation of department policies and practices. In this process, they review and approve placement proposals and accompanying documentation, facilitate inter- and intra-departmental communication about each proposal, manage student and department concerns, and troubleshoot problems as they arise.
Field Support personnel research potential resources, and recruit, process, and onboard community agencies that provide student field placements. They negotiate individual contracts with each agency to ensure that both agency and LU requirements are met. Additionally, there is a public relations function as field personnel maintain frequent contact, perform site visits, and facilitate relationships with those agencies to secure ongoing internship opportunities, and address any challenges to maintaining a partnership.
Field Support personnel interpret and implement policies and procedures related to field enrollment. They are responsible to ensure the proper implementation of pertinent Education and Policy Standards as articulated by Council on Social Work Education. Field Support personnel may also represent the department at various internal LU and external community functions. They may provide trainings on field education policies and practices both internally and externally.
Field Support personnel administer the process for submission of proposals, utilizing CORE field placement software and the Canvas Learning Management System to review and approve such proposals and track essential data. They also communicate with other LU departments such as the Registrar, Contracts Department, Career Center, and University Events, as well as assist the Field Coordinators, Directors of Field Education, and other department leadership with other duties as assigned.
The candidate is to demonstrate excellent interpersonal skills and be able to communicate clearly through oral and written correspondence to students, faculty, staff, and community members. They require the ability to reason, analyze information, exercise discretion, and apply judgment to implement best practices and solve problems. The candidate is to be both professional and graceful while handling multiple projects and interacting with students. Responsibilities
Enhabit Home Health & Hospice is looking for a Licensed Clinical Social Worker (LCSW) to provide professional, comprehensive, family-oriented services to individuals in the home setting.
* Assess patients' and families' psychosocial, environmental, and financial needs.
* Formulate, implement, and evaluates a plan of care in collaboration with patient, family, and other caregivers, and provides case management as appropriate.
* Assist the team in understanding the social and emotional factors related to the patients' health problems.
* Maintain documentation in patient's record per internal regulatory and professional standards.
* Monitor, observe, and evaluate changes and progress in patient's condition and environment. Report changes, progress, or lack of progress to physician and/or nurse case manager.
* Acts as key source in patient situations such as: ineffective patient/family coping and decision making advance directives long term or assisted living placement substance abuse abuse/neglect and/or bereavement.
Qualifications
* Must be a Licensed Clinical Social Worker (LCSW) in the state of employment.
* Must possess one year of social work experience in a health care setting.
* Medicare home health or hospice experience is preferred.
* Must be capable of making independent decisions relevant to client's needs.

Field Support personnel are critical members of the Social Work Field Team which facilitate and administer the field education (internship) enrollment experience for Social Work students and contribute to the success of the department in equipping students for the profession. Essential Functions and Responsibilities
Field Support personnel conduct student interviews to assess student needs and interests, and exercise discretion in matching students with quality field placements. They coordinate with the Gate team to assess student readiness for field and specific strengths and areas for improvement to consider in arranging placements. They ensure the correct implementation of department policies and practices. In this process, they review and approve placement proposals and accompanying documentation, facilitate inter- and intra-departmental communication about each proposal, manage student and department concerns, and troubleshoot problems as they arise.
Field Support personnel research potential resources, and recruit, process, and onboard community agencies that provide student field placements. They negotiate individual contracts with each agency to ensure that both agency and LU requirements are met. Additionally, there is a public relations function as field personnel maintain frequent contact, perform site visits, and facilitate relationships with those agencies to secure ongoing internship opportunities, and address any challenges to maintaining a partnership.
Field Support personnel interpret and implement policies and procedures related to field enrollment. They are responsible to ensure the proper implementation of pertinent Education and Policy Standards as articulated by Council on Social Work Education. Field Support personnel may also represent the department at various internal LU and external community functions. They may provide trainings on field education policies and practices both internally and externally.
Field Support personnel administer the process for submission of proposals, utilizing CORE field placement software and the Canvas Learning Management System to review and approve such proposals and track essential data. They also communicate with other LU departments such as the Registrar, Contracts Department, Career Center, and University Events, as well as assist the Field Coordinators, Directors of Field Education, and other department leadership with other duties as assigned.
The candidate is to demonstrate excellent interpersonal skills and be able to communicate clearly through oral and written correspondence to students, faculty, staff, and community members. They require the ability to reason, analyze information, exercise discretion, and apply judgment to implement best practices and solve problems. The candidate is to be both professional and graceful while handling multiple projects and interacting with students. The candidate must have strong organizational skills and be calm and positive in difficult situations. The candidate is expected to work well both independently and as part of a team.
