Job Description:
Master's level therapist to provide mental health treatment to adults with Severe and Severe Persistent Mental Illnesses on level III outpatient treatment team. This team is currently operating under a hybrid “remote - on site” model.
Requirements:
Key Responsibilities:
Provide office-based (and/or virtual) individual psychotherapy through the use of evidence-based practices
Provide targeted case management (to eligible clients)
Provide occasional functional-support-services in the community (to eligible clients) when needed
Development of individual treatment plans based on client's individual wellness goals
Experience:
Experience working with individuals with severe and persistent mental illness preferred
Experience with evidence-based practices such as CBT, IMR, DBT, LEAP, and MI preferred
Licensure/Certification:
LICSW or LCMHC preferred

Requirements:
Key Responsibilities:
Responsibilities consist of assessment, diagnostic screening, treatment planning, safety planning, ongoing care, and referrals to community resources with your identified client. Role Overview:
Responsible providing individual, family, and group therapy services in acute care settings for youth and adolescent clients experiencing a range of emotional and behavioral challenges. The Clinical Psychologist will complete psychological screening and assessments across programs. Role Overview:
We are hiring for a Therapist for our Assertive Community Treatment (ACT) and Specialized Treatment Services (STS) Teams.

Essential Duties:
Provide action oriented brief therapy to clients starting services
Provide Evidence Based treatment for a range of mental health and substance use disorders
Provide on-going assessments, crisis intervention and case management to clients and their families
Develop and monitor service plans, treatment progress, and maintains clinical documentation and other reporting data
Coordinate services in a treatment team model with internal as well as external services
Have a strong understanding of how to adapt service delivery to meet the client's needs.
Required:

Role Overview:
As a Behavioral Health Therapist on the Mobile Crisis team, you will serve as an integral member of the service delivery team, providing comprehensive support to individuals experiencing behavioral health crises.
Services are delivered to individuals whose behavioral health needs cannot be safely managed in the community or in less restrictive settings, including those who may require longer-term respite care. Providing on-going crisis assessments, crisis intervention techniques, conduct de-escalation, safety planning, and maintaining clinical records and other reporting data for clients with mild to severe mental health and/or substance use disorders.
Observe clients in a variety of situations; assess mental status of client and formulate treatment plans and goals. Perform case management services as needed to assist clients to meet their goals as stated in their service/treatment plans. Refer clients to supportive services such as vocational, medical, social security, housing authority, etc.
Conduct crisis intervention via telephone and/or face-to-face. Intervene and de-escalate crisis situations with clients. Assess the risk of harm to self or others, create comprehensive safety plans, and arrange needed resources.
Assist clients in improving their well-being and quality of life by training in behavioral skills; educating clients to make informed choices; developing clients coping skills, helping clients to problem solve, and negotiating tasks of daily living.
Document treatment plans, progress reports, group notes, individual notes, case.
management notes, letters, faxes and phone calls. Follow the proper protocol for documentation as set forth by Quality Assurance and to third party payor's requirements that have contracts with SummitStone.

The Crisis Clinician will provide emergency services, de-escalation and crisis intervention for consumers at the Walk-In Center. In this role you will work collaboratively with local Police Departments and other community partners. Initiates involuntary mental health holds (if licensed) when needed to ensure the safety of the consumer. Determines the level of follow-up care needed and facilitates the consumer's direct access to that level of care. This position is on site, in Wheat Ridge, Colorado. This position will work evenings - Noon - 8:00pm, Monday through Friday.
If licensed (LCSW, LPC, LMFT, or LP) clinicians may serve as lead clinician for the shift when no identified supervisor is on duty and may initiate involuntary mental health holds as allowed by statute in order to ensure client safety. The Assertive Community Treatment Team (ACT) provides a high level of outpatient services to clients experiencing Severe and Persistent Mental Illnesses (SPMI) and who have many case management needs. Working primarily in the community, the goal of ACT level of care is to provide “wrap around” support to clients to help avoid inpatient level of care, avoid criminal justice involvement or police contacts, and/or to assist with transition from hospital level of care to the community. ACT clinicians serve as part of an interdisciplinary team that works together to meet the needs of clients within the framework of the CO-ACT Model of care.

Conferred Master's Degree in social work, counseling, psychology, or related field required.

Provide life changing and individualized direct clinical services including individual, family, multi-family, and group therapy for a blended caseload of youth with significant trauma, mental health, and behavioral health needs, which may include youth in need of offense specific treatment. Clinicians typically carry a caseload of 7-9 youth.
Most frequently, for this position, clinical services provided will be for youth placed in one of Shiloh House's residential treatment programs and/or a day treatment program and will occur on site. However, from time-to-time clinicians are asked to support a youth and their family on an outpatient basis that may occur in home or in office.
Develop, implement, maintain, and utilize behavior management techniques with clients to ensure that their clinical needs are met. This may include detailed planning and instruction to other direct care staff members.
Collaborate with residential and education staff to foster a trauma informed and strengths based environment in school and milieu settings where all youth can thrive.
As needed, complete clinical documentation including, but not limited to: assessments, treatment planning, progress reports, discharge summaries, court reports, and notes in a timely manner and in accordance with the guidelines set forth by local and national licensing and accrediting bodies.
As needed, complete case management needs such as remaining in contact with families and members of multidisciplinary teams. This role will allow you to grow your experience in working with a variety of systems and individuals including probation officers, case workers, district liaisons, Guardian ad Litems, and more!
After an introductory period as well as robust training and shadowing, all Clinicians and Clinical Supervisors participate in a Clinical On Call rotation. Clinicians are typically only on call 2-3 times per quarter and are rarely asked to physically respond to facilities. Rather, Clinical On Call needs are addressed virtually with support from other professionals via Shiloh House's layered on call system!

The Children's Crisis Stabilization Unit (CSU) Therapist's purpose is to provide individual, family, and group therapy to children/youth who are admitted to the CSU. Essential Duties/Responsibilities:

* Primary coordinator for discharge planning and transition of care to community resources for each child/youth on caseload

Therapist - Child and Family
Work with YCC families with an average of 4 individuals/household in need of individualized services including community-based assessment and short-term therapy/bridging to higher level of care
Utilize key concepts of recovery including client choice, empowerment; hope, creating meaningful activity, advocacy, and consultation with key supports, to facilitate client centered care that will help individuals achieve their therapeutic goals.
Provide comprehensive assessment, evaluation, crisis intervention and psychotherapy to individuals, families, and groups of clients with trauma, mental health and substance use problems
Administer standardized screening tools to identify clients with behavioral health and/or substance use problems as well as tools to measure progress through treatment. Utilize these tools in the development of care plans. Modify care plans as needs change.
Assist clients with symptom reduction (e.g. insomnia, anxiety, depression, and post-trauma symptoms, stress management, emotional dysregulation)
Use evidence-based models for treatment, including but not limited to CBT, solution-focused therapy, behavioral activation, harm reduction, motivational interviewing etc.
Provide an integrated care approach to assessment and intervention for clients having difficulty managing their health due to multiple conditions/chronic disease.
Work closely and collaboratively within a team to provide support and coordination in addressing social issues and basic needs that are barriers to effective treatment and the client's ability to thrive.
