Provides direct psychosocial services to mentally ill patients in the community who are experiencing psychosocial difficulties. Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers. Provides information and consultation to other community agencies and other disciplines, including other services of the Agency. Assists in the overall functioning of the Program. Works under general supervision.
• Responsible for screening telephone referrals. • Performs psychosocial evaluation and assessment of mental health service needs of identified patients and their families through professional knowledge, skills of observation and interviewing. • Develops and implements short-term service plans for patients, in conjunction with other members of the Program. • Provides counseling, care management and appropriate referrals for long term and supplemental treatment. • Prepares case histories and prepares and maintains case records, in accordance with the Program record-keeping mechanism. • Encourages resistant clients to accept mental health services through interventions with clients and/or family members and friends concerned with the client's welfare. • Participates in interdisciplinary team meetings and case conferences of the Program. • Provides referral and provision of information to appropriate long-term mental health services and social services and social services providers, or long-term residential facilities. • Coordinates and follows up on linkages made between clients and other service agencies and mental health providers to ensure continuity of care. • Liaison with, and consultation to, community agencies. • Provides outreach services as part of the Program to mentally ill individuals in the community who are experiencing, or are at risk of, psychosocial difficulties and require mental health intervention in their home. • Serves as resource person to the program and other components of the Agency, when requested, pertaining to psychiatric nursing. • Participates with the team and other appropriate Agency staff in the development and implementation of an in-service training component. • Assists and collaborates with the Program Coordinator in the overall functioning of the service. • Assumes Program Coordinator's functions in his/her absence, as requested. • Participates in community programs and education, as requested. • Contributes to the formulation of clinical and administrative policies and procedures and the preparation of policy and procedure manuals, as required. • May provide clinical supervision for Mental Health Technician, graduate Social Worker students or junior staff Social Workers. • Monitors mental status of Senior Citizens accepted to the program. • For Mobile Crisis - Adult (4711) FLOAT Team Only: Assists other Mobile Crisis Agencies across the five boroughs with 24-hour notice. This means you'll be part of a flexible response team, ready to provide crisis intervention and psychosocial support wherever needed in NYC. This requires adaptability and strong coordination skills. • Participates in special projects and performs other duties as assigned.

Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City's efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.
Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management.
The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. Screen and assess clients for the treatment of co-occurring substance use and MH disorders through application of appropriate tools and methods combined with participation in interdisciplinary team consultations.
Provide individualized intervention for problematic behaviors using techniques such as CBT therapy, crisis intervention, and de-escalation, to prevent and/or manage crisis.
Persistent engagement and continued observation to help assess client's improvements and/or ongoing needs or behavior.
Assist in developing and implementing treatment plans to address behavioral issues.
Advocate and liaise for clients within the criminal legal and/or shelter system and support them throughout the treatment process.
Communicates with client, client caregivers, teachers, and/or families (if applicable). Educates families to support implementation of treatment plans and help them understand their loved one's condition.
Assist clients in securing medical care, entitlement and other community support that promote integrated physical and mental health, safety, wellbeing, and recovery.
Provide or connect clients with employment counseling, vocational rehabilitation, completion of housing applications and placement, and provision of life skills training.
Complete assessments and maintain accurate progress notes in a timely manner using best practices for records retention and document storage of sensitive information.
Collaborate with external stakeholders, such as hospitals, shelters, and primary care coordinators to achieve consistent engagement and positive client outcomes.
Provide services in a strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner.

Communicates with staff comprised of diverse identities, experiences, and professional disciplines in a respectful manner that acknowledges differences, expertise, and experiences to promote collaboration and better client outcomes.

REQUIRED QUALIFICATIONS
Bachelor's Degree required and minimum of two years' experience working with clients who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal justice involvement.
Alternate Requirement: Candidates without a Bachelor's Degree must have 5 years' experience working with clients who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal justice involvement plus a CASAC
Candidates without a CASAC must have 6 years of experience working with clients who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal justice involvement plus a CASAC-T
Other Desired Qualifications & Experience
Experience in a multidisciplinary setting and field-based clinical work that includes knowledge or established relationships with Queens based community organizations and social services.
Strong commitment to social justice and CJA's mission and core values.
Certifications as CASAC, CASAC-T, or other SU or MH certifications preferred (i.e., certified professional counselor, certified alcohol, and drug counselor (ADC) or behavioral health certificate.
Individuals with lived experience in recovery from a mental health condition, substance use disorder, or both, or experience with homelessness or the NYC shelter system, or the criminal legal system, are encouraged to apply.
Ability to support the development and implementation of standard operating procedures; suggesting improvements or modifications as needed.
Fluency in Spanish a plus

Hours & Scheduling Expectations
This is a visible role with direct client-facing responsibilities engaging with clients, staff, and other stakeholders. The successful candidate will possess the required qualifications and skills, and the ability to work in-person 5 days a week.
Hours: Tues-Sat 1-9PM
To provide optimal coverage for client engagement, regularly scheduled workweeks will include weekends and night shifts.
Flex hours will be occasionally required to perform client visits either in their homes, or at community-based locations.
Travel within the 5 boroughs required for client engagement or meetings with community partners.
