About us: Step by Step, Inc is a non-profit organization that provides supports to over 2000 individuals with intellectual disabilities, autism, mental illness, and substance abuse. Our community-based programs provide a setting where individuals we serve can be independent.

Responsibilities

-The Case Manager- Mental Health Housing position is primarily responsible for the overall management of housing services

-The Case Manager is required to make face to face contact with a minimum of 50 tenants and a maximum of 75 tenants at least once a year and as needed if housing problems arise.

-The Case Manager must respond to calls from Tenant Services Liaison (TSL) within 24 hrs to ascertain concern or need

-The Case Manager will be required to meet with participants and team members in the field to provide service delivery or for quality assurance purposes

- Maintain regular contact with landlord, Property manager, TSL and Supportive Services Case Manager

-Develop and maintain a tracking system for information as required by TSL and Step By Step, Inc Administration.

-Participate in Mediation to Tenant/Landlord Disputes required by program guidelines.

-Must Complete 24 hrs of training within the training year

-Associate degree in Human Services/Social Services, Healthcare, or a related field and 3 years of residential and or supported housing experience preferred or any combination of experience, education or training that would provide the level of knowledge, skill and ability required. -Valid Driver’s License, a Clean Driving Record and no prohibitive criminal offense.

Headway is seeking a dedicated and organized professional to coordinate Case Management mental health services for children and families. Case managers work with families and are responsible for arranging and facilitating services for children with significant mental health symptoms that impact their daily lives.

The Case Manager I, II or III for Adult Mental Health Services, provides case management and clinical services for individuals with mental illness, substance disorders use and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.
QUALIFICATIONS

Case Manager I Qualifications:

* High School diploma with at least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is required• Bachelor's Degree in education, social work, psychology or human services related field is preferred.• Experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is helpful.

* Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment.

Case Manager III Qualifications:

* Bachelor's Degree is required in psychology, counseling, social work or special education.• Registration as a QMHP-A or C is required. Must maintain QMHP status and all required trainings/CEUs.• At least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is required.
* Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment.
