Quality of Clinical Services:
The Medical Social Worker III provides quality counseling services by assessing patient/family system needs then developing and implementing treatment plans in accordance with departmental and medical center policies, and in compliance with the I standards of the social work profession. As a Per Diem Home Health Medical Social Worker, you will play a vital role in providing care to our patients in various areas of San Diego County. Description The Center for Elders' Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, including an Adult Day Health Center, and promoting autonomy, quality of life and the ability of individuals to live in their communities.The Position: Under the general supervision of the Social Work Supervisor, the Social Worker is a mental health professional, helping the frail and elderly PACE participants find solutions to a wide range of social problems, serves as an integral member of an Interdisciplinary Team (IDT) and is responsible for performing psychosocial assessments, developing and implementing plans of care, and facilitating communication between the participant, family and CEI staff. Serves as an interdisciplinary team (IDT) member responsible for developing and implementing a comprehensive, integrated plan of care, monitors progress toward objectives by evaluating behavioral adjustments, and communicates participant changes to team members.
Participates in regularly scheduled behavioral management meetings with other mental health professionals.
Attend ongoing psychosocial rounds with consulting psychiatrist; presents cases as indicated by IDT.
Completes initial assessments, re-assessments, and care plan updates for participants while continually evaluating the participants' social service needs and their families.
Complete chart notes accurately and timely.
Makes referrals to community resources, including those necessary for financial management, benefit eligibility, and to address related participant needs.
Review historical information, including requisitions for prior medical records; make an effort to resend for these if not in the chart at the time of initial psychosocial assessment, including requisition of prior records and information.
Manages assigned caseload; prepares narrative and other reports, documents, and correspondence regarding client status; documents case files.
Responds to, investigates, and reports complaints of elder abuse.
Conducts 30-day conferences and annual reviews, and as-needed family and/or caregiver conferences.
Develops and maintains working relationships with other agencies, such as the Department of Social Services, psychiatric facilities, hospitals, social agencies, and may participate in hospital and/or SNF discharge planning.
Maintains the confidentiality of all company procedures, results and information about participants, clients or families in conformance with Health Insurance Portability and Accountability (HIPPAA) principles.
Makes home visits, hospitals, and travels to other places to meet with participants and service providers.
Performs related duties as assigned.
QUALIFICATIONS:
Masters' degree in Social Work from an accredited school of social work
One year experience working with a frail and/or elderly population preferred.
Demonstrated ability to work in a multicultural and multidisciplinary team environment.
Ability to work with minimal supervision.
Excellent oral and verbal communication and meeting facilitation skills.
Experience working with severe mental illness is desirable.

Incumbents must be able to visit clients in their homes, in the field, and other settings to perform complete assessments of risk factors, and be able to visually observe injuries and the client's environment as mandated by the State. Positions in Children and Family Services (CFS) require ability to lift and carry children up to fifty (50) lbs.
Travel: Travel throughout the County is required. A valid California Class C Driver License is required to carry out the essential job-related functions of this position. Proof of automobile liability insurance and clean driving record must be maintained. (Mileage reimbursement is available at current IRS rates).
Availability: Some positions are assigned to 24 hour programs which may require incumbents be available to work the following shifts: day, swing, night, rotating, and/or weekend. Make sure to indicate a willingness to work these alternate schedules on your application.
Promotion: Incumbents in the class of Social Worker II Trainee are eligible for promotion to Social Worker II ($4338- $5952 monthly) upon completing the trainee period and receipt of a satisfactory Work Performance Evaluation. Incumbents must be promoted within twelve (12) months of hire or be terminated.

Education: Thirty (30) semester (45 quarter) units of completed college coursework, half of which must be upper division, in behavioral science, psychology, sociology, social services, human services, behavioral anthropology, gerontology, cultural and gender studies, or human development. (Note: Candidates with degree titles not specified above (e.g. Criminal Justice, Political Science, etc.) may be considered but must provide coursework for review.)
Required Documentation: A list of completed coursework or copies of transcripts must be submitted with the application unless the applicant possesses a bachelor's degree (or is within 90 days of degree completion) in one of the specified areas listed above. Job offers may be made contingent upon degree completion; verification of qualifications will be required prior to hire. Qualifying degrees or coursework must be completed at a college or university accredited by an accrediting association recognized by the United States Department of Education. BSW/MSW degrees or coursework must be approved by the Council on Social Work Education.

Proof of the required completed coursework and/or degree(s) will be required at the time of the interview.
Bilingual Skills: Applicants fluent in American Sign Language, Arabic, Korean, Mandarin, Spanish, Tagalog and/or Vietnamese are strongly encouraged to apply. Additional compensation is associated with positions requiring the use of bilingual skills. Testing of bilingual skills may be required to receive additional compensation.
Social workers must have strong interpersonal skills enabling them to work positively and compassionately with clients of various backgrounds who may be experiencing difficult life circumstances and stressors such as homelessness, illness, behavioral health issues, or family crises. Crisis intervention experience is highly desirable. Proficiency with Microsoft Office Suite and ability to learn/use departmental computer systems a plus.
