About the Division of Recovery Services (DRS) MHA's Division of Recovery Services (DRS) supports individuals of all ages who are navigating mental health and substance use challenges. Our youth residential programs serve adolescents involved with the Department of Children and Families (DCF) who are working through behavioral health needs in a safe, structured environment. DRS also provides residential and outreach support for adults in recovery or currently living with addiction, many of whom may also experience co-occurring mental health concerns. Through compassionate, trauma-informed care, we help individuals build coping skills, restore stability, and work toward long-term recovery and wellness. ____________________________________________________________________________________________ Position Summary The Case Manager provides case management to homeless individuals being referred for housing placement in a variety of settings. The case manager is responsible for outreach, assessment, enrollment, and placement, in addition to creating treatment plans with the participant which will have a focus on stabilization and a goal of permanent housing. Perform responsibilities professionally, in accordance with MHA's Core Values (Respect, Integrity, and Compassion), policies, practices, program funding and applicable regulatory agency guidelines. Pay Rate: $20 an hour Schedule: Monday through Friday 9am-5pm (40h), some flexibility required Key Responsibilities Engaging in activities to provide outreach, engagement, and assessment to potential program applicants. Gather necessary supporting documentation, complete intake assessments and enter necessary data into program database. To support the participant across a range of services, the case manager will develop and maintain collaborations and partnerships with entities including but not limited to homeless networks, Continuum of Care, and state and local agencies which serve homeless individuals including drop-in centers. Actions taken to support the implementation of the Individualized Action Plan and to monitor the progress made towards Individualized Action Plan goals. Linking participants to a range of state and community-based services including, but not limited to, mental health; substance use; primary health care; income benefits; housing; legal assistance; education; peer support and job training. Develop crisis and pre-crisis plans with the program supervisor and clinical director to provide intervention and stabilization in the event of a psychiatric crisis. As the participant nears meeting the established goals, the case manager will work with the person and his/her treatment team to develop a discharge plan and identify a projected disenrollment date. Job Specifications Minimum Required * Education / Experience: * High School diploma or equivalent required and 2 years of equivalent related work experience (experience in Child Development, Social Work, Psychology, Human Services, Family Studies, or related field); or * Associate's degree in the field of Psychology, Sociology, Child Development, Social Work, Human Services, Family Studies, or related field (as approved) * Technical Requirements: * Demonstrated proficiency and experience with Windows-based programs and data entry. * Work Schedule: This position is 5 days onsite. Ability to work a regular, full-time schedule. Overtime may be required. * Travel: Occasional travel/business related driving required for all staff, large divisional events, and specified meetings. Staff may choose any mode of transportation (driving, walking, bicycling, carpooling, etc.) to arrive to and depart from the location where attendance is required. About the Division of Supportive Housing (DSH) MHA's Division of Supportive Housing (DSH) provides safe, stable housing and individualized wraparound services for individuals who have experienced chronic homelessness. Many of the people we serve are also navigating challenges related to substance use, incarceration, or trauma. DSH programs are designed to support long-term stability and help individuals successfully transition to independent, permanent housing. Through person-centered care, housing navigation, and community-based supports, we empower each individual to build a foundation for recovery, self-sufficiency, and a better quality of life. ____________________________________________________________________________________________ Position Summary The Case Manager provides trauma-informed, person-centered support to individuals in substance use and/or mental health recovery who have a history of homelessness. This role involves assisting participants as they transition from homelessness into their own studio apartments within a congregate supportive housing site. Once housed, the Case Manager continues to provide ongoing support to help participants achieve their goals-such as stabilizing income, strengthening coping skills, accessing medical care, and pursuing employment. The Case Manager is also responsible for completing documentation accurately and in a timely manner. This position will be based on-site at Chestnut Crossing but will spend a significant amount of time in the community working directly with participants and will regularly travel to MHA's Main Office in Chicopee. Pay Rate: $20 an hour Open Shift: Monday through Friday 9am-5pm (40h) Key Responsibilities Coordinate all documentation required for program entry, including but not limited to: homelessness verification, substance use assessments, eligibility screenings, and obtaining IDs, birth certificates, social security cards, income verifications, etc. Complete initial Adult Comprehensive Assessments, updates, and service plans. Collaborate as part of a multidisciplinary team to develop Individualized Action Plans (IAPs), utilizing participant voice, choice, strengths, and needs. Implement strategies outlined in the IAP and document progress as required. Develop, monitor, and revise safety plans in collaboration with participants and the team. Adjust support based on individual needs. Assist participants with the transition to stable housing. Maintain strong communication with staff at Chestnut Crossing. Support tenants in navigating responsibilities such as apartment maintenance, communication with property management, and understanding housing regulations. Support participants in seeking and maintaining employment, vocational training, or education. Educate and assist participants in accessing public benefits and resources (e.g., Social Security, EAEDC, SNAP, PVTA, Fuel Assistance). Encourage community integration by helping participants access public spaces, support groups, recreational activities, and vocational programs. Promote skill development in areas such as ADLs, money management, transportation, and social participation.
