Job Description:
The social work care advisor provides supportive advocacy to patients and their families as part of an interdisciplinary health care team to overcome barriers to progression-of-care. This individual will provide both direct care management to a high-risk population, as well as provide clinical consultation to other members of the care team. The social work care advisor will work closely with providers, nurse care managers, case management, and other CoxHealth staff in order to have a patient-centered approach for coordinating the healthcare needs and wellness goals of our patients.

* Responsible for program development, recruitment, and development of Medical Foster Homes (MFH), recruitment and placement of Veteran residents, case management, development of policies and care processes, and coordination with HBPC interdisciplinary treatment teams.
* Serves Veterans who tend to have serious, frequent and severe crises, lack of family or an adequate community support networks, lack of self-monitoring; frequently fails to comply withinstructions and treatments; have significant deficits in coping skills andrequire continuing psychosocial and medical support
* Responsible for marketing the MFH model of care and reaches out to community groups and agencies to promote the foster care program
* Interviews and conducts psychosocial assessments; interviews Veteran and family members and/or significant others to establish facts about the Veteran's unique situation; presenting problems and their causes, and the impact of such problems on the Veteran's functioning and health as a part of a comprehensive psychosocial assessment; assesses high risk factors, acuity and need for services. The socialworker interprets and explains VA treatment and benefit programs
* Develops psychosocial treatment plans in collaboration with Home Based Primary Care (HBPC) interdisciplinary team, including goals for psychosocial clinical treatment

Key therapist responsibilities may include:

Requirements
Must have a Master's degree in Social Work
Must have successfully completed CMS's Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).
Must have at least two (2) years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities.
Must maintain current licensure to practice as an LCSW
Demonstrated history of independent decision-making skills to direct and effectively manage the survey process.
Ability to set priorities independently and collectively in performing survey tasks.
Ability to openly discuss conflicts/controversy, and to seek assistance when appropriate to make decisions and resolve conflicts.
U.S. Citizenship is required for this position
Ability to travel up to 75% of the time on a regular basis is required

Key Responsibilities:
• Provide individual, group, family, and/or other practical counseling in various settings to meet client/patient needs and treatment goals.

• Assist clients/patients through the transition process and complete final correspondence such as transition summaries and plans.
• Represent the agency within the community in a professional manner.
• Handle crisis situations as they arise and coordinate emergency services as needed.
• Facilitate group counseling, group education, family therapy, and individual sessions, including specialty services such as trauma and co-occurring therapy.
Education, Experience, and/or Credential Qualifications:
• Must possess at least a Master's degree in Social Work, Psychology, Counseling, or a related field.
• Holds licensure with the applicable board of professional registration for the state of practice as a professional counselor or socialworker.
• Licensed Professional Counselor (LPC), Licensed Clinical SocialWorker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
• Must be willing to become licensed in additional states beyond the current state of licensure.
• Must be comfortable providing telehealth services to individuals served (training will be provided).
Additional Qualifications:
• Current driver's license, acceptable driving record, and current auto insurance.
