Position Summary: The Vice President of Compliance manages quality, safety, regulatory compliance, risk management at Mental Health Cooperative. They are responsible for the development and effective operation of MHCs' quality & compliance programs. They are accountable for the operations, outcomes, and financial productivity for the offices of compliance, quality & safety, risk management and quality training. They provide overall quality guidance and support to all lines of business to ensure the agency is meeting its quality, accreditation, regulatory and compliance goals. Position Responsibilities: Responsible for the development and effective operation of Quality & Compliance Programs to ensure consumer safety, champion continuous quality improvement, monitor, and control risks, and meet regulatory and compliance requirements. Provides divisional support and leadership in collaboration with operations for all government payor initiatives including, but not limited to, TennCare and all value- based care initiatives. Accountable for maintaining company compliance of all applicable Federal and State laws, regulations, rules, and requirements (including Medicaid) affecting the delivery and payment of health care to the population, including those that prohibit fraud, waste, and abuse (FWA) of health care resources. Responds promptly to regulatory inquiries. Responsible for providing oversight and direction to staff for the offices of quality& safety, compliance, risk management, and quality training. Responsible for overseeing quality & compliance functions to meet full reaccreditation requirements with The Joint Commission. Assures compliance with The Joint Commission and other licensing/quality standards. Advises senior leadership and MHC board of key issues, emerging risks, and risk mitigation and quality activities. Cultivates and maintains an organizational culture that provides a safe, satisfying, and enriching environment where all personnel understand that compliance and quality is everyone's responsibility. Evaluates and analyzes regulations/requirements, interprets significance, and recommends policies to comply with the highest degree of quality. Oversees quality & compliance reviews including all agency audits. Creates meaningful quality metrics & strategies to advance quality effectiveness. Collaborates with department/service area leaders to define and support the achievement of quality goals, consistent with the agency goals. Ensures the most effective quality operation of the agency through service development, process improvement and coordination of quality activities across all lines of business. Executes strategic quality plans to achieve goals or establish effective quality priorities collaboratively across the agency. Other Responsibilities: Responsible for attending all mandatory in-services, continuing education, and annual health requirements Comply with all Corporate Compliance policies, practices, and procedures Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator Will be compliant with the relevant regulations concerning the privacy and security of consumer's protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Perform other duties as assigned. Follow all policies and procedures set by the Agency. Essential Skills and Experience: Master of Business Administration (MBA), Master of Nursing (MSN) or relevant competencies If Master of Business, BSN required Five plus years' experience in quality, knowledge and experience in compliance, operations, The Joint Commission accreditation, quality improvement and fiscal management Excellent communication and leadership skills supervising staff Passion for underserved populations with integrated care needs Quality certification preferred Experience in value-based payment programs preferred. Job Title: Consumer Navigator FLSA Status: Exempt Department: Clinic Reports to: Practice Manager Position Summary The Consumer Navigator plays a critical front-line role in engaging consumers who present onsite at the Metro Nashville clinic, ensuring seamless connection to care, closure of care coordination gaps, and completion of required care management and THL activities. Remain visible and accessible in the lobby to answer consumer and family questions, provide reassurance, and assist with de-escalation when needed. Assist consumers with navigating the clinic by escorting or directing them to appropriate service areas as needed (e.g., pharmacy, front desk, care management, provider offices) to ensure timely access to services. Care Coordination & Gap Closure Review the consumer's chart and THL dashboard to identify gaps in care and to determine whether the care plan is due within 30 days or is past due, and update the care plan accordingly while the consumer is onsite. Determine whether a monthly THL encounter is due and complete the THL encounter when Consumer's Care Manager is not onsite, documenting services in accordance with organizational and payer requirements. Ensure consumers are connected to appropriate services, providers, and supports prior to departure. Proactive Outreach & Access Support During times when onsite consumer support is not required, conduct proactive outreach to schedule clinic appointments for the upcoming week. Focus outreach efforts on consumers who are overdue for care, have missed appointments, or are at risk of disengagement. Support clinic show rates by confirming appointments, addressing barriers to attendance, and reinforcing next steps with consumers. Collaborate with EPS services to enroll for THL/ Level 1 enrollment. Respond to and support