Memorial Rehabilitation Institute provides highly specialized care ranging from complex multiple trauma, stroke and brain injury to physical and speech therapy. Inpatient care is offered at Memorial Regional Hospital South, with outpatient care available in five different locations. We also have a home health team and resources for long-term care at Memorial Manor, while also offering therapy for pediatric developmental diagnosis through Joe DiMaggio Children's Hospital.
Summary:
Evaluates and treats patient populations for physical therapy needs.
Responsibilities:
Supervises activities of physical therapy assistants, students and other support personnel.Designs and implements a plan of care which is appropriate to issues identified, required frequency of treatments, and involves the patient and family.Plans or assists with patient discharge from therapy services.Educates and counsels patients and families regarding treatment plans and progress. The **Home Health PT** provides hands-on care in the home for a group of patients consistently in accordance with the principles and practices of physical therapy. **Responsibilities include:**
+ Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
+ Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
+ Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
+ Manages the activities of the multi-disciplinary team providing care to the patient.
+ Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
+ Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
+ Interprets agency policy to patients/families.
+ Reviews patients visits weekly and provides plan to PCS and team assistant, including anticipated weekend visits.
+ Informs team assistant of any schedule changes and communicates caseload and patient care issues to Patient Care Supervisor

**Acute Physical Therapist Responsibilities:**
