General Purpose The primary purpose of your job position is to assist in the administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, the nurse supervisor or charge nurse, and the Director of Nursing Services. The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of this facility, and the requirements of this state.
Essential Duties
• Follow the policies and procedures of the facility governing the administering of medications to residents.
• Note and report errors in the administration of medications
• Assist in developing and implementing procedures or programs of the facility that seek to determine what medication errors are taking place, when, where, and why they occur, and how they can be prevented.
• Perform administrative requirements, such as completing necessary forms, charts, reports, etc., and submit these as may be required.
• Maintain a friendly, productive, working relationship with other members of the department of nursing, attending physicians, the Medical Director, and pharmacists.
• Report any complaints or grievances made by residents to the Director of Nursing Services.
• Participate and cooperate with any facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services.
• Participate in Quality Assurance programs, the Pharmaceutical Services Committee, and any other facility committee or program that affects the medication pass and seeks to improve the performance and accuracy of the medication process.
• Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for resident use by the attending physician or the Medical Director.
• Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy or pharmacist
• Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Director of Nursing Services, including the documentation requirements.
• Follow the labeling policies and practices of the facility.
• Ensure that all medications administered are properly labeled.
• Follow facility procedures in regard to charting medications.
• Assist in documenting and removing medications that are discontinued by the attending physician.
• Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Director of Nursing Services.
• Ensure that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you.
• Follow facility procedures in holding medications for residents who are transferred from the facility or when the attending physician discontinues medications
• Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal.
• Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents.
• Verify the identity of the resident before administering the medication treatment.
• Accurately measure, record, and report the vital signs of residents.
• Follow the facility's procedures allowing residents to self-administer his or her medications.
• Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.
• Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your certification on a current status
• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Report any known or suspected unauthorized attempt to access facility's information system.
• Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.
• Follow work assignments, and/or work schedules in completing and performing your assigned tasks.
• Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents.
• Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

Follows re-fill process for medications.
Participates in the development of the Service Plan and monthly updates.
Takes and records temperature, blood pressure, weight, pulse and respiration rates.
Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.

Location: Iraq
The Medical Laboratory Technician (MLT) is responsible to provide comprehensive health services to DOS, DOD, and contractor personnel stationed in Iraq. The MLT performs lab tests and procedures, mostly comprising of microscopic, bacteriological, hematologic, immunologic, and chemical diagnostic analyses on samples of tissue, blood, urine, stool, synovial fluid and semen. The MLT looks for clues to the absence, presence, and extent of the causes and progression of diseases through accurate and efficient lab tests.
Key Responsibilities:
Collect and process biological specimens for laboratory testing.
Conduct routine and specialized tests in hematology, microbiology, chemistry, and blood bank.
Maintain laboratory equipment and troubleshoot any technical issues.
Adhere to safety protocols and maintain a clean laboratory environment.
Record and report test results accurately and in a timely manner.
Collaborate with pathologists and other medical staff to interpret results and ensure quality patient care.
Maintains safe and clean working environment by complying with procedures, rules, and regulations.
Actively participates in Mass Casualty exercises and events.

The Lead Medical Scribe supports back office, team members and main role as a scribe to multiple Medical Providers, including Pediatric and Adult Medicine. It will be the responsibility of the medical scribe to annotate any dictated or written information for the treatment of patient following all local, state, and federal guidelines for documentation into the electronic health record (EHR). The medical scribe will be responsible for clearly recording each patient's symptoms, history, physical exam, and documenting the diagnosis as well as treatment plans. The medical scribe is not a direct patient care role. Only Licensed Independent Providers (MD, DO, OD) are allowed to use scribes.

Lead- Specific Tasks and Duties:
Lead Scribe has the ability to delegate as necessary to Medical Scribes. Lead Scribe has authority to invoke disciplinary action including verbal warnings in conjunction with the Clinic Manager and/or Regional Clinic Director.
Lead Scribe plans and delivers training and in-services with input/support from Clinic leadership including but not limited to Medical Director, MA Supervisors, Lead RN, Front Office Supervisor, Clinic Manager and Regional Clinic Director.
Lead Scribe will work with the Clinic Manager regarding employment (hiring/terminating) of Medical Scribes.
Lead Scribe is responsible for assisting Clinic Manager with Medical Scribes performance evaluations.

