OVERVIEW: The Adult Case Manager provides targeted case management and community psychiatric support treatment for consumers. The Adult Case Manager is responsible for the assessment and collaborative development of consumer goals as outlined by Evidence Based Practices. In addition, the Adult Case Manager is responsible for the coordination of services and assistance needed to meet identified goals, with the overall objective of rehabilitating the consumer in the community at their highest level of functioning.
ESSENTIAL POSITION RESPONSIBILITIES
:
1. Provides services in order to maintain required productivity/billing standard.
2. Meets deadlines and ensures accuracy of various reports/paperwork, mileage sheets, and timesheets.
3. Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.
4. Submits required progress notes/billing information in a timely manner as per agency, MCO/Medicaid, and COMCARE guidelines and contracts. This includes providing service within established MCO caps for service types. Completion of OTRs is expected with any service overages due to consumer need.
5. Assists consumers in setting recovery goals in utilizing the strengths assessment process and update quarterly.
6. Completes treatment plan using goals identified from strength assessment and update quarterly.
7. Employs recovery strategies in working with consumer using the recovery goal worksheet & other evidence based practice tools.
8. Assists consumers to set and achieve educational/employment goals and independent living.
9. Ensures consumers are able to meet ADL's.
10. Ensures 30% of caseload is referred to Supported Employment.
11. Meets with consumers to assist and provide interventions necessary to meet identified goals and work toward recovery and reintegration.
12. Monitors status of consumers and provides level of personal and other supports consistent with consumer status. Notifies physician and Director &/or Coordinator of case management of change in consumer status.
13. Coordinates other services required to meet identified goals based on the needs of the consumers and congruent with the consumer's level of functioning.
14. Monitors progress of other services on an ongoing basis to evaluate effectiveness of service and providing revision as necessary with documentation on progress notes.
15. Assists consumers to access/maintain all available entitlements including SSI, SSDI, food stamps, medical cards and others.
16. Assists consumers with crisis situations, accessing crisis supports and/or in developing a crisis plan.
17. Attends and participates in individual, group and field supervision as established by EBP's and Program Coordinator/Director.
18. Adheres to all Evidence Based Practice guidelines for Strengths Base Case Management.
19. Works in collaboration with ACM Care Managers for comprehensive care and quality outcomes.

POSITION REQUIREMENTS
:
Applicants must possess a Bachelor's degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education.
Intermediate to Advanced computer literacy required.
Preferred areas include knowledge of community resources, housing alternatives and vocational services; experience working in the community with individuals who have serious and persistent mental illness; ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with consumers, constituents and staff.

POSITON OVERVIEW: The Assistant Property Manager will assist the Director of Property Management and Facilities, the Coordinator of Maintenance, and Property Manager with operations and HUD requirements for all residential properties and of licensing office buildings.
ESSENTIAL ASSISTANT PROPERTY MANAGEMENT RESPONSIBILITES:
Assisting in collecting and processing of rent checks, printing rent statements, assisting in notifications of non-payment of rents, and general tenant notices from the management team.
Assist in scheduling and completing quarterly property and unit inspections, move-in and move-out inspections and pest control inspections. Assist with scheduling required fire inspections following city and state guidelines and any other required inspections per property or program requirements.
Ensure properties are being maintained per HUD NSPIRE standards and submit facilities tickets timely.
Maintains files and records consistent with HUD guidelines while ensuring tenant confidentiality. Maintaining the file check out system with all parties accessing HUD files that are consistent with the HUD EIV policies procedures.
Assisting with key check out/check in system for offices and residential properties. Developing and maintaining the system for the inventory of keys.
Assisting in maintaining wait list for all housing projects. Prequalifying potential residents by collecting proper documentation along with application. Adding potential residents to the waitlist in OneSite software.
Assisting with follow-up for vendor quotes, paperwork & general liability insurance. Providing updates to proper parties involved. Scanning and attaching documents to the facilities database.
Assisting any special projects or duties as assigned.
Will be responsible with oversight for fully managing one HUD property. This will include move-in, move-outs, recertifications, inspections, and handling of any tenant issues with cross training from Property Manager and Director.
Manage TBRA grant and tenants, submitting rent roll monthly, completing or scheduling required HUD inspections at initial certification and annually thereafter. Maintaining files consistent with KHRC and HUD guidelines.

POSITION OVERVIEW: The Director is responsible for accepting and processing qualified referrals, coordinating with prospective residents Case Management and treatment teams with respect to assessing and addressing client needs in the residential environment. The Director is responsible for providing regular supervision to all Residential Coordinators and Housing Team Lead, as well as provide oversight and supervision to Behavioral Health Specialists and Case Managers as needed. The Director will handle employee and consumer complaints/concerns, monitoring consumer general circumstances as it involves housing, and assuring general safety and cleanliness. Provides supervision to Residential Care employees, assures program staff are supported and trained in accordance with policies.

