Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

The primary objective of the VD-HCBS Program is to provide enhanced options for veterans choosing to receive extended care services in the home and community rather than in an institutional setting. Program implementation and support depend on a VD-HCBS Program Coordinator, who will take on responsibility for day-to-day operations of the program, and will oversee the program's effectiveness in meeting its objectives. The VD-HCBS Coordinator will report directly to Non-Skilled In-Home Care Program Manager. Duties include but not limited to:
* Screening/Case Management - Participates in the screening of veterans for the H/HHA and VD-HCBS. Determines bed or space availability in the appropriate setting.
* Provides case management to veterans in these non-institutional settings, visiting the veteran in the community agency according to established policy. Documents according to service level and medical center policy.
* Treatment Planning/Goal Setting - Coordinates with the accepting agency and the treatment team to develop an appropriate plan of care for the veteran, documenting the treatment plan within the veteran's electronic medical record.
* Investigates any concerns of the family or patient while in the community agency and consults with the Non-Skilled In-Home Care Program Coordinator determine if further action is needed.
* Manages the screening of veterans to accurately determine the strengths and limitations of each veteran being referred for care.
* The social worker will consult with the accepting agency to determine an appropriate plan of care for the veteran.
* The social worker is fully responsible for investigating any concerns of the family or patient while in the community agency and consult with the Non-Skilled In Home Care.
* The social worker will facilitate meetings with agency staff when appropriate to allay either VA or beneficiary concerns.
* Supportive counseling - Provides the veterans and their caregivers with ongoing supportive counseling to deal with the psychosocial impact of coping with chronic/disabling illness(es), onset of a catastrophic illness and/or dying, need for non-institutional or institutional services as needed during the course of the veteran's enrollment in a program.
* Establishes and maintains positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies.
* This position requires a current, valid driver's license to drive a government vehicle on a regular and recurring basis to perform the duties of this position.
Work Schedule: 8:00 am - 4:30pm, Monday - Friday - subject to change based upon the needs of the facility.