Scribe-Specific Tasks and Duties:
At the start of the visits all scribes must check the “document scribed” checkbox on the intake template at the top center.
The primary role of the Medical Scribe is to assist the Provider with documentation of each patient's medical chart during his/her visit.
The scribe accompanies the clinician into the patient examination area in order to transcribe a history and physical examination as given by the patient and provider.
The scribe, under the direction of the provider, transcribes patient orders, including laboratory tests, radiology tests, referrals etc. When transcribing orders start with “Per. [PROVIDER NAME, TITLE] order.”
The scribe documents any procedures performed by the provider or nurses, in the EHR.
The scribe transcribes any consultation or discussions with family and/or the provider.
The scribe completes the patient's chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient's workup complete so that the provider is able to make his/her decisions regarding that patient.
The scribe lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the provider.
The scribe does not directly assist with patient care. The scribe documents orders generated by the provider.
The scribe assists the clinician and support staff with paperwork, telephone calls, faxing, and chart abstraction.
The scribe is able to process portal communications and either task to appropriate staff or get information from the provider to respond to the patient in a timely way.
Scribes are prohibited from the Following Tasks:
Medications: A scribe cannot prescribe medications. Scribes cannot stop, start, or make any manipulation to any medications in the "medication module”.
Orders: A scribe cannot order any in-house medications that are injected, ingested, or inhaled into the body including nebulizer treatments. The expectation being immunizations.
Document Sign Off: The provider should always review the record and attest to its correctness and completeness. At the end of the day or shift, the provider should review the scribed documentation to ensure all events were recorded accurately. The provider generates the document, creating the completed "Master IM." The scribe does not.
Patient Care: The scribe is not directly involved with patient care; however, he/she may be asked to give patient paperwork during visit or discuss discharge instructions with a patient. They interact with patients on a limited level. For exceptions to this section, please see the “Optional Trainable Skills” section below.
General Documentation: A scribe cannot document any of his/her personal objective findings or be involved in any of the medical decision-making. A scribe cannot act independently; instead, he/she must simply document the provider's dictation and or/activities during the visit.
Optional Trainable Skills:
With the permission of medical/operational leadership and participation in an in-house training/competency program, a Scribe can be trained to perform vitals, room patients.

**Responsibilities:**
+ Performs clinical laboratory specimen handling, processing, testing, analysis, and records maintenance according to standard operating procedures (SOPs)

+ Assists with quality control and quality assurance activities

Clinical Laboratory Scientist (AHCL)
* Accommodates STAT orders consistently reporting within one hour or as soon as available. Calls all critical values and critical test results to providers within 60 minutes of being available. Completes all required documentation.
* Assists Director of Laboratory Services in ensuring that all lab procedures and safety protocols are followed. Alerts supervisor when abnormalities or irregularities in the test are encountered while maintaining a workload productivity of at least 90%.
* Assists in ensuring that the department is continuously prepared for all regulatory surveys.
* Backs up Laboratory Assistants by obtaining venous blood and arterial blood samples as required, following aseptic techniques. Checks for patient identification by using at least two patient identifiers (patients full name and date of birth) before drawing sample and compares all demographic information to ensure accuracy.
* Completes required tests and/or observations of patients specimens, blood samples or other products required for testing; results reflect attention to detail and accuracy.
* Follows established policy and procedures when drawing venous and arterial samples. Allows for adequate time (direct pressure) to ensure hemostasis and calls for assistance if unable to obtain blood sample after two attempts. Insures validity and accuracy of results by continually checking QC and properly maintaining instruments. Contacts and follows through with AH Biomedical, vendors and/or customer technical service when mechanical problems occur. Maintains adequate levels of reagents, calibrators, controls and supplies. Maintains and meets expectations on time for all competencies, license, certifications and education requirements as outlined by local administration, Adventist Health (AH), The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS), and all other regulatory agencies.
* Supervises Laboratory Assistants as needed and supports the Director of Laboratory Services as assigned.
* Verifies that appropriate specimen is being analyzed for proper patient and for indicated test comparing against patient test orders. Completes test order entry accurately.
* Willingly performs other duties and innovations as assigned.
