The LCSW is responsible for conducting comprehensive diagnostic evaluations including clinical interviews and administering the standardized assessment tool, ADOS-2 and administering and ADIR for children suspected to have autism. This role includes direct administration of the assessment with the learner, engaging with families throughout the diagnostic process, and completing high-quality written reports that support accurate and timely diagnosis.
You make your own schedule, and work can be done remotely, except the ADOS appointment does need to be conducted at our Kalamazoo East center. Position requires LCSW to complete a minimum of 1 assessment per week with report writing (approx. 10 hours).
Key Responsibilities:
Assessment Administration
Conduct assessments using standardized tools such ADOS-2

Meet with families before, during, and after assessments to gather history, provide education, and explain next steps, if applicable

Report Writing & Documentation
Complete detailed diagnostic reports summarizing assessment results and diagnostic considerations
Ensure documentation is accurate, thorough, and submitted in a timely manner
Collaborate with clinical team members to integrate multidisciplinary input when applicable
Collaboration & Compliance
Attend team meetings, case reviews, supervision, and training sessions
Maintain compliance with HIPAA, company policies, and ethical standards of care
Keep up to date on relevant diagnostic tools and continuing education in autism spectrum disorders
Qualifications:
Master's Degree, with associated Licensure, in Social Work, Mental Health Counseling, Applied Behavior Analysis, or related field
Current Michigan license
Certified with administering the autism diagnostic tool ADOS-2 and ADIR
Exceptional written and verbal communication skills
Strong clinical judgment and ability to engage with children and families

Responsible for the delivery of social work services for a variety of patients including infant, pediatric, adolescent, adult, and geriatric populations to all medical areas of the hospital. * Complete legal paperwork to purse involuntary psychiatric hospitalizations and probate court guardianship hearings. Collaborate with hospital attorney and testify when required.
* Specialized knowledge of chronic illness and the impact on patient mental health.
* Knowledge of community service organizations (i.e. CMH, DHHS, shelters, transportation, etc.) and the types of assistance they provide.
* Work by consult (i.e., physician, RN, patient, family, etc.).
* Participates in multidisciplinary rounds and provides necessary report to advance patient care.
* Collaborates with other members of the patient care team to understand each situation from different perspectives and recommends or provides immediate treatment and solution focused interventions.
* Identifies needs, develops plan of care, and makes referrals to community agencies and other healthcare specialists and plans for patient discharge to ensure the appropriate follow-up treatment and support is available and provided to support a safe discharge.
* Thoroughly documents each case and enters appropriate information in patient's electronic medical record in an effective and timely manner.
* Participates in the development of departmental policies, procedures, and protocols.
* Identifies and provides for learning needs of patients, families, and health care co-workers.

Optimal Care Hospice Medical Social Workers are trained professional Social Workers who assist patients and families living with terminal illnesses. QUALIFICATIONS
Required Education and Experience:
Master's degree in social work.
Full approval by the Michigan Department of Education as a Licensed School Social Worker or have received a recommendation for temporary approval from a Michigan school of social work training program by obtaining an SSW-310 form.
If full or temporary approval from MDE is not in place a waiver can be submitted to MDE allowing the social worker to enroll in the necessary classes to obtain the SSW-310.
With the waiver in place, GRPS has agreed to waive the requirements under Article 14, Section J.1.a which requires two years of employment at GRPS before a GREA member is eligible for tuition reimbursement. This waiver only applies to the classes required to obtain the SSW-310 certification.
Candidates that access tuition reimbursement will also be required to sign an agreement indicating they will remain employed by GRPS for three years after obtaining their SSW 310 certification. Reimbursement will be required if the candidate does not fulfill this requirement.

SUMMARY OF JOB RESPONSIBILITIES
The Social Worker is responsible for the implementation of social work services for the patient/family/caregiver from the time of admission through the date of discharge, transfer, or death. The Social Worker is a member of the Interdisciplinary Team and provides interventions to meet the psychosocial needs of the patient/family/caregiver.
RESPONSIBILITIES AND DUTIES
Provides psychosocial assessment, brief therapy, crisis intervention, education, and planning with patient/families caregivers to promote optimal functioning as they are coping with living with a terminal illness. Additionally, the Social Worker assists with the Comprehensive Assessment for each patient, conducts an Initial Bereavement Risk Assessment, and determines potential needs for the bereaved following the death of the patient.
Interventions include education and counseling to the patient/family/caregiver related to adjustments to illness or terminal prognosis, anticipatory grief, stress, care giving concerns, environmental and financial needs, coping mechanisms, and other issues as identified in the patient's Plan of Care.
Participates in the development of the Plan of Care, updating the Interdisciplinary Team of the patient/family/caregiver psychosocial status during report and team meetings.
Provides information and referral related to community resources for patient/families/caregivers based on needs identified in the initial/ongoing assessment and Plan of Care.
Provides education and assistance with Advance Directives as needed.
Serves as liaison between patients/families/caregivers and community agencies, when patient consents to contact with a particular agency.
Serves as a member of the Interdisciplinary Team and maintains collaborative relationships with agency personnel to support patient care.
Assists physician, volunteers, and other team members in understanding significant social and emotion al factors related to health problems and death/dying issues.
Participates in discharge planning when needed. Assists with alternatives to the care giving situation, including use of shift care or placement in Adult Foster Care Homes, Assisted Living, or Nursing Homes.
Provides community education and facilitates groups when assigned.
Shares in the responsibilities of providing weekend on-call and weekend Social Work visit coverage to patients and caregivers.

QUALIFICATIONS & EXPIERENCE
Education & Experience: Master's degree in Social Work (MSW) with current LMSW or LLMSW licensure and at least one (1) year of healthcare experience; or Bachelor's degree in Social Work (BSW) with current LBSW licensure and a minimum of five (5) years of healthcare experience.
Prefer two (2) years' experience. A minimum of one (1) years' experience in health care, Hospice experience is preferred.
Experience with issues concerning the terminally ill and death/dying.
Presents a negative TB skin test and/or chest X-ray and other tests as required by Centrica Care Navigators.
Maintains a valid Michigan driver's license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
Remains up to date on all routine immunizations required by LARA and agency policy.

Duties and Responsibilities:
Provide group and individual counseling sessions and classroom support to special education students as mandated per student IEPs, as well as progress monitor IEP goals associated with mandated services and participate in student IEP meetings.
Provide group and individual counseling sessions to general education students, as well as monitor the students' responses to the interventions identified.
Develop and monitor student responses to behavior intervention plans, including providing support and consultation to general and special education teachers.
Complete psychosocial assessments to assist in the determination of special education services.
Conduct functional behavior assessments and communicate the results to all team members in parent-friendly language
Complete risk assessments on students referred for school social work services.
Establish relationships with both students and parents and participate in all aspects of school planning.
Assist in identifying needed community resources for students and make appropriate referrals to resources to facilitate educational and behavioral interventions recommended for student success
Provide staff consultation on behavioral-emotional-environmental issues affecting student participation in the learning process and collaborate with school staff and other school system personnel in implementing strategies to promote student learning.
Provide crisis intervention services.
Maintain state-required clinical records and submit appropriate documents for statistical reports with adherence to program standards in school social work.
Qualifications:
Hold a master's degree in social work; exceptions apply in North Carolina and Ohio
Hold a valid social work license issued in the state you are applying for.
Candidates for positions in Michigan must have state approval as a school social worker.
Knowledge of special education processes and procedures is preferred.
Candidates with experience working with at-risk students is preferred if applicable to the school.
