    Job Description The QIDP Specialist is responsible for the overall coordination, implementation, and monitoring of individualized services for people with developmental disabilities in accordance with the standards and regulations of the New York State Office for People with Developmental Disabilities (OPWDD). The QIDP ensures that each person's Individualized Service Plan (ISP) and Individualized Program Plan (IPP or Staff Action Plan) are person-centered, compliant, and effectively implemented to promote independence, community inclusion, and quality of life. ESSENTIAL DUTIES AND RESPONSIBILITIES: Serve as the primary advocate and coordinator for the individuals assigned on their caseload. Develop, implement, and monitor Individualized Program Plans (IPPs) or Staff Action Plans consistent with each person's Life Plan and OPWDD's Person-Centered Planning Process. Participate in Interdisciplinary Team (IDT) meetings, Life Plan reviews, and case conferences to ensure that supports are aligned with assessed needs and personal outcomes. Ensure documentation of habilitation and service delivery meets OPWDD, Medicaid, and agency compliance standards. Monitor program data, progress notes, and service documentation to ensure the quality and integrity of habilitative services. Conduct on-site observations and staff coaching to support implementation of behavior support plans, nursing plans, and habilitative outcomes. Collaborate with clinical and direct support staff to address barriers to progress and ensure continuity of supports. Complete monthly and quarterly progress summaries, data reviews, and other required reports. Provide staff training and mentoring in person-centered approaches, habilitation documentation, and data collection. Maintain compliance with all OPWDD regulations, agency policies, and applicable federal/state laws. Participate in internal and external audits, program reviews, and quality improvement initiatives. Support individuals in self-advocacy, informed choice, and achieving meaningful life goals. QUALIFICATIONS: Education: Bachelor's degree or higher in a human services field (e.g., psychology, special education, rehabilitation, social work, or related discipline) required per OPWDD regulations. Experience: Minimum of one (1) year of experience working directly with individuals with intellectual and developmental disabilities. Knowledge: OPWDD regulations (14 NYCRR Parts 624, 625, 633, 635, and 635-10). Person-Centered Planning and HCBS Waiver service documentation requirements. Behavior support principles, habilitation documentation, and data analysis. Skills: Strong organizational and communication skills. Ability to write clear, measurable goals and objectives. Proficient in electronic health record (EHR) or documentation systems (e.g., Therapy, Foothold). Ability to work collaboratively across disciplines. Work Location: In person