IVR calls routed from PSR to clinic and Care Management staff, as needed, ensuring timely follow-up and appropriate triage. Clinic Flow & Collaboration Collaborate closely with Care Management and clinic teams to align outreach and engagement efforts. Prioritize support for Care Management teams with high caseloads and low engagement rates, based on identified needs and data trends. Participate in regular coordination meetings or huddles to adjust priorities and ensure efficient use of Navigator support. JOB TITLE: Peer Support Specialist, Homeless Services (Nashville, TN) SCHEDULE: Monday-Friday, 8:00am-4:30pm SALARY: $19.79 (base pay) with additional incentive for language skills and behavioral health experience. JOB SUMMARY: To provide homeless individuals the support needed to obtain and maintain stable housing while in recovery from a mental illness, provide direct follow up with C's considered as high-utilizers, and connect C's to IHC and other various programs. RESPONSIBILITIES: Work with Consumers to reduce psychiatric symptoms and facilitate their ability to live in the least restrictive setting. Provide Consumer care and support, utilizing a time-limited model of support. Provide linkage and referrals to peer-facilitated groups and supports. Provide follow-up and contact with Consumers upon discharge to ensure discharge plan is working and assist Consumer if discharge plan is not working in order to develop alternative plans. Monitor Consumer's behavior, level of cooperation, and ability to remain safe in the community. Prepare and facilitate 1:1 or group discussions for Consumers experiencing homelessness or at risk of homelessness. Support Consumers in understanding their psychiatric diagnosis, treatment and medication. Re-engage or re-establish contact with Consumers who have not followed through on necessary services. Research and be familiar with available resources and share resources with Consumers and appropriate staff. Respond to Consumers in crisis situations as needed by providing support, collaborating with team members, and referring to crisis as needed. Support Consumers and Care Managers with discharge and treatment planning, including securing housing placements, and other treatment plan goals. Work cohesively with Care Managers in treatment planning and coordination. Ensure timely and appropriate documentation as required by the agency and program leadership. Document using the Homeless Management Information Systems (HMIS) and participate in monthly data entry meetings. Develop and maintain community and MHC inter-departmental relationship through prompt and courteous follow-up and communication. Attend and participate in staff meetings, training opportunities, team meetings, and community meetings as designated by supervisor or Program Manager. Attend monthly supervision meetings with direct supervisor, team lead and/or Program Manager. When appropriate, advocate for consumer needs. Transport consumers as needed. Participate in community coordinated events as appropriate. Attend annual PATH Network meeting. Provide direct collaboration between consumers and the following: Rep. Position Summary: To perform a combination of behavioral health clinical responsibilities, including enrollment assessments to MHC services, and therapy with Consumers. This role will serve enrollment, and therapy needs remotely via telephone and telehealth, and report to MHC locations to conduct in person therapy when needs arise for continuity of care. Position Responsibilities: Perform enrollment process for individuals who have been referred to MHC, evaluating needs for all service lines (Care Management, Medication Management, Therapy). Complete all enrollment assessment documentation to organizational standards. Disposition Consumer to appropriate external resources for other mental health services when ineligible for MHC services. Identify individuals that are in need of crisis services rather than a routine intake. If appropriate, facilitate crisis intervention. Assist in resolving any grievance received from a consumer regarding referral. Provide quick access to care via high responsiveness and proactive engagement to eligible individuals and referral partners. Utilize Intake Committee for all Consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity. Contact Consumer with decision. Engage in therapy with Consumers experiencing serious and persistent mental illness, serious emotional disturbances, substance abuse, suicidality, relationship issues, and other stressors. Utilize evidence based therapeutic modalities to counsel Consumers and their supports, via individual, parent/caregiver, and group sessions, to assist in overcoming challenges, utilizing coping skills, and improving outcomes. Develop and implement treatment plans in collaboration with Consumers based on clinical experience and knowledge. Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other community partners. Evaluate the effectiveness of interventions and Consumers' progress in resolving identified problems and moving towards defined objectives. Refer Consumers or family members to community resources or to specialists as necessary. Meet with families, clinic providers, care managers and other team members to collaborate on Consumer goals and improve their outcomes. Collaborate with other staff members, primary care practice partners including pediatricians, doctors, nurse practitioners, office staff, and referral sources as assigned to perform clinical assessments and develop treatment plans. Meet productivity standards as indicated by department goals.
