Under the direction of the Program Coordinator in partnership with community providers, the Licensed Social Worker (LMSW) will provide oversight and direct services to individuals experiencing a crisis. Essential Duties: Addresses referrals from community providers, including referrals via warm hand-offs. Leads daily huddles at change of shifts. Provide assessments to individuals in crisis. Work in collaboration with community providers, team members, and external partners. Completes and signs off on required documentation and paperwork implementation in a timely manner. Demonstrates cultural competency and commitment to individuals. Maintains a Trauma-Informed environment. Demonstrates commitment to the philosophy of the agency. Acts as an advocate and helps individuals with linkages to supports. Perform crisis intervention and assessments in community based settings. Other duties assigned. Working closely with the Westchester County Department of Community Mental Health, community providers and surrounding Police Departments in providing services to individuals who are in crisis, with the goal of intervention, assessment and linkage to help individuals live safely and prevent future crisis. Ideally, the candidate will be flexible and work as part of a cohesive team that provides 24/7/365 mobile crisis intervention. The ideal candidate will possess the following: Master's degree in Social Work or Mental Health Counseling (required). One of the following certifications certifications/licenses: LMSW or LMHC license( not required upon hire, but must have ability to obtain license must be obtained within 6 months of hire) or Certified Psychiatric Rehabilitation Practitioner, Licensed Marriage & Family Therapist, Licensed Occupational Therapist, or Registered Professional Nurse. NYS Drivers license required. English/Spanish a plus Crisis intervention and response skills a plus. Experience with community-based services. Work well independently and within a team environment.
