What You'll Do
As a key member of the interdisciplinary team, you will:
Clinical Care
Provide individual, family, and group therapy to LTC residents.

Ensures compliance with discharge planning standards as defined by regulatory and accrediting agencies.
14.
SocialWorker/Case Manager coordinates and makes referrals to various agencies as needed based on the patient's plan of care. Advocates for or assists patient in obtaining needed community services/resources.
15.
Knowledge of principles and practices of social work and case management techniques used, including federal regulations and state laws. Has knowledge of current information pertaining to the symptoms, control, treatment, and effects of substance abuse, mental illness and family dynamics in families dealing with chronic illness. Had advanced interviewing skills.
16.
Evaluates insurance data, denials, and quality issues to identify areas of concern and enhance effectiveness of service. Ensures proper notices & reviews are completed for Medicare, OBS and Swingbed admissions.

Cooperates with other departments to ensure that the hospital receives maximum reimbursement for its services.

A Brief Overview

Maintains department policies, procedures, objectives, and quality assurance program, safety, infection and environmental control.

Prepares and maintains patient documentation in EMR.
+ Maintains department policies, procedures, objectives, and quality assurance program, safety, infection and environmental control.
+ Assists with data collection, analysis and reporting as required by the hospital, administrative council, care teams, JCAHO, and other agencies.
+ Conducts individual, marital, family and group therapies as necessary. Coordinate care conferences as needed. Serves as the social work representative in multi-disciplinary team approach to patient care.
+ Utilizes research findings to improve outcomes and support, and provide quality of life for patients and their families.Works collaboratively with Case Management in providing coordinated services, which facilitates continuity of care for our patients.
