Financial Planner
35 hours per week / $25.00 - $26.00 per hour

Summary: Under direct supervision of the program manager, assist assigned program participants in obtaining financial services and in managing their financial resources based on the psychosocial rehabilitation model of treatment and services.

Work closely and cooperatively with program manager, Accounting, assigned program participants and their Personal Service Coordinators, to develop and administer a money management budget/program that identifies and plans for member's individual financial needs and goals.
Provide active oversight for various program features such as interim funding and member loans.
Work actively to troubleshoot and resolve questions and issues when member financial changes occur
Consult and cooperate closely with Accounting to communicate the full range of information necessary for the bookkeeper to manage and maintain all program participant accounts in an accurate and timely manner
Provide a full range of representative payee services for members who request it, or for whom it is requested by the case management team or required by an outside funding agency such as Social Security
Provide money management skills training to members.
As appropriate, actively assist money management participants in moving through the established program steps and toward graduation from the program.
Upon graduation research, solicit, and coordinate banking or check cashing services for members with a local bank or local check cashing firm
Work closely and cooperatively with the supervisor, bookkeeper, accounting team and PSCs to function as the site's liaison to the Social Security office.
Work as a liaison to coordinate and support members reaching their financial goals by taking into account SSI's rules, case management team desires, and accounting requirements
Work cooperatively with co-workers/other team members to help assure smooth and seamless delivery of comprehensive services to program participants.
Participate in staff and community meetings, and in the day-to-day operations of the program as needed.
Participate actively in the site's training efforts as they relate to money management
Maintain financial charts in compliance with Social Security Audits and provide Payee Reports as needed
Advocate and help resolve monetary disputes with landlords and other payers as appropriate
All required data collection documents please review to support member to submit to Social Security in a timely manner
Work cooperatively with Social Security department to maintain relationship and provide update to accounting/payee representative to assist in maintenance of their benefits
Maintain a billing goal as assigned by supervisor and complete and maintain all records and documentation needed for program accountability and compliance in accordance with MHALA standards and funding source requirement
Maintain a billing goal of 60 billable hours for each month
Provide financial planning groups and teaching of financial literacy to members
Complete the budgets by 15th of each month and review the disbursement sheet with the accounting team
All billing and supporting documentation shall be entered into MHALA's EHR (electronic health record system) within 24 hours from the date of service delivery
Provide employment services including but not limited to adult work experience, work adjustment, employment preparation, job development, job placement and retention services.
Provide collateral supports with include contacting family members and/or significant other with the individual's authorization to discuss how they can assist the person with their employment goal.
Determine member Employment needs, as well as social, functional, financial, an educational/vocational barrier to employment stability.
Identify and present resources available to meet those needs.
Work collaboratively with DMH, DOR and community partners to help members meet their employment goals.
Be an active part of the learning by participating in mental health or related trainings.

Summary: Provide high-quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the Department of Health Services (DHS) Housing for Health (HFH) program. Provide a client-centered approach and excellent customer service, sensitive to the challenges that people experiencing homelessness with a range of medical and behavioral health issues face as they move into and maintain permanent supportive housing.

Provide case management support to a caseload of 40 low-acuity members.
Assist individuals at every stage of the housing stabilization process, including flexible services to meet clients' individual needs for all LACDA/HACLA-linked subsidies. Complete all LACDA/HACLA-related documentation, interviews, and certificate utilization and serve as a point of contact for LACDA/ HACLA subsidies.
Outreach and engagement, including processing referrals from the Department of Health Services (DHS), CES partners, and other HFH partners, establishing rapport with clients, providing a “screening in” philosophy, conducting a face-to-face screening, obtaining consents, linkage to other needed providers and services.
Conduct intake and enrollment activities with eligible clients, including assisting clients with gathering other program eligibility documentation, completing project intake forms, authorizations, and enrolling into ICMS. Assistance with temporary housing until permanent housing can be secured.
Assist at all levels of move-in to permanent housing, including assisting clients with becoming document-ready, assisting with all applications and paperwork, coordinating client move-in, and orienting new tenants to their unit/building.
Assist clients with accessing services and addressing their immediate needs, conduct DHS-approved comprehensive psychosocial assessment, and develop and implement an individualized case management services plan in collaboration with and authorized by the client based on the client's comprehensive assessment and/or reassessment. The service plan shall address the needs identified in the comprehensive assessment and describe the client's goals, steps to reach goals, timeframes for completing goals, and disposition of each goal as it is met or changed.
Ensure clients are linked to and accessing health, mental health, and substance use disorder services as needed, including assisting clients with establishing a medical home and maintaining continuity with their medical home. Assist with maintaining medication and treatment regimens.
Assist clients with obtaining income and/or establishing benefits, with locating and securing employment and volunteer and/or educational activities, assist with basic needs, life skills, community participation, improving and/or maintaining daily independent living skills, social/leisure, personal hygiene skills, budgeting and money management, legal issues and making appropriate referrals to overcome barriers to accessing and obtaining permanent housing, and provide transportation as needed.
Provide housing location services (if scattered site) and educate clients on tenant rights and responsibilities. Educate the client on the appropriate use of crisis intervention services versus 911 emergency calls, etc. Provide eviction prevention counseling; work with property management and HFH partner to help clients resolve issues that threaten their housing stability, including tenant rights and responsibilities.
Assist and support program documentation standards of staff in accordance with MHALA, LACDMH County Contracted Provider standards and funding source requirements, and documents in Electronic Health Records (EHR), including 24-hour turnaround documentation.
Must participate in the On-call Crisis Prevention rotation schedule; ensure that HFH clients are notified of their ability to access the crisis prevention line 24-hour/7 days a week.

