Summary:
Coordinates and provides clinical pharmacy services to patients and works collaboratively with the interdisciplinary team. Provides leadership and oversight to pharmacy technicians and other support staff for the distribution, dispensing and compounding of sterile and non-sterile medications and related supplies in compliance with accreditation, legal, regulatory and safety requirements.
Responsibilities:
Provides Clinical Pharmacy services which may include but is not limited to: renal/hepatic dosage adjustment, pharmacokinetic dosing, anticoagulation monitoring, total parenteral nutrition, antimicrobial stewardship.Reviews, interprets and evaluates medication orders, labs and other clinically relevant information to ensure safe and optimal therapeutic outcomes and documents interventions appropriately.Manages and recommends drug therapy regimens as a part of an interdisciplinary team to provide patient and family centered care.Provides drug information to patients and healthcare providers including medical, nursing, and pharmacy staff.Supervises and delegates tasks to technicians and other support staff to ensure timely prescription/order verification, preparation, and distribution of medications, compounding and/or supplies and coordinates workflow as needed.

Infusion Pharmacist Responsibilities:
* Reviews and verifies all prescription medication orders received from the prescriber or provider's agent and evaluates appropriateness and completeness of prescribed therapies in a home setting.
* Assesses the patient at the initiation of IV drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy.
* Ensures drug, related necessary supplies and all patient collateral (education materials) are accurately provided to our patients
* Facilitates communication with the patient/caregiver, physician, provider, home health nursing staff, and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure optimal clinical outcomes.
* When required, must be able to compound intravenous and specialty pharmaceuticals products using aseptic techniques according to established Florida Board of Pharmacy, federal agencies, USP sterile guidelines and company policy.
* Must be knowledgeable of the proper standards, use and handling of both sterile and hazardous medications.
* Ensures compliance with all FDA, DEA, Florida Board of Pharmacy, OSHA and other regulatory and accrediting entities. Follows all company and department policies and procedures.
* Provides direct and indirect oversight of pharmacy technician and pharmacy support staff.

* 1+ years of practicing Pharmacist experience working in home infusion, hospital, or related healthcare environment
* Knowledgeable in various infusion and specialty related therapies and ability to work through the complexity of therapy related orders, and the ability to fill medication orders accurately and in a timely manner
* Willing and able to take calls after hours, weekends and holidays as scheduled
* Experience working collaboratively with an intradisciplinary team and independently with minimal supervision
* Preferred:
* Strong clinical experience with IV Antibiotics, Inotropes, Parental & Enteral Nutrition, Immunoglobulins and/or Biologics
* Home Infusion experience and/or specialty pharmacy experience
* Sterile Compounding experience
