Documents patient progress through counseling and interaction through groups. Completes patient psychosocial and an individualized treatment plan within the required time frame. Identifies any clinical/case management needs and works to address those needs. Performs individual, group, and family counseling as required. Performs direct one-on-one patient contact through individual or group counseling. Reports patient abuse, neglect and exploitation as required. Reports patient grievances as required. Educates patient in all aspects of treatment, corresponding health issues and steps to recovery. Obtains Urine Drug Screens and initial patient photo identification. Assists in monitoring all patient activities on center premises. Actively participates in community relations activities as directed and authorized. Ensures the reading and understanding of the Policy and Procedures Manual. Assists in supervising Care Coordinators during initial intakes. Provides supervision and consultation to staff and student interns, as appropriate. Provides patient-centered therapy interventions with an emphasis on patient involvement as an integral part of the treatment team. Utilizes the electronic medical record to document patient screenings, psychosocial assessments, progress, and referral information and completes documentation within the agency's required timelines. Assists in the process of advocacy for patients' access to community resources. Collaboratively develops treatment plans including input from the patient, families, treatment team members and other agencies, as appropriate. Coordinates and communicates with the treatment team to provide the best care to patients. Uses multiple interventions such as group, individual and family therapy to assist patients in meeting their identified goals. Complete thorough comprehensive assessments for adults and children, as scheduled and/or needed. Development and implementation of recovery-focused service plans, which include measurable objectives stated in appropriate terms. Provide evidence-based brief and solution-focused individual and family therapy. Maintain accurate and timely documentation, per agency standards. Provide individualized services in the community, including in their homes and other community settings. Carries a caseload and provides case management services to patients as needed as part of a multidisciplinary team.