Summary
The Lead Case Manager (internally referred to as Lead Care Manager) performs essential functions of care management and care coordination as part of the ECM Program. 988 Chat & Text Counselor (Olympic Suicide Prevention Center) This is a fully remote position. The pay for this position is $24.00 per hour with a $3.00 overnight differential. The schedule is Friday from 11:00am-3:00pm and Saturday and Sunday from 7:00am-3:30pm PST, totaling 20 hours per week.
This role requires a three-week 40-hour per week training at the start of employment.

Perform assessments and approves other documentation as required by funding sources based on the psychosocial rehabilitation model of treatment and services. This position is primarily field-based.
Essential Duties and Responsibilities

Provide license dependent clinical services that may include but not limited to psychosocial assessments, diagnosis, clinical treatment or linkage to mental health services
Provide coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such as grooming and hygiene, coping skills, public transportation and other independent living skills under principles of psychosocial rehabilitation model for treatment and services.
May occasionally assist as part of the team, individuals in locating and obtaining requested services, including maintaining housing, food, health care, substance abuse treatment, benefits and entitlements, etc.
Supporting in completion of clinical reports, letters, documentation, IHSS applications, SSI, DPSS forms and applications to support, mediate and advocate on behalf of individuals in maintaining entitled benefits.
Knowledgeable of the harm reduction philosophy and effective in the practice of motivational interviewing
Responsible for conducting outreach and engagement and assess individuals for eligibility and enrollment into program services
Support in assessment, managing, de-escalating, and intervening in potential crisis situations with members and assessing safety and need for 5150 or supporting in de-escalation.
Provide collateral supports which include contacting family members and/or significant other with the individual's authorization to discuss how they can assist the person with their care/treatment goals
Work collaboratively within the team to provide clinical guidance, such as identifying member needs as necessary and initiate communication within the integrated team to provide the resources to meet those needs
Be an active part of the learning in this project by participating in other trainings mental health or related trainings) that introduce different interventions that could be an effective strategy with this population
Maintains all records and documentation, utilizing various technological resources needed for program compliance and accountability in accordance with MHALA standards and funding source requirements
Interact with members in the community, such as at their residence, hospitals, at appointments, etc., in order to engage them in services and to help them live successfully in the community
Active participation in regular team meetings with pro-active communication around member concerns
Assist and support program documentation standards of staff in accordance with MHALA, LACDMH County Contracted Provider standards and funding source requirements, and documents in Electronic Health record (EHR), including 24 hour turnaround documentation requirement. Maintain a billing average of 4.25 hours per day into the CareLogic System for 85 hours per month and complete all required paperwork associated with members.
Must have a LPS designation or be qualified in a timely manner by the LA County Department of Mental Health to write involuntary holds per section 5150 of the California Welfare and Institutions Code.
Must demonstrate the skills and ability to observe, notice, reflect on and report thoughts, emotions and/or physical sensations experienced when engaged in decision making and interactions with others in the performance of duties and tasks related to the position.
Must meet target billing and productivity goal(s) within 24 hours, 85 hours a month for full time staff.
Must demonstrate a “safety first” mentality and engage in practices that promote a safe, healthful environment for all program participants, staff and visitors and providing the best care, welfare, safety and security for everyone at MHALA
All required data collection documents (as applicable to TAY FSP and OCS program) are submitted on time

Position Requirements:
Master's degree in Social Work (MSW) or Masters in Counseling (MA/MS) AND valid registration with California Board of Behavioral Sciences as ASW/AMFT/APCC required
Minimum of one year experience (including internship) providing services to individuals who have a mental illness and who are homeless preferred
Experience working with Transitional Age Youth and young adults (ages 18-~30), with a strong understanding of the developmental, clinical, and social service needs unique to this population is highly preferred.

Summary
As a Substance Use Counselor, you will provide comprehensive intake assessment and individual, family, and group counseling services to adults with substance use issues and coordinate support services to assist clients with case management needs.
