JOB SUMMARY:
Duties:
Provides direct clinical social work service, counseling, discharge planning, and case management services to hospital patients and their families including assessment and documentation of emotional, social, financial, environmental or follow-up referral needs.
Works collaboratively with the interdisciplinary healthcare team.
Formulates a psychosocial plan of care related to assessment of patient's unique strengths and needs.
Develops a discharge plan for case load as designated by the team.
Monitors progress toward goals of care and identifies barriers to meeting estimated length of stay.
Facilitates integration with community-based resources to optimize patient health.
Activities are consistent with System mission and values and in compliance with applicable regulations and System policies and procedures.
Partners with the physicians, nursing, and other members of the healthcare team to establish, document, implement and evaluate a plan of care based on the patient's unique health strengths and needs.
Brings psychological assessment skills; acute, long term, and community health expertise; learning theory; and multidisciplinary team expertise to the interdisciplinary team.
Monitors progress of the plan of care and communicates any changes with the care team.
To the extent possible, provides integrated - mind, body, spirit - service to the patient and family through a mutually agreed upon plan of care and patient advocate relationship.
Supports System processes related to the health management of patients across the continuum of care, e.g. QI and/or clinical practice team membership, clinical research, quality monitoring, community networking.
Assesses needs of patients/families and conceptualizes problem situations within social work scope of service using appropriate age-related skills.
Provides direct social work services and counseling to patients and their families. Interprets social, psychological, emotional, financial and family issues for attending physicians and other health care team members.
Completes documentation required in patient's medical record on department forms and reports, and in MIDAS+ and in Cerner.
Demonstrates continued enhancement of professional skills.
Complies with department and hospital policies and procedures.
Supports organization goals to provide the best/optimal care to our patients and families, and community to our highest customer satisfaction.
Performs other duties as assigned.

Job Summary:
The SocialWorker is responsible for performing social work screenings and interventions for patients in the Skilled Nursing Care. Responsibilities:
Interviews all new residents in assigned area within five days of admission, obtains pertinent social history, completes initial social assessment, assigned sections of the RAI/MDS/RAPS and initiates a resident care plan entry within the required time frames as stated in the department policy and procedure manual. Completes timely and comprehensive progress notes and Quarterly assessments specific to the residents' social service needs and problems within required time frames as stated in the department policy and procedure manual.
Attends all care planning meetings for assigned residents and provides psychosocial information to other team members.
Participates in the development and implementation of Social Service resident care policies and protocols and provides advice and guidance in handling special cases or resident needs. Exceeds criteria by volunteering to research, develop and implement new programs in the facility. Involved in the implementation of programs to assist residents and their families with the social and psychological factors of illness, adjustment to the nursing facility and use of facility services and programs. Exceeds criteria by distributing informational items that promote the use of such services.
Participates in the quality assurance activities for the department and promotes the completion of quality assurance studies to identify trends or concerns that may have a negative impact on quality resident care. Works with the quality assurance team to develop plans to improve the delivery of care and perception of quality by residents and families. Exceeds criteria by directing and assuring the consistent completion of monthly quality assurance studies over a six month period. Interpret the department's policies and procedures to employees, residents, visitors, government agencies, etc. Exceeds criteria by developing an information tool (bulletin board, pamphlet) to advertise the department.
Refer resident and families to appropriate social service agencies when the facility does not provide the services or needs of the resident. Exceeds criteria when written information of the referral agency is obtained and provided to the resident and family.
Demonstrates ability to set priorities, work in a crisis situation, and complete required documentation in a timely manner 95-98% of the time.
Attends and participates in Resident and Family Council meetings, documents areas of concern and assists in developing a written social work treatment plan of care to alleviate or diminish identified concerns. Exceeds criteria by initiating a facility committee or sub-committee designed to identify solutions and make recommendations to address the identified concerns.
Maintain a reference library of written material to use in the education of residents and their families concerning the psychosocial aspects of illness, medically related social services, community resources, legal issues, financial resources, orientation and preparation for discharge and orientation to facility programs and services.
Maintains a written record of the resident's complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint.
Demonstrates the ability to suggest or implement creative alternative in providing individualized resident care. Assures that resident's right to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints daily by reporting immediately any suspected or observed incidents of resident abuse per the facility policy and procedures.
Demonstrates the ability to recognize the need to utilize the facility ombudsman for resident concerns.

Program and Role Summary
Wayfinder's Contra Costa Psycho Social Assessment program assesses resource families, including adoptive families, by completing resource family evaluations within county timeframes.
The primary focus on the SocialWorker, is to conduct resource family assessments for relative and non-relative families; and write the assessment in the county contract's allotted time. Psychosocial Assessment (PSA) SocialWorkers may also assist with finalizing the adoption for these families.
This is a part-time position budgeted for 29 hours/week with a varied schedule (weekend and evening availability, as needed). The PSA SocialWorker will provide support to both Contra Costa and Sonoma Counties, as needed.
Primary Responsibilities:
Assesses county-referred applicants to become an approved Resource Family.
Conducts a home environment assessment, and psychosocial assessment, and prepare a written report. Incorporate an assessment of an applicant's mental health and substance abuse into the risk assessment.
Provides ongoing assessments of family needs and makes appropriate referrals to services such as counseling, support groups, and other applicable services and coordinates with county workers and/or other community agencies to identify assistance funding services.
Maintains accurate and thorough records of all services provided and document all assessments, meetings, decisions, and reports.
Communicates needs, services, and progress with the referring agency, service provider, and applicants.
Provides therapeutic interventions on an ongoing basis and during crises.

Qualifications
Education and Experience:
Master's Degree from an accredited university in social work or social welfare, marriage, family and child counseling, child psychology, child development, counseling psychology, or social psychology is required.
Bilingual (Spanish), preferred.
Valid CA Driver License with a minimum of three (3) years good driving history and must be approved by the agency's insurance carrier to drive agency vehicles.

Qualifications
An Average Day:
(Includes, but not limited to)
Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
Provide an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plan intervention based on evaluation findings

Requirements:
Must have a master's degree in social work from an accredited university.

Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
Must understand hospice philosophy, and issues of death/dying.
Certified Hospice and Palliative SocialWorker is desirable

Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
