Personal Service Coordinator (Temporary)
35 hours per week / $24.00 per hour

Summary: Under direct supervision, serves as the primary support and case manager for young adults between the ages of 18-25 who are experiencing mental illness, to support them in transitioning into adulthood by helping them achieve their quality of life goals including, but not limited to stable place to live, employment, education, and relationships.
Essential Duties and Responsibilities
: This is not an exhaustive task list, and other duties may be assigned as deemed appropriate or necessary by the supervisor or program director. Develop a Recovery Plan and/or Personal Service Plan with each member that identifies his/her personal needs and goals
Mediate and advocate on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements etc.
Visit members in their place of residence, hospital or jail on an as-needed basis, in order to help them live successfully and to prevent them from becoming homeless.
Transport members to appointments and on errands when appropriate.
Oversee and monitor members' individual and group activities both inside the program and out in the community.
Provide coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such as money management, grooming and hygiene, meal preparation, public transportation and other independent living skills
Devise and implement strategies for outreach to members who tend to isolate themselves.
Knowledge of community resources, housing, social security and general relief systems and ability to navigate within these systems.
Knowledgeable and practices in motivational interviewing and harm reduction.
Assist members in achieving community integration and employment goals.
Participate in staff and community meetings and in the day-to-day operations of the program as needed.
Required to meet the monthly billing requirements of 85 hours and enter notes into the Electronic Health Record within 24 business hour period.
Maintains all records and documentation needed for program compliance and accountability in accordance with MHALA standards and funding source requirements including but not limited to recovery plans, progress notes, assessments, and outcome measurements.
Maintain understating of HIPAA regulations and complies with these regulations when working with members.
Position Requirements:
High school diploma or GED required.
One year of experience providing psychosocial rehab related services to a disabled population.
Bachelor's degree in a human service or related field preferred.
Ability to provide a welcoming environment and skill in engaging new members.
Must possess a high level of tolerance, patience and understanding for individuals who present for services with urgent multiple needs, issues, and problematic behavioral characteristics.
Must have good basic English writing skills, good basic computer skills and be highly organized. Must have excellent communication skills.
Must be able to have the ability to juggle multiple tasks without losing productivity.
Must function well in a team-oriented approach to member services.
Knowledgeable in computer applications such as Microsoft Word, Excel, and Outlook.
Must possess a high level of patience and tolerance for a work atmosphere that is very busy and highly unstructured.

SUMMARY OF POSITION
Reporting to the Supervisor of Clinical Services, the Care Coordinator is responsible for linking patients with appropriate health and social services to address specific needs and achieve treatment goals. This patient-centered role complements clinical services, such as counseling, by addressing social determinants of health that may negatively impact treatment success and overall quality of life. The Care Coordinator ensures that patients receive support to increase self-efficacy, self-advocacy, basic life skills, coping strategies, and self-management of biopsychosocial needs.
DUTIES AND RESPONSIBILITIES
Connection:
Establish and maintain high-quality referrals and linkages to community resources, including housing, educational, social, prevocational, vocational, rehabilitative, and other services.
Actively assist patients with applications and maintenance of public benefits (e.g., Medi-Cal, Minor Consent Program, General Relief, and County-funded programs).
Support patients experiencing homelessness by helping them access the Coordinated Entry System (CES) and completing necessary intake and assessment documentation.
Develop relationships and protocols with external service providers to ensure patients have actual access to necessary services rather than just providing resource lists.
Ensure benefits are transferred when patients move across counties.
Coordination:
Facilitate patient transitions between Substance Use Disorder (SUD) Levels of Care (LOCs), including scheduling assessment appointments and coordinating documentation transfers.
Coordinate with physical health providers, managed care health plans, community health clinics, and mental health providers to ensure integrated care.
Work closely with county and state entities such as DPSS, DCFS, Probation, and Housing Providers to align health services with social services.
Follow up with patients post-hospital discharge, emergency room visits, or transitions from residential care to ensure continuity of care.
Track referrals until confirmation of patient enrollment in receiving treatment agencies.
Communication:
Serve as the primary point of contact between SUD care, mental health care, medical care, and social services.
Communicate patient updates and treatment progress to service providers, county agencies, courts, and other relevant stakeholders.
Advocate for patient needs with healthcare and social service providers, ensuring that patients receive timely and necessary services.
Educate patients on their rights and responsibilities related to care access and service coordination.
Provide required documentation and correspondence, including letters for legal and social service agencies verifying patient participation in SUD treatment.
Special Population Considerations:
Address the unique needs of special populations, including individuals experiencing homelessness, persons with co-occurring disorders (CODs), pregnant and parenting women (PPW), youth, LGBTQ+ individuals, and those involved with the criminal justice system.
Advocate for patients in school, court, or correctional settings by preparing necessary reports, letters, and in-person representation.
Coordinate reentry services for justice-involved individuals, ensuring seamless integration into community services.
Documentation and Compliance:
Utilize the ASAM CONTINUUM assessment to determine patient needs and develop an individualized care coordination plan.
Maintain accurate and timely documentation of Care Coordination activities in Progress Notes and Treatment Plans.
Ensure that care coordination services are provided per county, state, and federal regulations, obtaining necessary Release of Information (ROI) documentation.
Monitor patient progress and adjust care coordination strategies as needed to align with treatment goals.
EXPERIENCE/QUALIFICATIONS
Bachelor's degree in social work, psychology, public health, or a related field, preferred (Master's degree preferred).
Minimum of 2 years of experience in care coordination, case management, or a related field in behavioral health or social services.
Knowledge of SUD treatment, mental health care, and social service systems.
Familiarity with Medi-Cal and other public benefit programs.

Builds and grows these services so they are effective clinically, operationally, and fiscally. Develops strong partnerships with agencies and key stakeholders in the community that could serve as referral sources and experts to grow the programs.

Management Duties
Oversees recruitment, hiring, onboarding training, performance evaluation, and professional development for program staff across the Crisis & Disaster Care Department.

Identifies training needs and ensures the development, delivery, and evaluation of relevant trainings in collaboration with the Training Division; facilitates or co-leads training as needed to support program quality and staff growth.

Program Quality, Compliance and Reporting

Financial Duties
In collaboration with the Business Development Director, regularly examines metrics that drive business operations and makes changes to optimize program efficiency and growth.
Helps ensure long-term financial sustainability by participating in planning efforts related to new funding opportunities, grant applications, and service expansion, in close collaboration with the Grants Team, Business Development, and Finance.

Position Requirements
Current California License as a clinician (LCSW, LMFT, LPCC, PsyD, PhD) and at least 5 years post-licensure.

Summary
