Job Title: Psychiatrist (PRN) Job Summary: The Adult or Child & Adolescent Psychiatrist will provide psychiatric services across the agency. They will evaluate and diagnose individuals who are experiencing a psychiatric emergent/urgent or routine condition in an outpatient setting, face-to-face, and/or by telehealth. They will determine pharmacological treatment for patients as well as facilitate admitting patients to the hospital or respite as needed. Schedule: PRN as needed Responsibilities: 1. Evaluate and diagnose individuals who are experiencing a psychiatric emergent/urgent or routine condition in an outpatient setting, face-to-face and/or by telehealth. 2. Pharmacological treatment for patients including prescribing medications using InfoScriber, as well as providing follow-up. 3. Facilitate admitting patients to the hospital or respite and consulting with MCRT when needed. 4. Supervise Nurse Practitioner and/or residents as assigned. 5. Coordinate medical aspects of treatment with Nurse, Care Management Supervisor and Care Managers. 6. Participate and provide supervision of multidisciplinary treatment team meetings and provide consultation by phone when needed. 7. Maintain active role in treatment team. 8. Consult with outside Physicians regarding care managed consumers as necessary and assist with communication to the PCP. JOB TITLE: Care Manager, Specialized Continuous Treatment Team SCHEDULE: Monday - Friday 8:00am - 4:30pm JOB SUMMARY: Care Manager, SCTT provides intensive care management services as part of an interdepartmental treatment team to Consumers ages 12 and older diagnosed with Intellectual and/or developmental disability and co-occurring behavioral health diagnosis who display severe dysfunction in daily living. Care Planning & Documentation: Continually assesses Consumers' strengths and needs via rapport building and assessment tools (DLA-IDD; CSSRS; PRAPARE) to collaboratively set SMART goals and support goal achievement. Documents timely individualized care plans, progress notes, and payor authorizations reflective of Consumer goals, progress, and needs. Crisis Prevention & Intervention: Assists with crisis planning to proactively support Consumers in navigating potential crises safely. Responds to urgent/crisis Consumer needs with support from Supervisor, Crisis Services, and Treatment Team by developing plans and interventions. Collaboration & Relationship Building: Engages in daily treatment team meeting, prepared to report on Consumer updates and collaborate effectively with team members. Builds and maintains strong professional relationships with Consumers and their supports, treatment team members, and community partners. Engages in professional development check-ins with Supervisor, prepared for case consultation and professional development topics. Other Responsibilities: Responsible for attending all mandatory in-services, continuing education and annual health requirements. This position is critical to addressing the intersection of mental health and criminal justice, promoting good quality of care, program integrity and system efficiency while protecting the rights and well-being of individuals being served. Key Responsibilities FACT team Provide clinical and administrative oversight of the FACT Care Management team and ensure consumers are seen according to agency standards, program requirements and level of clinical need. Provide support and consultation for Care Managers with consumers experiencing a mental health crisis. Attend and facilitate weekly FACT team meetings and ensure a multidisciplinary approach is utilized. Promptly address team concerns and work collaboratively to reach a resolution. Coordinate prompt assignments of consumers to Care Managers and ensure appropriate contact. Oversee and assist with planning for consumer needs and coordination of resources. Ensure consumers are seen upon Admission and Discharge from hospitalization, Crisis Diversion services and correctional facilities within agency and MCO standards. Incarcerated Youth Project Lead the planning, execution and oversight of screening and assessing incarcerated, Medicaid eligible youth (as defined by Project guidelines) for the purpose of developing individualized release/discharge plans and coordinating post-release Targeted Case Management. Coordinate with BlueCare, Department of Children's Services, local jails and detention services and the Tennessee Department of Corrections to develop referral and service pathways for this project.