Utilizing the person-centered approach, the Navigator is responsible for the coordination of care, collaborating with NFMH residents, guardians, NFMH Liaisons, CMHCs, MCOs, and KDADS to provide wraparound services for clients who express interest in existing an NFMH. Children's Behavior Interventionist

POSITIONS SUPERVISED: N/A
POSITION OVERVIEW: The Children's Behavior Interventionist (CBI) provides intensive services to identified high risk youth and their families to achieve stability and avoid placement disruption. The CBI works with the youth and their family 5 days per week, 3 hours at a time using the Strengthening Families curriculum and neurostimulator activities to assist youth and family to gain optimal recovery and stabilization. The CBI also assist family members with identifying formal and informal resources to connect with to help with current stressors and sustainability.
ESSENTIAL POSITION RESPONSIBILITIES:
1. Completes 40 hours of KDADS required trainings prior to meeting with first family and annual trainings thereafter.
2. Utilizes the Strengthening Family's curriculum with each family visit and is able to use the curriculum in a way that is relatable to family members.
3. Provides at least 3 neurostimulator activities each day when working with youth and family.
4. Able to implement trauma-informed interventions on each visit.
5. Works with identified youth on developing independence in daily living skills.
6. Builds rapport with caregivers and provides support on each visit.
7. Assesses identified youth's progress every 3 months utilizing the CAFAS.
8. Participates in the planning sessions with the child, family, and any other formal or informal supports identified to develop a Behavioral Plan. Behavioral Plan outlines the child's targeted behaviors, behavioral goals, individualized intervention strategies, crisis prevention, and safety measures.
9. Conducts 90 day reviews to monitor progress toward goals and make revisions as deemed appropriate in conjunction with child and family and any other providers involved with family.
7. Monitoring the safety, well-being, and progress of members of the family and notifying the Family Coordinator of changes in status.
8. Coordinating ancillary and community services to meet identified goals based on parent and child's needs. Ensuring that consistent and appropriate medical, community, employment, and educational services are being provided.
9. Completed progress notes for each child and family encounter within 24 hours of the service.
10. Completes required paperwork as needed and timely such as Release of Information forms and CAFAS assessments.
11. Provides level of ancillary support consistent with the demonstrated needs for all family members.
OTHER POSITION RESPONSIBILITIES:
1. Attending staff meetings and training as required.
2. Clocking in and clocking out for time worked, recording mileage, and other agency-required data entry accurately and on time.
3. Documenting and providing essential information required by the Family Coordinator and Program Manager.
POSITION REQUIREMENTS: The position of CBI requires a non-judgmental attitude and perspective of others, ability to be personable and relatable to person's served, experience with resource acquisition, and the ability to work professionally with internal and external personnel. In addition the position requires good communication skills, both oral and written, and successful experience in working with families or other vulnerable populations.

Certified Parent Peer Specialist

POSITION OVERVIEW: The Certified Parent Peer Specialist provides a specialized service that supports parents with children who have Serious Emotional Disturbance (SED), Substance Use, or co-occurring conditions. This service is provided to support the stabilization of the child and enhance the family's quality of life. The Certified Parent Peer Specialist is required to have lived experience raising a child with SED, Substance Use, or co-occurring conditions. This position is also required to complete the KDADS certification and training process to become a certified Parent Peer Support Specialist.
ESSENTIAL POSITION RESPONSIBILITIES:
1. Completes training and certification process in a timely manner as outlined by supervisor and the training and certification process.
2. 62.5% of clocked in time will be providing direct service.
3. Initiates and maintains a professional and collaborative relationship with Family's Together. Utilizes Families Together as a resource.
4. Meets face-to-face with parents to assist and provide interventions for child to meet identified goals.
5. Meets deadlines and ensures accuracy of all documentation, mileage, and electronic timesheets.
6. Maintains accurate and medically necessary documentation of service provision through progress notes. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals. Concurrent documentation is expected in collaboration with the parents.
7. Progress notes will be completed and signed either the same day of the service or by 9:00 the following business day. Notes for services that are completed on Friday will be completed and signed by the end of that day.
8. Certified Parent Peer Specialist will assist parents with participation, education, and support during times of child's hospitalization, with focus on the transition of treatment from hospitals back home. Parent Peer Support will aid parents in ensuring follow-up care within 3 days after hospitalization, developing transition plans, ensuring all medication information is updated and assessing community safety as appropriate.
9. Certified Parent Peer Specialist will assist parents with problem solving, accessing resources, completing referrals, treatment plan reviews, scheduling to meet identified needs/goals, facilitation and coordination of ancillary services and ensuring follow up with appointments.
10. This position services as a liaison between providers and parents as needed for service coordination and mutual understanding of treatment needs.
11. Participates in the treatment plan process with families to ensure parents are supported and assisting with updates and goal development as needed.
12. Provides access to supports by assisting parents in obtaining access to needed medical, social, educational, employment and other services - including assisting with arranging transportation to needed services.
13. Employs strategies in working with parents using Evidence Based or Best Practice interventions. Ensures family support by increasing the knowledge of their support system about the youth's condition, and advocating on behalf of the client/family.
14. Monitors status of youth and provides level of personal and other supports needed for parents consistent with youth status. Provides referrals to community supports and resources to ensure that needed services are available and accessed such as long-term care, substance abuse services, housing, transportation, employment, personal care, and basic needs.
15. Demonstrates excellent communication with Case Managers and other service providers to maintain a collaborative and strong approach to participation with the treatment team.
16. Assists parents with crisis situations and/or in developing a crisis plan in conjunction with assigned Case Manager. Completes Crisis Communication, Transition In Care Form and any other appropriate communication/contacts during times of crisis. This includes collaboration with external providers involved in consumer cases and COMCARE Crisis as necessary.
17. Provides comprehensive transitional care with parents in conjunction with Case Manager following an in or out-of-school suspension or expulsion including evaluation of behaviors that led to displacement, current services in place, a plan for out of school time, determining if safety plan is needed, and in collaboration with the treatment team and school.
18. Demonstrates exceptional communication and relationships with schools. Attends 504 and IEP meetings with parents. Works with parents to problem solve area's of concern with school and serves as a liaison between school and parents as needed to ensure support, understanding, and needs of youth are being met.
OTHER POSITION REQUIREMENTS:
Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed.

Preferred areas include knowledge of youth and mental health, school resources, community resources, housing alternatives and vocational services; ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with youth, family, constituents and staff. A valid Kansas drivers license and access to personal vehicle required.
